Causes of Anal Leakage

If you are dealing with the problem of leaking anus, then you need to ask this question. What type of fluid is leaking from there? This is biggest hint about the cause of leakage.

Types of Leaking Fluids

Mucoid/ Clear Fluid
It could be a transparent looking, slimy, mucoid discharge, coming out of the bowels. This usually indicates that there is some inflammation in the lower bowels. A physical exam with or without colonoscopy may reveal the cause.

Purulent
That is, pus containing discharge. This is likely to be due to some infection down there. There may be evident lesions, as are common in tuberculosis or some STDs. Or else, there may be an anal/ perianal abscess.

Watery
There may be a tear or a fistula around the anal opening, that is oozing out the clear liquid. Or, there may be an abscess that’s leaking fluid. You need to get examined.

Blood
The fluid may be blood stained or you may get frank blood coming out of the anus. The commonest cause for such a situation is Piles. Other causes include severe infections of the lower gut. Proper evaluation is needed to investigate the precise cause.

Fecal
A fecal discharge coming out of the anal opening may suggest incontinence of the anal sphincter. This means that the muscular orifice controlling the anal opening is not tight enough due to some reason.

Fecal discharge may also be seen in cases where there develops a tunnel between the anal canal and outside skin. This is medically termed as an anal fistula.

Fatty discharge
Some may get a greasy or fatty discharge from there. This happens when your stools contain excess of fats in them.

There can be two reasons for this, either you are consuming too much of fats, or your intestines are unable to assimilate the fat present in your diet.

Symptoms of Rectal Discharge

  • Staining of undergarments
  • Pain or ache while defecation
  • Feeling of wetness down there
  • Anal itching
  • Frequent urge to go to washroom
  • Foul smell from groin area
  • Redness or swelling around the anus

On the basis of your symptoms and the type of fluid leaking out, your doctor may proceed with investigating you. Some conditions where leaking anus is commonly seen are discussed below.

Common Causes

Inflammatory Bowel Syndrome

IBS is characterized by inflammation and irritation of the distal bowels. A clear or yellowish white discharge may come out. This is usually mucoid in consistency.

It feels slimy to touch. The patient may often perceive as some wetness down there. The  yellow color may be due to the bile pigments from the gut, or due to fecal contamination.

The pH of the fluid may be acidic sometimes, leading to itchiness and redness around the anal opening.

Proctitis

Certain infections affecting the distal anal canal may lead to leaking of inflammatory fluid from the anal opening. The fluid may be pus like, as in tubercular infection, or clear mucoid.

STDs may also affect this area. This is specifically common in people who practice anal sex. Lesions may be seen in or around the anus on examination.

There may be ulcers, rashes or overt growths. Itchiness may be there. A feeling or dampness stays in the groin area. There is a discomfort in movements.

The underwear may be blood stained or sometimes pus may be seen. Bodily symptoms like pain in lower abdomen or fever may be present.

Anal Warts

Warts are open growths in the region caused by certain viruses. They are usually sexually transmitted.

Perianal Abscess

Abscess is a localized collection of pus any where in the body. It commonly occurs near or around the anal opening, since this area is exposed to plenty of germs. Abscess in this location is called a perianal abscess.

Malignancy

A serous discharge may be seen in malignant growths. If infected, the exudate may be pus like.

Such growths are usually painless, irregular and attached to the underlying skin.

Piles or Hemorrhoids

A fishy smell in your anal discharge may indicate hemorrhoids. A careful examination by a doctor is required.

Blood coming out of the opening is, by far, mostly due to piles. However, since malignancy is another cause of getting blood there, it’s essential to evaluate.

Keriorrhea

This is a condition where orange to transparent colored greasy stuff leaks out from your bottom. The discharge may be as little as a tea spoon full, to larger than a cup, that needs to be evacuated in the commode.

This happens when you consume certain fish oils that are indigestible. The oils pass out from your anus as it is, without being used by the body.

Fecal Incontinence

Leakage or involuntary passing out of the stools  from the anus is called bowel incontinence or fecal leakage.

This ranges from slight fecal leakage to passing out of the whole stools. There is a foul smell from the discharge. The skin around the opening in constantly wet, and may itch or rot.

Causes of Incontinence

  • Chronic diarrhea/ constipation, which leads to irritation of the anal orifice
  • Denervation, that is, injury to the nerve controlling the anal orifice. This may happen after some surgical procedure for hemorrhoids or childbirth.
  • Medical conditions involving the nerves, as diabetes or multiple sclerosis
  • Muscle problem, sphincter loosing
  • After parturition, especially when forceps have been used or episiotomy done.

Management

For all cases of fluid leaking from your bottoms, it is suggested that you see a doctor for proper evaluation about the cause behind it.

Meanwhile, keeping the area dry down there is essential. This is to prevent maceration of the skin there, and subsequent chances of any infections. Butterfly body pads or liners are available in the market. They may be used to keep the region clean.

I have pimple like bumps around the rim of my anus that appear sometimes. That is to say that sometimes they are there and sometimes they are not. When they do appear I have only felt them and not actually had a visual because of awkwardness of viewing that area. I have squeezed them and what comes out is similar to a regular pimple of the face. When squeezing it I use two fingers from one hand and it is necessary to kind of get under it in order for it to discharge. Of course it has a foul smell but not unexpected for that area, has a black/brown smaller head to it and the remainder is a white discharge, however it’s unlike what one would get from the face/nose, the long skinny snake/worm like that continues to grow as long as pressure applied. I have had these off and on for years. They have never caused any pain or discomfort. Any ideas?

We have lubricating glands over that area. Appear a few of your glands are getting infected repeatedly.

Keep good hygiene down there. Try washing the area with plain water liberally after defecation, instead of just wiping it with tissue .This should make a difference.

Hi, I’m a 25 year old female. Last night I went number two and when I wiped I noticed there was a discharge on the paper. It looked very white and thick. I immediately thought it was a vaginal discharge and considered that I might have a yeast infection but upon flushing I noticed that it was in my stool. I am having a little cramping but that could be because of my time of the month approaching. This has never happened before. I have no known medical problems. I eat very healthy and there is no other symptoms (itching, burning, bumps ect) I have engaged in anal sex in the past but it has been about a week since the last time.
        Should I be concerned?

Likely to be blood mixed in mucous. You may observe for a few days. If it’s one time, nothing to worry. However, if it recurs, it would require investigations.

I have had a clear snot looking thing around my bum rafter going to the loo (for 24 hours )(it is when I wipe ) ( I started my periods 2 months ago so I don’t know if it’s to do with this )I’m a bit overweight and I don’t have the healthiest diet either!
I’m really worried and don’t want to ask my dad and I’m embarresd to go to the docs !

If it’s one time, that’s normal. You may get excess mucous from your anal canal once in a while. Happens more if you’re taking spicy meal, alcohol excess fat or junk food.

However, if you get that repeatedly, let us know.

        2. Shannon c.

I had the same mucus in my stool with constipation and diarrhoea on and off for years. Then I had sharp pains under the right upper abdomen, and started bloating. Doctor’s kept saying stop eating so much junk. Seemed to happen hours after I ate and I was ignored…after a few years, I then rapidly lost 40lbs… I started passing blood (in middle of stool). Total time now, about 9-10 years. I was 29 at this point. Finally a doctor said you need to see a GI doctor. They took a tissue sample of my intestines, which were inflamed, had colitis, and looked very red/irritated. The diagnosis? Celiacs disease. At 29, I now had celiacs. Mucus in your stool more than a few times is significant. It’s the sign your intestines are inflamed. Doctor’s ignored me for just about 10 years. A simple blood test would have determined what I had. My results were so extreme (I get itchy from playdoh which has gluten) and over 100, which positive is anything over ten. If you feel something is wrong, you need to be your own health advocate. Most doctors don’t listen or care like they should!

Nothing to be concerned, unless you have any gastric problems.

This is likely to be due to constipation. As you relief your constipation, it would get better.

I periodically have anal itching and irritation from a discharge/moistness. My anus gets moist and has a foul odor. There is a slight dark colored streak in my underwear but it’s not fecal matter. I use desitin when it happens to help with the soreness and irritation. It happens every few months. When i wipe a few minutes later it’s moist again. I eat healthy, no anal sex, regular bowel mvmts.

Looks like a gastric upset. You may have had some food that was mildly infected.

Eat easily digestible meals now. Avoid dairy for 2 to 3 days. The infection looks self limiting. It’s likely to get flushed out by itself in 2 to 3 days.

I often however have been finding myself constipated on and off for a few months. I was treated for gonorrhea abut 6 months ago if that plays any factor. I however do not have anal sex. The moisture doesn’t cause any pain or irritation it is really just bothersome.

Try keeping your bowels clean. You shouldn’t be constipated. You may use any over the counter whole fiber supplement for this (like metamucil).

No, it’s not worrisome. It’s your bowel’s response to the inflammatory process going on in there, probably due to MS.

Hi, I have been experiencing an irritated perianal region for 3 years.
There is a yellowish/clear liquid leaking and irritating the skin causing extreme itch.
I have had a colonoscopy (which was clear of any tumours, they banded two minor internal haemorrhoids) and seen numerous specialists that have all treated the skin with hydrocortisone/ steroid/ antifungal creams.
I personally think the skin condition is a secondary issue caused from something internal but what?
I have just started taking Metamucil to see if that makes any difference.

    Reply
    1. Buddy M.D. Post author

It is very likely that the skin condition is secondary to something inside. Since you have already had a colonoscopy, there’s no point repeating it.

Changes is diet are recommended. Reduce the amount of junk food you take. Limit to fried, fatty stuff, excess of spices, alcohol and smoking. Include plenty of fresh fruits and vegetables in your daily diet. Have wholesome food grains.

Drink plenty of water. Starting with metamucil is a good idea.

As your bowels clean up from top to bottom, your problem is likely to get resolved.

Hi, I’m a 25 year old man and I’ve been having the same irritable symptoms over the past 3 years. I get a feeling of wetness in my anus (approximately 30-45 mins after defecation). It’s not only a feeling of wetness, but often times extreme itchiness and a burning sensation if I don’t properly wipe enough or sometimes I wipe too much causing the burniness of the recess are smeared in and around the anus.

When I wipe, instead of seeing regular brown/dark stool, it often app are yellowish or slimy. I have changed up my diet as a response by eating less greasy food or foods that are high in saturated fat, and drink Metamucil constantly (2 glasses a day). While the Metamucil helps, I feel that my body has developed a tolerance to the Metamucil as the positive effects have slowly declined. This has been going on for 3 years and I feel miserable feeling like I’ll never be the same again thinking everyday how I’ll be irritable after defecation.

Please advise as to what my next steps should be. I seriously need help. Thank you and God bless.

OK. Let your diet be less is spices, rich in natural fibers from fruits and vegetables. Avoid all food stuffs that increase your troubling symptoms.

For most cases as yours, patients have already observed that certain food stuffs increase gut irritation, like milk, mushrooms, garlic, pungent sauces, sea food etc. This food list may be different for each. Avoiding the irritant food is the best.

Drink well filtered and boiled water only.

You may switch to any other brand like, Benefiber/ Fybogel, serving the same purpose as metamucil.

Instead of wiping down there, consider cleaning with water. Dab dry after adequate cleaning. This would reduce surface irritation there.

Ate salads, pasta and fruit with lots of water everyday for past 3 days but still feel constipated. Now getting pale pink mucus discharge only on incontinence pads with still a sense of fullness. It may be some food allergy. This can be pinpointed by exclusions. It may be wheat, rice, some vegetable, milk or other dairy products.

Leave a food at a time to pinpoint. For example, for a week to 10 days, stop taking wheat altogether. Just take rice preparations for some time. You may take corn, bajra or any any grain, but not wheat. See if this solves the problem.

If not, leave milk and all milk products and observe.

    Dear Doctor Sahib
Some yellow fluid is coming out of anus. On examination by the Doctor no Fistula/Fissure is seen. Some itching is also observed sometime in the anus. Kindly suggest treatment.

    1. Buddy M.D. Post author

Itchy may be due to the fluid itself. Any fluid coming out this way is likely to carry fecal contamination and is an irritant to the skin around the anal orifice.

Just the area down there clean and dry. This would limit your itch problem. Adult diapers may be worn for this.

Reason why you’re getting fluid needs evaluation. There appears to be some inflammation in your lower digestive tract. Have you gone for an examination by a doctor or Colonoscopy? If yes, let us know the reports.

Is that a yellow fluid or blood?

Occasional clear anus discharge, very smelly, like fish. Not sexually active. I thought constipation might be the reason. Substance makes skin itch. On days that this happens I make a butt plug of toilette tissue. I am thinking that maybe a tampon might work.

You need to keep the area dry. Adult diapers or liners may be used. This would help in avoiding irritation to the near by skin area and hence, the irritation.

Not wearing a diaper no way Dr. Using cotton balls and one a good movement happens it goes away. Had a thrombosis hemorrhoid a while ago. Extremely painful. Used cortisone cream and it went away after a long time. But before this, I tried to stick a pin into it. Did not work as the protrusion was like firm rubber. No bleeding, no diarrhea. I have always had constipation. I do take a fluid pill, Hydrocoloizide. Don’t think this is. Correct spelling.

Why do some people experience this.

This may be mucous oozing out of the lower mucosal layers of GI tract.

This may happen when you eat too much of fat, or there is some irritation in the GI tract. Irritation may be if you consume too much of spicy, smoking or have some disease there.

If you have no other problem, apart from the discharge, you may wait for some time. Your bowels are likely to heal up by themselves with time and the discharge would stop coming.

I had an advancement flap surgery 5 weeks ago for a fistula ive had for a year…which seems to have held and worked thus far. This week however, i got food poisoning and have had severe diarrhea and doctor prescribed me medicine to stop going to the toilet so much. Since I’ve had food poisoning, I’ve experienced leakage from my butt, it’s generally clear but mucousy. Im worried that my food poisoning has undone my surgery or i have an infection where my fistula is… should i worry or assume it’s all to do with the effect of food poisoning?

    Reply
    1. Buddy M.D. Post author

Most likely. it’s all to do with your food poisoning. The bacteria causing you this diarrhea has inflamed the inner lining of your bowels, hence, the mucous discharge.

However, if you feel strongly that something is wrong down there, you may go for a check up.

I used to see small spot of blood on tissue but not any more. Recently, I feel like going to toilet but when I sit nothing comes. My job is driving I seat 11 hours a day on my butt. My doctor gave me hemorrhoid tablet to insert it from bottom and cream also inserted did not help. I feel around my anus swollen some times. I always wash my bottom after toilet.

Lately, I’ve been having some clear, watery, and slightly slimy discharge around my anus. It feels slippery when I move my legs, and the fluid is slimy to the touch. Where I’m from, we usually wash with water using a bidet after doing a number 2. Several minutes after washing, it would get slimy again. I don’t think its coming from inside the anus. It’s more on the portion where the skin on my buttcheeks touch.

Your lower bowels, which are the ones that absorb water, are likely to be inflamed/ irritated. They may be producing clear mucous.

This is an inflammatory reaction of the bowel mucosa to some irritant.

You may avoid common irritants, like, excess of spices, alcohol, smoking, preserved food, junk food. Eat more of fresh fruits, vegetables and whole grains.

When I go check up the doctor recommended me to use 2 Medicine 1. is to handle peeing (3x a day) and 2. Is to let my Poo out(night). I use this medicine because I need to go to school.

    I can’t ask the doctor because I’m kinda shy with my situation;

Why does my Anus get wet even if it’s not hot? And when it’s hot It wets a lot and it’s smelly, I can’t even control it,

I sweat a lot in part of my anus rather in my Underarm, feet, hand. I think this will help you to figure my problem ; I sleep 1-6hrs a day, I don’t eat 3x a day, I’m malnourished, not exercising nor warm up in morning.

My bowel habit is regular, twice a day. I had loose motion a few day back, I took medicine and had banana in the diet. After a few days I noticed blood pouring after the stool passes. I include fibers in diet. That improved Bt some time it starts dripping after the stool passes. It starts dripping fast as pressure is applied. Sometime I feel wet at bottom and itching but this is not frequent. I feel bloated sitting on chair but fine while standing or walking. My doctor friend has prescribed me antibiotic and lactic for the sub clinical infection he thought. I taking them but bleeding after the stool pass is matter of worry for me.
It may be piles as it happened few months earlier and became normal after that. Plz suggest.

These medicines should be able to control your sweat. Drink plenty of water and try to stay relaxed. Anxiety may aggravate your symptoms.

Give it time for the medicines to act. Your symptoms would get better.

I’m been experiencing mild wetness in anal region for about a month now. There’s a sensation of leakage from there but, upon examination, no moisture appears on underwear, only a faint stain, suggesting that whatever it is is clear. It must dry up quickly, because there is no lingering moisture, but something is definitely leaking in small amounts, periodically through the day. There is no blood and no pain. Defecation is normal, 1-2 times daily, and I eat healthily (lots of vegetables, fiber, lean meat, low carb, etc.) and exercise regularly. Had this about three years ago, though then it was in conjunction with lower GI pain. Those symptoms cleared up after a colonoscopy, which revealed no problems (doc initially thought it could be colitis or some sort of IBS, but no formal diagnosis was made). Any ideas about what this could be and what can be done about it?

Let’s give it some more time. The problem is likely to subside on its own. Continue with the good eating habits, as you are doing.

May be you had some inflammation in your lower bowels in past years, that gradually resolved. Any inflammation tends to produce mucoid secretions. As the bowel linings are healing, you’ll get less of this discharge.

      Avoid excess spices, alcohol, smoking.

To follow up: left-sided abdominal ache, including numbness in descending colon-sigmoid region, developed over two months ago, while anal seepage continues.
The ache/numbness seems to follow digestive timetable and is similar to sensations felt three years ago that vanished following a colonoscopy (who knew that procedure could clear something up). Difficult to sleep on left side.
Stool is regular (once daily), matches the “ideal” smooth type on the Bristol scale. Tends to be not as big around as normal, though (about diameter of the average thumb, I’d say) but 6-8 inches in length.
Went to a gastroenterologist about six weeks ago who ordered a CT scan. Scan revealed no abnormalities but a 3mm kidney stone on left and “stool throughout colon, indicating possible constipation.” Dr. thinks leakage is related to hemorrhoids, gave me a few sample cream tubes to use, which haven’t helped the past six weeks I have had them. No follow up from the dr. about what to do. Meanwhile, saw urologist about the stone, says too small for lithotripsy, let’s wait a year and check back. Stone occasionally asserts itself. I can feel it, very localized.
Suspicions: the dull ache/numbness is from constipation that won’t break up, and the leakage is some sort of lubricant from the colon that is seeping out, related to the constipation. This is my best self-diagnosis so far. Does it sound plausible or are there other possibilities?
Any ideas about what to do?

The ache and numbness are very likely to be due to constipation. You need to solve this problem. Take a stool softener twice daily. Let us know if you feel any relief in a week.

To follow up: Had a colonoscopy last week, which revealed nothing but a normal, disease-free colon, just as it did three years ago. And yes, in the wake of that procedure, I have felt better.
However, there is still a lingering left-sided dull ache/numbness that comes and goes. I’m trying Metamucil cookies now, though bowel movements are regular, 1-2 times daily, appear normal and emptying feels complete. Anal seepage seems to have stopped.
Perhaps some of this is referred pain coming from the 3 mm kidney stone lodged in the lower pole of left kidney? It is otherwise asymptomatic.
Or is it IBS-C? It seems that age 50 is pretty late in the game to be developing this all the sudden. I just don’t think I have enough of the symptoms to match up with IBS. Certainly no diarrhea, no universal abdominal pain, etc.
Finally, could it be that “left-sided abdominal pain” is a syndrome without an official name, a collection of symptoms that don’t comprise a recognizable malady. I’ve read over so many descriptions of every abdominal condition (from anatomy textbooks to reputable medical websites) and none of them quite match up to this.

          2. Buddy M.D. Post author

left-sided abdominal pain is very likely to be due to the small kidney stone.

Kidney stones may give such vague symptoms. It’s not that the stone is actually pressing upon there, may be it has inflamed the nearby colon area touching it. There may be some localized inflammation there.

Likely that you suffered a mild irritation in your lower GI tract. This may happen after after very spicy food, smoking, alcohol consumption or any other irritant exposure in the gut.

Since the problem is improving, it’s better to give it time to heal by itself. Avoiding irritants in the diet would help. Avoid smoking, alcohol, excess tea, coffee, or caffeinated drinks.

I feel absolutely normal. Normal stools, I feel equally hungary, no pain, blood, itching. It’s just a constant feeling of something there in anus, with a bit of wetness which can’t be seen on undergarments. I have tried cleaning it but i find absolutely nothing. It does give a odor though which is not that much that even a person near me could smell, but I know it’s there. And I don’t know if it matters or not, when I wake up I feel amazing but as the day progresses that feeling intensifies. But as I said before, not much.

    Hello,

I have a multitude of elevated hormone levels all which we believe is due to the pituitary tumor I have. I have elevated cortisol, elevated aldosterone (5x high end of normal – abdominal CT did not pick up on any adrenal abnormalities), elevated testosterone (this fluctuates between normal and high), elevated insulin and elevated catecholamines/metanephrines. I also have an elevated parathyroid level and calcium levels that are high or borderline high (due to the PTH level). I have severe abdominal swelling from fluid retention and am on 20mg of torsemide, 2 pills twice daily one in the AM and one in the PM. I am overweight due to the hypercortisolism but am not diabetic…yet. My question is, can ANY of these hormones cause fecal leakage from the anus? Can the torsemide cause this? I am a 30 year old female. I know hypercortisolism can cause muscle weakness…. but can this actually affect the sphincter muscle as well??

    Thanks so much

Is it some discharge from the anal sphincter, or you see fecal leakage?

If it’s fecal leakage, it’s likely to be related to muscles there. May be due to high level steroids circulating in your body.

Along with this, I have stress incontinence (urine). The hormone levels have caused me to have cyclic vomiting and vomiting when my potassium drops.. so perhaps this is contributing as well?

It’s important to keep the area down there clean and dry. Wetness there may make grounds for infections.

The problem is reversible. Likely to improve as your hormonal levels stabilize.

I’m 23 years old. 3-4wks ago I became constipated and when I went to the bathroom all that came was gas and little bit of mucus. This was the morning after drinking. Since then I have been constipated (I feel like I need to go more but pass only a little) and my stools have been mucusy. Alchohol makes things way worse the mornung after so I stopped drinking. Coffee causes pain and I nesd to go bathroom asap after that. Was having cramps but they are gone now. 1 time there was a little blood in the mucus, but since then none. Doctor put me on a stool softenener, now I have diherria/constipation as of about a day ago so I stopped taking it. I have also had wetness around anus that leaks through everything, you can even see it where I was sitting.. Gross. Worried about colon cancer with all the mucus, what do I do? Is this serious?

It’s important to relieve your constipation first. If you still get blood or mucous down there, further investigations may be done to find out the cause.

Add more fiber to your diet. You may use some fiber supplement like metamucil, citrucel etc. Drink plenty of water. Include fresh fruits and vegetables in all your meals. A brisk walk for 20–30 minutes a day can help stimulate bowel motility.

Avoid tea, coffee, smoking and alcohol. All of them may worsen the condition.

Avoid psyllium if you’re taking any other stool softener.

Constipation is the commonest cause. So, alleviating it is likely to solve your problem. However, if it still persists, you may need further investigations.

    Hello,
I have been constipated for about three months and there is this clear-ish type yellow liquid that comes out. It is kind of sticky. I was afraid I might have something serious. I don’t take any medications so that shouldn’t be a factor. I also feel the constant urge to go quite often although most of the time I actually don’t need to go. Sometimes it hurts after I go but that only last for about 2 minutes. Any ideas?

May be due to chronic constipation.

Constipatoin is not a good condition. Let try to relieve it as fast as possible. This can be done by taking lot of fluids – around 8-9 glasses of water a day. Eat fiber rich food like fruits, vegetables and grains.

Apart from this, you may start adding metamucil or any other soft former in your diet on a daily basis.

Stay physically active throughout the day. This improves gut motility and pushes the food down.

    Hello,
I am a 30 year old female with vulvar eczema. The itch has extended to the backside however, I also have been diagnosed with PCOS and unfortunately I don’t follow a healthy diet due to my work schedule. For the past year or so I have had issues with my bowels. My stools are a soft orange like consistency and are generally small. I feel constipated most of the time and when I wipe it takes almost forever to clean up. When I wake up I have an intense itch and my anus seems to be oily. I tend to release a lot of oily discharge throughout the day and it is embarrassing because I can smell it and I can only imagine that others can smell it too. The smell is so bad that it doesn’t go away even after I shower. Help!

Since you’re having this discharge continuously, we can’t attribute it to any food stuff being taken. You’re likely to be having some GI problem.

You need to visit a gastro enterologist for examination.

Hey doc. I’ve had problems passing still for about a year now. Started with yellow discharge. Now it’s clear and smells fishy. Feel like I’m never fully evacuated and carry a smell and I sweat a lot in that area. I’ve had a colonoscopy done and everything was normal. Was given muscle relaxers to help with comfort but I still have problems with mucus and fully evacuating. I’ve also been using still softeners and citrucel. No one can find the problem and I’m trying to keep it together. I used to be on painkillers that caused constipation but the symptoms listed above didn’t start until I took myself off of them. Help is extremely appreciated

I forgot to mention I have to strain pretty hard to get it out as well. It feels like the mucus makes stool really hard to pass. I’ve been on a healthy diet for over a year now.

Two factors appear to be prominent, one is your constipation and secondly, some inflammation in your lower bowels.

If you’re getting mucous, there has to be some inflammation going on.

It’s important to relieve constipation to rule out one factor completely. You need to continue with a healthy diet, containing plenty of fruits and stewed vegetables. Limit to tea, coffee. Avoid smoking and alcohol. Both these chemicals irritate our body mucosa. Drink plenty of water.

Adding a stool softener is essential. Citrucel/ mineral oil, or any of your choice.

Stay physically active to help your bowels move down.

Let us know if it makes a difference.

This could be a sebaceous cyst there.

Keep the area clean and dry. You may start applying some topical antibiotic cream over it. Mupirocin/ bacitracin may be used thrice daily.

If you feel it’s still there and not drained completely, you may need to see a surgeon for complete drainage.

I’m a 21 yo female and have just recently noticed that I have a white, almost cottage cheese like discharge coming from my anus. I do not involve myself in any anal play during sex but I am a bit concerned. It is also extremely itchy down there (almost like a yeast infection) can this be an ER visit or a regular trip to the doctor? Along with the white discharge, I’ve noticed that there is clear and water discharge that goes along with it. What do you think this is?

This may be yeast. You may need anti fungal therapy for this.

This may be started immediately. Use this ointment every night with the help of an applicator that come along with it.

Which type of doctor should I see for a red, raw, irritated bottom. Also have leaking/discharge. Sometimes it is greasy and happens usually when I pass gas and sometimes it is just clear and wet. Also, have some pain when I wipe after a BM and sometimes the tissue is spotted with blood.

      Gastroenterologist.

You are likely to be having some Intestinal problem, may be inflammation in there. Such conditions cause production of mucous and discharge.

These discharges are irritants and are likely to be eroding and irritating the skin down there.

Keep the area down there clean and dry. Wear diapers or change cotton underwears frequently. Keeping dry there would help. Also, you may apply thick moisturizer creams, or even vaselines, down there around the anal opening. This would give some protection to the skin there and arrest symptoms there.

Hi and thank you for taking the time to answer these questions. About 2 months I had sharp stabbing pains left of belly button prior falling to asleep. Next day I woke up and noticed blood when wiping (not on or in stool or in water, only when wiping). This occurred 3 times that day and never again. Since then I’ve had intermittent RUQ pain and mucus with some bowel movements. The mucus is typically the same shade as the stool, no pus or blood. I don’t see it with every BM, it’s completely random. It’s important to note prior to this I was constipated for 5 years due to opiate therapy (I assumed this was the cause of the blood when I wiped) and now my stools are usually normal consistency (still have occasional constipation). I have good days and bad, sometimes no RUQ pain can be felt all day, sometimes I feel it all day. No rhyme or reason, and strangely enough food often relieves the pain (I’ve been on PPI for 6 weeks thinking it was an ulcer, but ulcer shouldn’t cause lower GI symptoms right?). Still no getting better. In fact, this morning I woke up and noticed a large, watery discharge on my underwear which I guess came out over night. The spot on my underwear was probably the size of a grapefruit. I’ve taken 10 home EZdetect colon disease tests to see if any unseen blood in stool, all negative. My mucus used to have a fishy smell, but over the last week it basically has no smell.

I am a medical professional myself and these symptoms have stumped me. Nothing really adds up here. I know the easy answer is go see a doc but I was really hoping this would eventually pass or I could figure out what the hell is going. Could it be some kind of lingering, localized infection? Running out of ideas here. Praying it isn’t cancer.

    Reply
  55. ConcernedRN

Also, the pain is really not bad. I’d rate it a 1/10 at times maybe a 2/10. It’s just enough to let me know something is there. It’s more of an annoyance than anything. It’s all the way up almost to my xiphoid process on the right side, and occurs over my right rib right next to the xiphoid, so I suppose it’s epigastric and not RUQ. Very puzzling. Also, if I do very deep palpation with 1 finger where I had the original stabbing pain 1 inch left of my belly button, I can find a really tender spot down in there that elicits a great deal of pain. It stays painful for several hours after messing with it too. What the heck could this be?

    Reply
      1. ConcernedRN

Not yet. Have you ever heard of a duodenal ulcer causing excess mucus in the stool and watery discharge from the rectum while sleeping? This along with the RUQ pain has me worried. I’m only 33 so I know the odds of it being cancer are like 1:50,000 but I’m starting to wonder what else could it be? Been having mucus in stool and RUQ pain for 2 months now.

Mucous suggest that there’s some inflammation in your blower bowels. Commonly, this happens in conditions as IBS.

I had some abdominal cramping that I thought was diharrea but when I went to the bathroom I had a yellow/brown mucus coming out of my anus and any stool that I passed afterwards had been solid. This mucus continues to come out of my anus and has a terrible fishy smell. Additionally, some of the jelly like mucus also contains a specks of bright red blood and is brown and yellow the rest of the time. I am not constipated and have a BM 1-2 times daily. I also don’t have any pain in my anal region. The mucus has a bad odor and is very annoying. Really appreciate any advice.

Your lower bowels are likely to be inflamed. This may be due to any of these reasons- infection, excess spices, alcohol, smoking.

You may wait for a day or two. If the problem improves with time, we may assume that your bowels are healing. If not, it’s better to go for a stool culture.

I also see reddish stains on my underwear. I went to see the doctor who gave me the procedure, and he said that is the sign of infection, and he told me not to worry. If it is infection, then when can it be stopped? Is there anything I can do and/or eat, such as medication, to stop infection? Thanks.

Probably one of my last hopes:

This year I’ve been getting tested 8 times because of the same symptoms: Discharge from the anus. What I noticed is that the discharge is always on the beginning of my stool, when it first comes out. That’s the only spot where I can see the purulent discharge. White, yellow, pus like discharge is what it looks like. Everytime I got tested with those symptoms, I always get negative on everything STD related, but honestly… do you have any suggestions on what it might be? I have no itching (maybe sometimes because I’m quite sweaty down there), no blood, no discomfort.

    What I do have:
    Staining of undergarments
Constant feeling of dampness around anus (and when I randomly wipe myself there during the day without any need to going to the toilet I can see the paper turns yellowish… pus, slime, mucous? It also has a typical smell)

    Hope you can help me

My problem is that I have a bad itch on my anus, but it only happens when I fart because i get a yellow-orange slime looking thing come out when I fart. It’s not much but if I fart more than three times it starts to get excessive. This has been going on for about 4 days now and it’s unbearable, sometimes it even wakes me up in my sleep. I also have pimples in the area but I’m not sure if they’re related to that same problem. Im not constipated, in fact my stool has been big and softer than usual lately. Is there anything I could do to stop this? Constantly stepping into the shower to make it go away is quite the nuisance.

Those pimples may be related to the discharge coming out.

This looks like a temporary problem and is likely to be related to your diet. Excess of fish oils usually causes such discharges. Have you recently tried any new food stuff?

For now, just keep the area down there well washed and dry. Apply any body moisturizer after repeated wash to avoid irritation.

I had the Mirena birth control and I was wearing a lot of pads because I was bleeding for about two to three months. I was getting a little itchy and I figured it was because of the pads. I didn’t wear tampons because of the birth control. Turns out I was having a miscarriage. I didn’t think I was pregnant. All signs pointed to no. One night it felt like contractions but I’ve had so many problems with the Mirena and I went to the bathroom and out came a ball of baby. I went to the ER. They couldn’t take out the Mirena. Some kind of law or something. ER doctor yelled at my OBGYN to take it out. He did rudely. He never listened to me. Rude as hell. He took it out. Not happily and told me it wasn’t the mirenas fault for the miscarriage. That was over two years ago and I still have an anal itch. It’s so bad now I can’t help to scratch. Itchy on inside and outside. Feels a little wet. No color to it. I don’t think I smell anything. I can’t take the itch anymore! I’ve even gone as far as putting yeast infection cream up there. I take probiotics. I’ve tried ice. I’ve done hemmoriod medicine. It doesn’t help!

Female,48 dealing with heavy periods lasting two weeks. Then it goes straight to copious amounts of watery vaginal discharge. Naturally I am constantly moist. I have been noticing a fishy odor. It took several months to realize it was coming from the anus. I have come to realize that after a bowel movement, I will wipe myself clean, only to find that hours later, I need to wipe again. It’s as if I never finished cleaning up, but I did. It is not enough that it has soiled my underwear, but enough for another couple of wipes clean. This can happen several times during the day, after a morning BM. I have been to the doctor about my periods. I told him about the fecal incontinence, and he is perplexed.
Any ideas? I’m not sure if my female problems are related to this or not. However they do coincide.
    Your opinion would be appreciated.

      May be connected.

Let’s first investigate the cause of your heavy bleeds. Was any scan done. If yes, let us know the report.

It may be that you have some growth in your uterus that’s giving you the bleed. This growth may be pushing the lower bowels, irritating them, and leading to anal secretions.

Yes. Had scans and endometrial biopsy. Three fibroids. 3 cm, 4 cm and 5 cm!
The uterus is retroverted to begin with. Now it’s full of fibroids.
I am trying progesterone to see if that cuts down on the heavy periods and discharge.

If it were due to the fibroids, wouldn’t it happen all the time? Why only AFTER a BM? It’s as if the bowel is not totally emptying??
It’s only a smudge, for lack of a better word, it is not like solid matter. It’s as if I didn’t finish totally wiping.
          Help!!!

Fishy odour from the anal discharge may also suggest it’s blood mixed.

You need to be examined down there by a surgeon for diagnosis.

  78. Boxergirl

Thought it was important to add that the vaginal discharge has been tested for infection. All cells are normal. No obvious medical reason for the copious amounts. The dr is thinking it is from my whacky hormones from perimenopause.
The incontinence(if that’s what it is) only happens hours AFTER a BM, not before having a BM.
Could the constant moisture down there cause the anal sphincter to weaken?? Why is this happening?!!!

For a while, my stools have been painful and there is a dark brown discharge noticable on toilet paper while wiping. I can feel it coming out and it takes forever to wipe, because it keeps coming. It doesn’t really look like blood, but it’s wet and dark. After more serious episodes, my bowels hurt for days after defecating.

I have a history of constipation and abdominal pain, and I have had episodes of intense bowel pain since I was 20 (the pain was so strong and seizure-like, but I had no idea what caused it at the time). However, this discharge started appearing only a few years ago. I am 39 now.

What could it be? Thank you very much!

It may be a slimy mucoid secretion contaminated with feces. It’s common to see it in patients with chronic constipation.

You just need to correct your dietary lifestyle, and hence, your constipation. Add more fiber to your diet. This means, plenty of fresh fruits and vegetables. Drink plenty of water. Keep yourself physically fit. A brisk walk for 20–30 minutes a day can help stimulate bowel motility.

If required, start taking a bower softener, like metamucil, on a regular basis.

Anal leakage – Sorry for long post but feel you need background for this. I had double dose RAI for severe, long undiagnosed Graves disease, thyroid storm – ended up severely hypo (TSH 73 while on Levoxyl. Affected digestive tract motility badly, caused terrible constipation . Also stomach acid production was nil. Developed mucosal rectal prolapse. Went to colorectal surgeon. Did exam and said to go to rectal pelvic PT to strengthen sphincter as prolapse not that bad and still have pretty good muscle tone. (Obviously to me it is not as good as it was).. Few days after exam noticed clear mucous discharge from anus (non irritating to surrounding skin but irritating to atrophied vaginal and urethral area when lying down). Went to PT and therapist insisted doing vaginal exam to check sphincter muscle. Said not too bad. Few days later developed yellowy vaginal discharge (think had some anal discharge in area that got pushed up into vagina with exam). Also anal discharge became yellowy and no longer clear. It has a very acrid sharp smell, not like feces and does not irritate outside surrounding skin of anus and no itching/redness. Is this normal with a mucosal rectal prolapse? Also could I now have a vaginal infection? Have IBS and mild Crohns/colitis and am 80 yrs old. Have only been on low dose Librium/Librax since 2002 as allergic to Crohns meds. Hypo numbers must be getting better as no longer horribly constipated and depending on what I eat actually go the other way which irritates the prolapse. It looks like a normal, colon emptying BM in bowl but so messy on me that I can use up almost a whole tub of baby wipes and still not be clean and have to get under the hand held shower to clean up. When I get washed at the anal opening it feels to me like it is kinda slimy on the side of the opening. I presume the prolapse is taking up part of the anal opening and it is open towards the side.. At my age I can not see it by using a mirror. Also flatulence is better – Endo said that is normal as the digestive tract can be extremely affected by the hypothyroidism and it may take quite a while to get the med to the correct amount so I am euthyroid. I also lost a lot of weight although eating like a horse and was only 112 lbs to start with. It seemed the more I ate, the more I would lose. I have gained a few lbs back but would like to get back up to 112.

Afraid to go to GYN as afraid leakage from anus (which seems intermittent) may go to front area and again be pushed into vagina. Any suggestions how to prevent contamination? Anything I put against anus irritates prolapse which does go in and out as it pleases ( walking, bending, sitting, anything, Also don’t know when in or out so it gets wiped and washed in the shower sometimes so does get irritated from that.. I guess that could cause excess mucous but not sure why the color change although I have read that yellow discharge can accompany IBS. My IBS fluctuates between some constipation and messy stool (formed and soft in bowl, messy on me. Cannot do Metamucil, etc. as I have bent esophagus and trachea at collarbone area from severe scoliosis and some trouble swallowing. Had aspiration pneumonia 2x. Before Graves high fiber was never my friend anyway.but I was able to manage my digestive issues quite well until Graves. When I was hyper I was eating all kinds of fruits, veggies, fiber, etc. and now looks like I can’t tolerate that type diet anymore. I was even able to stop my Prevacid and Zantac as I had no more acid reflux. Still don’t so presume my stomach is not producing acid as yet.

Even scared to go to surgeon as couldn’t stand another thing shoved up there.

Cant do colonoscopy as almost died from hemorrhaging in 2001 from explosive “D” (which started up the crohns/colitis)and former retired GI doc said any explosive “D” would start it up again and probably cause bleeding like that again and now cant take steroids.. New GI doc has bugged me about a colonoscopy for last 3 years. Also I have been told over and over about my “very thin walls, esp. between the vagina and rectum” from very strong steroids for a year in 2001 and then cortenema and protocort (then had bad allergic reaction).

Also I am allergic to most meds now including the steroids, all antibiotics except Cipro and Keflex families, etc. Should I begin with going for a urine culture? I do have some lower abdominal cramping lately and if I have a UTI it is most likely from the vaginal discharge so would Levaquin take care of the vagina as well as the urinary tract? Keflex does not work on my UTIs. I am taking Culturelle without the inulin and Phillips Colon Health probiotics. Is there a better one for me to take as my docs do not seem to know much about probiotics at all. I also drink 2 Ensure/day as well as a multivitamin

Thank you for your help and advice.

Since you’ve not mentioned any vaginal symptoms, it’s likely that there’s no infection there. Need not worry.

Anal discharge is due to the mucosal prolapse of your rectal musculature. This is normal and essential is such cases. The discharge is protective for the prolapsed layers of your gut.

Surgical options may be considered to correct it. You may also try Kegels exercises to strengthen the pelvic musculature. This would help reducing the degree of prolapse.

It’s better to avoid the anal discharge touching your vaginal area. Consider wearing a thin panty liner to minimize it.

If you’re eating so well, and still losing weight, it may be your thyroid again that needs to be checked up. You may be in a Hyper state.

Continue taking the multivitamin and the probiotic supplement. They’re good.

Thank you for your answer. However, what do you mean about vaginal symptoms – what would they be??

I do have some low abdominal cramping and the yellowy discharge and although I am always a bit sore from the vulvitis and vaginitis I do seem a bit more irritated in that area, perhaps from the vaginal discharge itself. Also I realize the low abdominal cramping could be coming from the colon as with the iBS/IBD I do get cramping there especially if I have a soft BM. The heating pad at night is becoming my new best friend.

I am not losing weight at the moment, and did gain a couple back but nowhere near 112 lbs.

Do you think it advisable to at least get a urine culture to make sure I do not have a UTI??

If you have any itch or pain there, it may suggest some local inflammation. Cause needs to be investigated. May be some infection.

Hello. I was recently diagnosed with H. Pylori. I just finished all my antibiotics and waiting a further appointment to confirm bacteria is gone. I had intercourse with my husband three days ago and normally I have slight discharge from vagina a day later. I wiped and notice small discharge after bowel movement the next day and shrugged off as vaginal discharge. Fast forward to today and I had another bowel movement and when I wiped I had a large amount of jelly, slimy, clear mucus coming from my anus. I have never engaged in anal. I have only had one partner (my husband) and I am currently 27yrs. This hasn’t happened before and I haven’t noticed any mucus in my stool. I will note my stool has been changing in colors. Sometimes dark brown sometimes light brown. Most of the time it is soft stool and I have bowel movements twice a day. Thank you for your time.

        Update…
tried to go to my dr because my side started hurting extremely. They couldn’t fit me in and said go to ER. Testing showed I had sludge around my gallbladder. And possibly a blocked Fallopian tube along with an ovarian cyst. All these on the right side. I was told to follow up with surgeon. Are these things life threatening? I changed my diet since ER visit and will wait for my insurance to give me the go ahead with a referral. Thanks

I am 71 and I have a leakage of clear fluid from my anus since early in 2013. It had some blood. I will sometimes have as many as 5 or 6 bowel movements (in an hour or two) in the morning with a lot of cramping but they are normal B.M.’s, not diarrea. Sometimes the cramping is so severe and located high in my abdomen that it will cause me to throw up. I only have occasional constipation, due to the pain medication I take for my scoliosis and degenerative disc in my back. I have had gall bladder surgery, two hemrrhoid surgeries and a banding I have also had two colonoscopys. I’ve seen 2 gastro. dr’s, two surgeons and P.T. who gave me kegle exercises. I take take fiber supplements and take sits baths when I have time. None of this has helped much. After my last pile surgery 2/2017, I thought it had been taken care of but after a few months being leak free, it has started again and yes I still have a bit of blood but not as much as I use to. This has made my quality of life really crappy (excuse the pun). I just want it to go away. I do wear panty liners when I leave the house which doesn’t help all that much. but it makes me feel I’m protecting my clothing.
Its so uncomfortable to constantly walkabout with toilet tissue between my butt cheeks. And it NEVER stays put. I am not sexuallty active now but when I was young there was occasional anal sex but never had any after effects. HELP PLEASE. I’m starting to loose confidence in my Doctors.

Leakage clearly suggest that your lower bowels are somewhat inflamed. Plus, the blood coming out indicates the likelihood of a new pile developing.

Needs an examination for treatment. Your Gastroenterologist is the best person to handle it.

Hi Dr, thanks for all the help so far in comments.

My problem is I experience a foul smell in anus, when I wipe throughout the day it is always wet, foul smelling, and appears oily on the toilet paper.

I recently had a colonoscopy that came back clear and I have tried incorporating fiber in diet. Could this be hemmoroids or fissure? Would my colonoscopy have revealed these two things? Another issue I have is I have to go to the bathroom many times a day and they are urgent bowel movements.

Please let me know what I should do or who I should contact.

Hemorrhoids or fissures show up on check up by doctor or colonoscopy. A Small fissure may not show up, however, it would give you pain.

The redness surround the penile raphe area too. And even the discharge seems to be coming from there….. help… Did I tear something? But Why the foul smell??

So I had anal sex about 2 months ago and a day or 2 after, it started to really hurt using the restroom and I noticed I would be very constipated. I actually used a mirror to look and I saw red inflamed vains from inside when I looked I thought it was a internal hemorrhoid. I thought it would go away but the pain mostly went away and I’m not as constipated now, but I do have white mucous that comes out with my stool when I use the restroom and sometimes there’s just yellow/greenish looking mucous on the toilet paper, it does feel wet near my anus sometimes but sometimes it’s dry, it still hurts a little pushing to use the restroom, I can’t be sure if it’s hemorrhoids or a tear and is it infected because of the green/yellow goop?

There appears to be some source of infection there. May be some fissure or cuts you suffered during sex, that later got infected. These lesions may be oozing out pus.

Small amount of brown liquid leaking only if I try to force a bowel mmovement. Once I had a small amount of blood pass, without a bm. I have hemorrhoids. And sometimes get pain upon eating on the left side of stomach right at the rise of my rib cage. Could this be related to my thyroid and or medication? Mostly concerned because I’m severely constipated and is evident by appearance, clothing fit, and overall general feeling of fullness and uncomfortableness. Does this seem serious or fairly common given the circumstances? What can I do, as this is becoming unbearable.Thank you in advance.

The left sided tummy pain may also to be related to chronic constipation, where you expect impacted stools present throughout the colon.

You need to correct your constipation state. Change diet. Remove all refined food from it, adding up fresh and fibrous fruits and vegetables. Drink plenty of water.

Also, you need to walk a lot to aid peristalsis. Is your hypothyroid state under control?

Your bowels appear to be inflamed. That’s the reason, they are oozing out fluid.

You need change your dietary pattern. Preferably, limit tot spices. Get a less spicy food. Avoid smoking, alcohol or excess of tea and coffee. All these are bowel irritants. Avoid fatty food, junk food, fried stuff. Home made food would be better for you.

You may take coconut water, alkaline fruits like bananas, custard apples etc in between.

For now, taking purified water would be better, till your bowels recover.

Looks like your lower bowels are a bit inflamed. So, they’re oozing out fluid. Such situations are generally temporary.

Try limiting on spices, tea and coffee. Take a relatively bland diet, containing plenty of fruits and vegetables. Abstain from alcohol. Avoid outside food. Things should improve within a week.

As it heals, the anal orifice would find a better position by itself. You need to avoid being constipated at all costs. Also, avoid putting on weight.

As you do Kegels and strengthen your pelvic musculature, you’ll feel assured that it won’t tear any day.

This evening it appears that I am having a clear, odorless liquid discharge around my anus, that returns after wiping, and creates a damp feeling. This happens when your lower bowels are superficially irritated. It may be due to the hemorrhoid or else, there may be local inflammation of the lower bowels.

Commonest cause of local inflammation is taking spicy food, alcohol, smoking, stress etc.

You may wait for a few days and observe.

Your lower bowels are likely to be inflamed. Probable cause- lower bowel infection.

Most of these infections are self limiting. The bowels get rid of the bug load each time you evacuate them. So, nothing needs to be done, unless you get fever or see blood in stools.

Take an easily digestible bland diet on a daily basis. Avoid alcohol, smoking, excess spices and excess of tea or coffee. Any emollient may be applied over the skin surrounding the anal opening. This is to soothe it of inflammation.

This may be pus. It’s important to ascertain from where this pus is coming out. Better to go for a physical examination by a doctor.

You appeared to have had an urethral infection. Anal symptoms are likely to be a part of that overall illness. Likely to heal with time.

However, if you get more of such lesions, you need to be checked up. Possibilities of other infections than syphilis need to be ruled out.

    Hello doctor,
I have been debating whether to go to the specialist or not. About 4 weeks ago, there were 2 days where I didn’t have much of an appetite, but I ate what I could manage to eat. After the 2 days I felt drained and exhausted. My appetite started to come back on the 3rd day. For a couple of days after that, I did not have a bowel movement, which I figured was from the lack of food. Then, for a couple days I felt like I had to go to the bathroom, but all I did was pass gas and balls of off white to brownish mucus. Then, for a couple days, I started to pass some solid stool. The problem with the stool was that most of it was covered in mucus, the color of some of the stool was white and it had a foul smell. A day later, I noticed a little bit of blood in my stool, but nothing alarming. Now my stool is a normal color, a little choppy looking, a little bit of mucus and still smells fishy. Everytime I pass gas mucus comes out. During this whole time, I’ve also have trouble falling asleep and staying asleep. I am a 33 year old female. When I was about 23, I had 20 polyps removed from my colon, but the symptoms were different. When I was about 26, I had another colonoscopy, but it just showed some intestinal inflammation. We figured out I had IBS caused by my daily intake of excedrin extra strength. I barely take excedrin anymore. Famy history is IBS, Crohn’s and colon cancer. I was going to a little bit longer to see if things get better, but I don’t know if that’s a good idea. Thanks

Lower lower colon appears to be inflamed. This may be IBS and requires treatment. Also, you need to take care of your diet accordingly.

Avoid excedrin and all other pain medicines.

Hello, I’m a straight, married Male. Same partner for 12+ years. No Anal intercourse. History of hpv. I have been experiencing anal leakage on and off for almost 1 year. At first this leakage was a clear to yellowish watery liquid that would usually manifest itself shortly after bowl movements. More recently it has been more fecal in nature, still manifesting itself after bowel movements, but happening throughout the day, long after a bowel movement. Sometimes I’ll go weeks without any symptoms only for it to return. I’ve had an examination as well as a colonoscopy. Everything came back normal; no polyps, piles, hemorrhoids etc. I exercise sporadically and notice when I jog sometimes the wetness seems to bloody, not full red blood but faintly redish when I wipe. Just this past week I have been having stomach pain accompanied with the leakage. I’m pretty fed up with this condition. It’s more of a nuisance than anything but I want to figure out was is going on. No insurance so I can’t afford further testing. Any info is appreciated. Thanks

Hi, Im an 18 yr/old girl! since I moved out I’ve been experiencing being bloated often, I’ve lived away for about 3 months now(?)

I’ve had issues going to the bathroom, and only really passed gas when I went, a few times I’ve had tiny balls of poo, but it was covered in bright red blood. Today I was walking home with my boyfriend when suddenly I felt my trousers and underwear get wet rapidly. When I got home I went to the bathroom and found that it had been soaked in clear watery liquid. It had no odor what so ever and no discoloration. I have had a slight change in diet when I moved, but since I noticed changes I’ve gone back to what I’d normally have back home- but the syntoms show no signs of going away by changing my diet. Atleast from what Ive tried. Ive had some itching down there and recently I noticed my vaginaly discharge smelling somewhat differently than usual. I’ve had 5 sexual partners in my lifetime, one I’ve not been with for over a year, and the other 3 for 5 months. I am currently with my boyfriend and living with him. We are sexually active and usually go at it once a day, but this concerns me and I dont know what it is.

I hope you would be able to provide me some answers, I live a long way from home and the trip to see my regular doctor would be more expensive for me than the actual appointment.

Hi, so I have gotten yeast infections and uti a lot lately. Last I had it was last month but I am all cleared. Today I felt my stomach was very upset and I felt moist down there so I went and did #2 and then when I wiped my vagina first some brownish looking hard discharge came out. So then I wiped my rear and it had a mucus like clear discharge along with some of the brown. I tried to wipe my front area again and it had some of the same gunk but then started wiping like normal pee. My rear then just looked like slimy when I was wiping but no more of that mucus. I’m confused and worried about why that happened.
I’ve never had anal sex and I try to be healthy with my diets but I do tend to eat sugar a lot and sometimes I skip meals , I also have been told I might be anemic I don’t know if that will have anything to do with this. I am 21 and female.
Please help doctor , thank you

Discharge coming out of the vagina may be taken as normal. It may be a part of normal cleaning work done by your vaginal canal. However, let us know if you itch there, or get any pain or any further vaginal discharge.

Anal discharge may indicate some inflammation in your lower bowels. We can observe it for a few days. Most of such inflammation go on their own with time.

Though not associated, correcting your iron levels would be better.

You’ll first need to correct your constipation. This may solve your problem to a big extent.

Include plenty of fibrous fruits, stewed vegetables in your daily diet. Take whole grains, avoiding all junk food, refined food stuff and excess of sugars.

Frank sugars are the first culprits to give you smelly farts.

Also, you need to walk, cycling won’t solve the problem. A minimum of 1 hour walk, divided into two separate sessions, may be adequate to move your peristalses.

Chronic constipations may lead to internal prolapse. However, let’s first work on constipation itself.

    Hi Dr.
I am a 25 year old female. I’ve been having to urinate more frequently than normal with the urine being foul smelling. I get a very heavy ache/pain in my pelvic area before and after urination. I also get a bit of a yellowish/white dishcharge. The past few months I’ve been getting constant bladder/kidney infections that most antibiotics don’t cure the first time. I’ve seen a urologist and been to get an ultrasound and Cystoscopy and was told everything looks normal and healthy. So I don’t understand why I’m still having these issues.

Along with my bladder issues that started occurring about 4 months ago, I also started having pretty severe constipation. I’ve been somewhat constipated ever since I was a baby. However lately it’s gotten a lot worse. I get horrible stomach pains, sometimes I get nauseas and pale and start sweating. It usually takes me 20-40 mins to have a bowel movement. With every bowel movement I have there is an excessive amount of blood in the toilet bowl and in my underwear even after wiping. I do have a hemorrhoid that can be quite itchy and a bit sore but I take Venixxa for it, as well as put witch hazel on it and preparation H. I’m not sure if it’s even getting inflamed still with my bowel movements or if it’s just my anus causeing the blood. However, after having moved my bowels my anus stings/burns and is very sore for a couple hours. Everyday I have to change my underwear up to 3 times a day because there is a yellowish oily liquid that is leaking from my anus and it smells terrible. No matter how many times I clean down there, I can still smell it. I did take fibre pills but for some reason the amount of water I drink isn’t enough to make the fibre pills work and it just makes the constipation worse. I do drink usually 1.5 L a day. I was told to take magnesium citrate and it should make my feces softer so it might not make my anus bleed but it just makes me need to move my bowels more frequently but still having constipated feces.

I don’t like leaving my house cause I feel disgusting and all my underwear is stained. Ive gone to multiple doctors but they never take me seriously. Are my bladder and bowel issues related or separate concerns? If they are separate or connected, what is causing these problems? I look forward to your answer Dr. Thank you.

The first issue to attend to is your constipation. You need to solve this problem. Fiber pills would be good. Try increasing your water intake more than what you’re taking. They’ll work.

Also, you need to move around, increase your physical activities. Include daily walks, at least twice daily, in a park or elsewhere. This would help your gut muscles regain their motility. Peristalsis would be better.

Eat fiber rich food, rich in fruits and vegetables. Avoid all refined foods, junk food stuffs and fried items.

A healthy diet would also boost up your immune system. May be, you get less of UTIs in future!

There is blood when I wipe to clean myself, it stains red, but sometimes when I wipe to dry myself it’s a light faded brown, I’m not sure if it is blood or feces. I wash myself with soap and water though so I am not sure why it would be feces. There is not a lot of blood though. Also, there is a smell after I wipe. However, the staining I am unsure what it could be.

I am a 45 year old male. I’ve suffered from hemorrhoids and anal bleeding for nearly 20 years. At times, bright red blood will fill the toilet and cover the toilet paper after wiping. My most recent and bothersome problem has been affecting me for the last two years. After working in extremely hot conditions which would leave my underwear extremely soaked with sweat, I developed a persistent rash around the anus as well as on the top of my crack. My primary doc gave me a strong cortisone that will temporarily heal the crack, but it returns in no time and eventually begins bleeding. The skin around my anus looks raw and i can see multiple small lacerations everywhere. I went to a dermatologist who gave me some eucrisa but that did nothing as well. I take stool softeners and psyllium husk daily to help against constipation, but I sill deal with some difficult bowel movements. I don’t necessarily have a lot of constipation while using these, but I do have difficulty feeling completely empty. This does create problems with abusive wiping. After being clean, I will eventually get a dampness around my anus that is clear and sticky. I’ve used Prid on my crack and will sometimes develop a small hole that closes when I switch to the cortisone. I am so tired of putting so much stuff on my butt without results. My doctors don’t seem to be of help, and I don’t have the money to continue to spend on “trial and error” treatments. With several cancers in my family, I begin to worry about that as well. What is there to do?

Though an exact diagnosis requires a physical exam, it’s likely to be dermatitis around your anus. Best is avoiding all irritants in that area to the maximum. For this, you’ll need to keep the area dry all day. Avoid use of alcoholic wipes. Always wash it with ample water after defecation and dab dry, instead of wiping. If you sweat a lot, consider changing your underwears more often.

      Wear lose cotton bottoms only.

Cortisone is the only medical help that works in such cases .However, use it with caution, to a minimum.

Could be a parasitic infection, like Giardia. Any fever?

In most cases, the parasite passes out by itself via feces. You just need to take a light semi liquid diet and keep re hydrating yourself. If your tummy aches, just pop an antacid, one pill or two, if required.

In sever cases, you’ll need to see a doctor. Appropriate antibiotics may be needed.

Since you have an itch else where too, this may be due to some irritation due to the anal secretions, or may be a fungal infection.

You need to take care that you’re well hydrated, as you may go to be toilet many times. This would eventually wash off the bug from your body. Take small, easily digestible meals.

There appears to be some digestion problem, that’s giving the gas as well. Let him take a light diet. Include easily digestible fruits, stewed vegetables, cooked whole grains it your meals. Avoid alcohol, smoking and excess of tea or coffee. Limit to sugar/ jaggery intake. Sweet food stuff lead to more gas production.

These steps would relax the gut and may heps overcoming the digestive problem.

I am a 25 year old female, very healthy and active. I have never been able to run outside because I always need to poop and can’t seem to hold it in while running. Other excercise is fine. For the past few years i have had on and off constipation that is usually caused by change in environment or excessive traveling. Since a couple years ago, when I go to make a bowel movement and strain too hard, it feels like my bowels are going to fall out of my butt and I have to shift around in order to push it back in. I generally have healthy normal bowel movements these days, but this last year I am finding anal leakage that is so much that I must wear a pad at all times in order to not leak through my clothing. It is usually clear slime, sometimes bright pink blood, and occasionally a lot of blood. When I look down there I see what looks like a fissure, but I’m not sure. I also have a more recent burping issue where I actually feel pain and need to belch very loudly after eating multiple times in order to feel better. don’t know if all these things are connected or what is going on. I had a colonoscopy and they didn’t see anything unusual… any thoughts very appreciated..

Since you are seeing blood coming out of your anal orifice, it’s better to yourself examined physically by a surgeon. Pinkish blood suggests fresh bleeding from some lesion just near or outside the anal orifice. This may be a fissure/ piles or else.

This may be a fungal infestation. Keep the area clean and dry. Wear lose cotton lowers to let the region be well ventilated throughout the day.

You may start using any anti fungal cream or powder (miconazole/ clotrimazole) over the affected area. Apply it twice daily, including once at night time, so that the medicine gets a better chance to act over the region. Let us know if it gets better.

No relief from the medicine suggested?

If yes, this may be some skin condition. You’ll need to visit a dermatologist for diagnosis.

Most cases with similar symptom have some inflammation in the lower colon area. You need to be examined for diagnosis.

As for the blood from penis, if it is slight and looks superficial, the cause could be just dryness.

    Hello Dr.,
I’m a 41 year old female. I have been experiencing a clear/light browsish mucus when I pass gas. The mucus has either no odor at all or a bleach/chemical smell at times. Occurred is the mornings usually. No itch or irritation associated with it. Sometimes I feel a lot of pressure before it happens. I had fistula repair surgery 4 months ago but I’m not convinced that’s what this is. I’m also wondering if a Dr can diagnose a fistula with the naked eye or if imaging is required. I’m often bloated and eat normal foods- not overweight. Drink coffee & alcohol occasionally. Thank you

This is not normal. Could be a fistula leaking. It’s usually not possible to diagnose it without imaging.

Hello I’m a 22 year old male. For the past couple of months occasionally when I wipe there will be very faint traces of blood on the toilet paper. It’s not every time and like I said before, it’s usually very faint, however when it does happen my anus has a lot of pain afterwards that recedes after probably 10 minutes unless I scratch it. My anus is often quite itchy as well, some days more than others, and at the end of the day my whole anus area is quite moist. It seems like it could be sweat but its like there’s too much for it to be that and it definitely doesn’t smell like sweat. Thanks for the help.

You need to be examined physically by a Doctor for diagnosis. There may be some mass down there that’s giving you the bleed. Or else, may be some patch of irritation.

    Dr,

I have had wetness around my anus for a few years now, I’ve mentioned this to my primary, to a gastroenterologist, and a PA. The gastro took a quick look about a year ago and tested my blood but he didn’t find anything. I never went back. Primary suggested sweat pills but I’m not a fan of daily pills. It seems to be worse after a bowel movement and when I’m nervous/ or in social situations. I will have soaked underwear right where the crease of my buttocks is. It has a faint fishy smell but evaporates with no stain. My diet is decent not much fast food. Should I go back to the gastro and specifically ask them to check for abscess? If I take a look in the mirror I can feel two sort of hard parts on either side of my anus (normal glands?)

Thank you so much for your medical opinion.

From what you’ve written, it appears to be excess of normal secretions, may be triggered by stress factors. The solution would be managing stress.

However, if you feel two hard nodules down there, you may go for a check up to be sure.

    Dr,

I am 20 years old, female and my problem is that when ever I pass my stool later I find an orangy, yellowish and brownish fluid coming out of my anus.
It isn’t itcy or there is no pain also. I do not have any problem of constipation. I also have pain in my lower right abdomen and on my one leg, sometimes on right and sometimes on left but one at a time.

    Hello,

I had a perianal abscess removed on Oct 31, 2018. Otherwise healthy female – not sure what caused the abscess other than an E. Coli infection. I’ve been having soft stools, and the two times I did strain a bit I got an external skin tag (around 11/21) and this past weekend (12/22) I’ve been having a slight discharge from my anal area (it doesn’t smell awful, it’s pink/brown in color, and it’s liquidy….no mucous).

My abscess is almost completely healed – one bit of skin left build in order to cover it all up. I am scared of another infection there though. I’ve been using panty liners and ABD pads ever since my surgery to soak up any dampness down there.

I am not sure if it is a fistula or not or if it’s just my anal splinchter muscle. Any help would be awesome!

PS- my diet has been high in fibers, no alcohol, no coffee, very little sweets.

You are on the right kind of diet.

Since there’s no foul smell, it’s unlikely to be a fistula or another infection. May be, you strained someday at stools.

You may wait for some time and observe. If the problem still persists, seeing a doctor for physical exam would be better.

My brother did not believe I could get such a thick white discharge. But then we did a couple hits of meth and I jacked him off in 4 strokes. He had such thick white cum. Anyways hoping you could help me with this.
