Q: What is the non-surgical treatment for right adnexal complex cystic mass of 65 x 60 mm with surrounding adhesions?

It would be necessary to first determine the histopathological nature of the adnexal mass. Mostly such masses are benign, but the rare chance of malignancy should be ruled out. 

Such chances are more if the patient is perimenopausal.

Medically speaking, we may try using oral contraceptive pills as a non- surgical way of treating the mass. A regular follow up of the patient would be required

Surgical Intervention May Still Be Required If-

  • The mass grows in size
  • Turns hemorrhagic
  • Malignant transformation of the mass occurs (though rare)

    Hello Dr. I am diagnosed with left adnexa solid lesion mass of 5*4.2*4.3 cm with two small cysts measuring 2.8*2.4cm within it. I am unmarried and 23 years old. My symptoms were lower abdominal pain and frequent urination. My periods are normal. Please suggest me is surgery required or not.

      Medical treatment may be tried first. Surgery is not essential.

      You may talk to your gyne regarding this.

    Hello Dr. I am diagnosed with left adnexa solid lesion mass of 2.9*2.6*2.4 cm ,left ovary is not visible clearly. I am married and 30 years old. No Kids yet.My symptoms are lower abdominal pain,fever, vomiting. My periods are not normal , it goes for month bleeding If I don’t take oral contraceptive pills(Novelon).. Please suggest me is surgery required or not?
    doctor has given me antibiotics started from May 18.
    On May 18, 2017, my lesion mass size was 5.1*4.5 cm.
    Now it is reduced to 2.9*2.6*2.4 cm.

      Your adnexal lesion was too big. However, it seems to be responding to pills, as it is reducing in size.

      Since how long have you been taking the pill? Your age?

        Hi, my age is 30.
        I started taking pill and antibiotics from 18th may.
        My symptoms were dirty black color discharge from vagina.. 1 month 20days continuous menstrual period.
        Fever, vomiting. Irritation.
        Then i went to doc, she asked for ultrasound and adnexal lesion comes on left ovary.. Some free fluid too.
        She gave me oral contraceptive pills and antibiotics of higher strength , then asked me to come after 21 days with another ultrasound report.
        In TVS, on 5th june 2017 my size of adnexal lesion come as 3.1×2.4×2.6.
        Doctor changed my medicine to low strength antibiotics and asked me to come after 3weeks with TVS report.
        In between my badluck that i had tooth removal operation, i skipped medicine for 4 days.
        But i continued oral contraceptive pill.
        I went for TVS ultrasound and size came us 2.9×2.6×2.4cm.
        Doctor changed medicine to very low strength antibiotics on 6th july 2017
        Doxylab 100mg.
        Intrasys 100mg
        Enzomac
        Novelon.
        Now I got my periods on 4th july and my menstrual bleeding and pain is still there.
        Doctor asked me to wait for 2weeks and then get an ultrasound done.
        I need to get pregnant.
        I got married in feb 2017.
        Please suggest..
        I will be highly grateful.

          You are under the right treatment regimen. Just be patient. Looks like the adnexal mass is reducing in size, indicating that it is responding to medicines being given.

          Treatment may take 3 to 6 months. Then, you’ll start having normal menses and a healthy conception would be possible.

            Doctor earlier i have pain in left abdominal part, noe from 2-3 days i am experiencing pain in right abdominal area too..
            Is that something i need to worry about.
            Doctoas asked me to wait uptill 2weeks and then get an ultrasound done..
            Should i wait uptill.. Or i should go to visit doc.
            Is lesion mass is something which is spreadable??
            I mean is it spreading to right ovary too?? Kindly put some light doctor.

            The lesion is not spreadable. This may be a referred pain, that is, you have the lesion on the left side but pain in the right due to some common nerves there.

    Hi, please reply, i am worried… I got my dates on 4th july and on 6th july doctor changed my medicines.. Problem now is i am still bleeding and lower abdominal pain is there…
    Doctor said on that day to come after 2weeks with ultrasound report..
    I was fine with earlier antibiotics she gave.. But after she gave lower strength antibiotics on 6th july.. My problem came again..

    My doctor ask me to go for pelvic scan and it read like this
    Normal sized,non-gravid and anteflexed uterus measuring about 5.1*3.5cm in length and Ap diameter with homogeneous myometrium echopattern noted
    Noted at the left adñexa extending to the right adnexa is an oval systic mass containing SEPTA’s and fairly thick wall measuring about 10.8*5.5cm in diameter
    Pls doctor can this cause infertility and what did you suggest I should do?
    Am 32years and I normally have back pain

    l have done an usg showing cyst in my left andexa measuring 80×52mm. extended into pod.
    i m married having one baby.
    is surgery necessary.

    A cyst of size (2.7×1.5) cm is seen in the right adnexa with thin septation. Left adnexal and pod are free. Myometrial echo texture is homogeneous. I am 27yrs trying to conceive

    Can this cause delay in conception? I saw blood after 6wks 3days. And it came like normal menses. Without any pains or cramps.

    I started menses after 6wks 3days. And it came normal and I have never missed a period since I started menstruation. It came without pain in cramps. And d urine strip test showed negative. No symptoms again

      This cyst is unlikely to interfere with conception. Any other symptoms, like pain lower abdomen, gastric troubles etc?

    good morning sir, please help am 27years old am married since December 2017 and up till now am not pregnant, I do have pain in my left side of my stomach after 10 day of my menstruation, I don’t have kid yet please help me, this is my PELVIC SCAN RESULT

    UTERUS; THE UTERUS IS ANTEVERTED AND MEASURE 4.2CM IN ITS WIDEST AP DIAMETER.
    MYOMETRIUM IS HOMOGENOUS.
    NO GASTATIONAL SAC
    THE ENDOMETRIAL CAVITY IS INTACT.
    AN OVAL THIN WALL SONOLUCENT CYST IS NOTED ON THE RIGHT ADNEXIUM, IT MEASURES 2.21CM × 1.54CM IN SIZE IN KEEPING WITH FOLLICULAR CYST. THE LEFT ADNEXIUM IS FREE.
    NIL FLUID COLLECTIONS AT POD
    URINARY BLADDER; CLEAR WITH URINE CONTENTS.

    IMPRESSION; SIMPLE RIGHT OVARIAN CYST LIKELY PHYSIOLOGIC.

      For report looks fine. No cause evident for not getting pregnant from this report.

      Did you go for a hormonal assay? If yes, can you share the report here.

    I am 53 years old, no period for the past 8 years, never been pregnant, pain on my abdomen both sides, and pain on my back, more frequent unrination, experiencing gassiness,no dischage or bleeding, no vomitting or nausea. I went in for an ultrasound becasue I felt hardness mass on my left lower abdomen and experiencing pain cramps, and back pain. I am not on any medications or hormones. Would this surgically be removed? Are there any herbal remedies?
    Can you give me a summary and recommendations?

    adnexa. This mass does demonstrate some solid peripheral
    nodular components that demonstrate color flow. Cystic
    components contains some dot dash internal echoes..

    Gynecologic evaluation is needed.
