Management of Cystic Mass in Right Adnexa

Q: According to my ultrasound report, a cystic area was found in the right adnexal region, measuring about 6.6 cm X 6.0 cm. Please advise me what should I do, except operation.

This is a big sized lesion. Has your doctor taken any biopsy? What is the nature of the fluid in the cyst?

Usually, such cysts need to be operated because there is a risk that they may complicate. The cyst may get twisted or infected.

Sometimes doctors just aspirate out the fluid inside the cyst, shrinking it for the time being. This fluid is then tested for the nature of cells in it. This is very important. Conditions like malignancy need to be ruled out.

As far as non surgical methods are concerned, hormonal pills may be tried. You may talk to your doctor about it. Read here more about non-surgical ways of adnexal cyst management.

    My doctor is not giving me answers! Ultrasound found a 4.4 by 4.7cm retrovesical/right adnexal mixed echogenic mass on my right ovary. My DR. said the CT scan means I need to see a gyno I have pain, bloating, constipation, spotting, severe fatigue, sometimes nausea, feeling full with barely any food. I just need an answer this not knowing is driving me crazy.

    I’ve also had HPV going on 8 years.

      Your doctor is right. You need to see a gyne at the earliest.

      The adnexal mass is likely to be the cause for all your problem. This mass needs to be investigated further.

      You may require a biopsy to be taken from the mass to know its exact nature.

      No, it is not dangerous. However, it’s not normal to have cysts in your ovary. It is an indication of some pathology going on in your body, may be some hormonal disturbance.

      What are your symptoms? Any menstrual disturbance?

    I am a 26 year unmarried lady have done USG of whole abdomen showed Anechoic cystic SOL measuring (5.76 cm x 5.10 cm) is noted in right adnexal region. Right ovary is not seen separately. Doctor suggests operation. I just want to know without operation if there is any medicine to remove this cyst or not?

      Since your cyst is big in size, it’s advisable to remove it by surgery.

      Medicines are unlikely to reduce it, since it’s quite big in size. If you leave it as such, there are chances it may get complicated, like it may get infected or twisted.

    I am 28 years old married lady. I have recently done USG of whole pelvis and abdomen region. It showed hemorrhagic cyst
    measuring (4.2 cm x 3.6 cm) is noted in left adnexal region. Left ovary is not seen separately.
    Doctor already given 2 months femilon tablet. And she has again suggested to continue femilon tablet. I just want to know that if it necessary to continue pills or to discontinue it. I want to conceive. But my query is that if that cyst will create problem to become pregnant.

      It is advisable not to conceive till the hemorrhagic cyst shrinks and becomes normal. Such cyst may interfere with pregnancy. So, get it treated first.

      You may continue with the tablets being given. They may help reducing the cyst. Your doctor may ask you to get another U/S done after a few months to assess the status of the cyst.

    I am a 28 years old lady with a first son of 8 years plus. Trying for my second child but pregnancy seems not forthcoming. Took a pelvic area exam and it read this.
    Enlarged anteverted uterus measuring AP 49mm*LS 89mm. Presence of multiple fibroid noodles in the upper and lower half of the uterus. Presence of a cystic mass in the right adnexa with fluid present in the pouch of Douglas.

    What is exactly wrong with me? I’m afraid, can I still get pregnant?

      Sometimes, due to hormonal disturbances, you may get abnormal growths inside the uterine cavity. This is common.

      However, it’s important to treat them first, before you try to conceive. These fibroid growths and the adnexal mass are likely to interfere with conception.

          Your doctor will first ascertain the exact nature of this cyst. It appears to be from the ovary. After that, you’re likely to be prescribed hormonal pills for some time.

    I AM A 45 YEARS OLD LADY . I Have two child by seazure operation 1st one in the year 1996 and 2nd one is in the year 2001 with ligation accordingly and after that an another operation has done in the year 2010 and cut and remove uterus and left overy . Presently it is notified in USG and others report that there is an CYST in my right overy measuring 8.4 x 5.5 cm. My Doctor has advice me for the operation as early as possible for remove it. NOW I AM AFFAIRED and WANT TO KNOW THAT ;-
    01) IS THERE ANY CHANCES OF CANCER? IF SO HOW MUCH THE %?
    02) IS IT SAFE FOR OPERATION ? CAN I LIVE NORMAL LIFE AFTER THE OPERATION?
    PLS SUGGEST ME THE BEST MEASURE TO RESCUE FROM MY PROBLEM . MY DOCTOR HAS FIXED THE DATE OF OPERATION ON 05-11-2017.

      This is a big sized cyst. Surgical removal would be the best option in this case.

      It’s unlikely to be cancerous.

      You’ll need a period of rest followed by a physiotherapy session to gain back your muscle strength. Also, since you are young, your doctor may consider giving you Hormone Replacement Therapy after the operation for a few years.

    I am 22 years old and unmarried. I have a right adnexa cyst (4.5*4.1)cm by USG report.my doctor suggests that conjugate estracon tablets.I have menstrual disturbance for 3 months ( menstrual off >=110days).I have pains in lower abdomen,allergy,lack of appetite, some others problem after taking tablets (estracon .625mg+ medroxy 10 mg)..I want regular menustrual.what can I do ? How can I prevent from this problems?

      Need to continue with this pill for some time, before you can see its effects. In most cases, this may take 6 to 9 months.

      Are you diabetic? Have your blood sugar been tested?

      Lose weight, in case your obese. Eat healthy most of the time, avoiding fried food, junk food and excess of non vegetarian food stuff. Limit your intake of tea and coffee.

        yes,i am not diabetic and have tested my blood sugar, its normal.CA 125 is 42u/ml. CA19-9 is <2.00 u/ml.hTSH .53 uIU/ml….so many tested are being normal. before 3 days i tested again USG(report is right ovary size 2.8*1.6*2.5cm and cyst in appearence) .I am worried for my menstrual cycle .May I be surgery operation for this as i am unmarried? I have already ended my tablets as pescribe and wait for 20 days.But can it become a big problem after 20 days{14 days remaining} or menstrual cycle continue?how can i reduce size.

          It’s worth giving these pills a try, before thinking about any surgery. In most cases, these pills regularize menses and also make the cyst shrink.

    Hi i have done my surgery 2weeks ago .remove the right ovary with 8cm and left ovary just 3cm.The biopsy result is malignant .So we go for oncologist and fallow the blood test by CA125 and ultrasound.my CA125 is 45.8 and my ultrasound of whole abdomen is normal except the minimal free fluid in right adnexa.So the onco.said i need to undergo the chemo in 6cycles.I refused to it .I hope there is other option about it.please help me.

      would depend upon the exact type of cancer you have. Also, its spread.

    Right Adnexal cyst seen 4.2*4.2cm in size with internal fibrin strand in keeping with hemorrhage. Color doppler show no viscosity.
    Kindly suggest either i go for surgery or possible to cure with treatment

    Hello doctor. I had a 20 cm cyst on the right cyst and it was borderline cancer. A cyto reductive procedure was completed and with the cyst al other organs were taken out. Biopsy report says there was no cancer in other organs. Today I did an ultrasound and there is a cystic lesion of approx 3.2×2.4×2.6 cm with vol 11cc. I will go to my gynae later in the week.

    Just wanted your opinion if this is concerning. Because it had a surgery in May this year. Do you think I would need to be operated again.

    halo doctor ,
    I have a cyst measuring 4.5*1.3 cm abutting the right ovary found by a us scan 2 dys before. my doctor said to me to wait 3 months for observation. I am 40 years old and mother of 2 childrens. by the by, my uterus removed 3 years ago because of uterine fibroid. is there any surgery need to remove the cyst or can it solve by medicines, please advise me.

      Your cyst appears quite big, and may require surgical excision. You may observe for some time and give it a chance to regress on its own.

          Yours is a big sized one. So, surgical intervention would be effective. However, treatment with hormonal pills may be attempted.

    My mom is 40 years old and have two children.
    First kid: 25 years old
    Second kid: 20 years old
    and she had a lot of pain in her lower belly area. So the doctor consult her to do some blood test and ultrasound. There is a cystic lesion measuring about 4.0×2.0 seen in the left tubo-ovarian region. Internal echoes are seen within the cyst. She doesn’t have any menstrual problem.
    TSH result is 2.19mIU/L and she has mild hepatomegaly with fatty changes in her liver.
    So what should we do now? Will it need any surgery or it can be cure by medicine? Is it cancerous?

      This is a big sized cyst. Unlikely to resolve on its own. You’ll need to get it treated.

      Surgical removal may be suggested.

    I am 53 years old, no period for the past 8 years, never been pregnant, pain on my abdomen both sides, and pain on my back, more frequent unrination, experiencing gassiness,no dischage or bleeding, no vomitting or nausea. I went in for an ultrasound becasue I felt hardness mass on my left lower abdomen and experiencing pain cramps, and back pain. I am not on any medications or hormones. Would this surgically be removed? Are there any herbal remedies?

      The usual treatment done in such cases in surgery, where they remove the affected ovary and adjoining tissue. Medical treatment with hormones is unlikely to help much.

        Would you recommend a hysterectomy because of high risk of reoccurence on the other ovary? Also tests CA-19 and CA125 are high.

          Hysterectomy is usually recommended in such cases, particularly in any in your family from your mother’s side happens to have any history of malignancy.

    I am a 51 year old woman that has a hysterectomy at 38. I only have one ovary left. I had a physical that shows I appear to be through menopause but had a moderate amount of blood in my urine. I had a CT scan of my abdomen and pelvis that showed a low-density mass in the left adnexa having density measurements close to waterr that measured 4.7 cm in axial dimension, and a satellite cystic mass measuring 2.2 cm. The 2 are separated by an apparent thin septum. What would you recommend as next steps?
