Causes of Anal Leakage

If you are dealing with the problem of leaking anus, then you need to ask this question. What type of fluid is leaking from there? This is biggest hint about the cause of leakage.

Types of Leaking Fluids

Mucoid/ Clear Fluid
It could be a transparent looking, slimy, mucoid discharge, coming out of the bowels. This usually indicates that there is some inflammation in the lower bowels. A physical exam with or without colonoscopy may reveal the cause.

Purulent
That is, pus containing discharge. This is likely to be due to some infection down there. There may be evident lesions, as are common in tuberculosis or some STDs. Or else, there may be an anal/ perianal abscess.

Watery
There may be a tear or a fistula around the anal opening, that is oozing out the clear liquid. Or, there may be an abscess that’s leaking fluid. You need to get examined.

Blood
The fluid may be blood stained or you may get frank blood coming out of the anus. The commonest cause for such a situation is Piles. Other causes include severe infections of the lower gut. Proper evaluation is needed to investigate the precise cause.

Fecal
A fecal discharge coming out of the anal opening may suggest incontinence of the anal sphincter. This means that the muscular orifice controlling the anal opening is not tight enough due to some reason.

Fecal discharge may also be seen in cases where there develops a tunnel between the anal canal and outside skin. This is medically termed as an anal fistula.

Fatty discharge
Some may get a greasy or fatty discharge from there. This happens when your stools contain excess of fats in them.

There can be two reasons for this, either you are consuming too much of fats, or your intestines are unable to assimilate the fat present in your diet.

Symptoms of Rectal Discharge

  • Staining of undergarments
  • Pain or ache while defecation
  • Feeling of wetness down there
  • Anal itching
  • Frequent urge to go to washroom
  • Foul smell from groin area
  • Redness or swelling around the anus

On the basis of your symptoms and the type of fluid leaking out, your doctor may proceed with investigating you. Some conditions where leaking anus is commonly seen are discussed below.

Common Causes

Inflammatory Bowel Syndrome

IBS is characterized by inflammation and irritation of the distal bowels. A clear or yellowish white discharge may come out. This is usually mucoid in consistency.

It feels slimy to touch. The patient may often perceive as some wetness down there. The  yellow color may be due to the bile pigments from the gut, or due to fecal contamination.

The pH of the fluid may be acidic sometimes, leading to itchiness and redness around the anal opening.

Proctitis

Certain infections affecting the distal anal canal may lead to leaking of inflammatory fluid from the anal opening. The fluid may be pus like, as in tubercular infection, or clear mucoid.

STDs may also affect this area. This is specifically common in people who practice anal sex. Lesions may be seen in or around the anus on examination.

There may be ulcers, rashes or overt growths. Itchiness may be there. A feeling or dampness stays in the groin area. There is a discomfort in movements.

The underwear may be blood stained or sometimes pus may be seen. Bodily symptoms like pain in lower abdomen or fever may be present.

Anal Warts

Warts are open growths in the region caused by certain viruses. They are usually sexually transmitted.

Perianal Abscess

Abscess is a localized collection of pus any where in the body. It commonly occurs near or around the anal opening, since this area is exposed to plenty of germs. Abscess in this location is called a perianal abscess.

Malignancy

A serous discharge may be seen in malignant growths. If infected, the exudate may be pus like.

Such growths are usually painless, irregular and attached to the underlying skin.

Piles or Hemorrhoids

A fishy smell in your anal discharge may indicate hemorrhoids. A careful examination by a doctor is required.

Blood coming out of the opening is, by far, mostly due to piles. However, since malignancy is another cause of getting blood there, it’s essential to evaluate.

Keriorrhea

This is a condition where orange to transparent colored greasy stuff leaks out from your bottom. The discharge may be as little as a tea spoon full, to larger than a cup, that needs to be evacuated in the commode.

This happens when you consume certain fish oils that are indigestible. The oils pass out from your anus as it is, without being used by the body.

Fecal Incontinence

Leakage or involuntary passing out of the stools  from the anus is called bowel incontinence or fecal leakage.

This ranges from slight fecal leakage to passing out of the whole stools. There is a foul smell from the discharge. The skin around the opening in constantly wet, and may itch or rot.

Causes of Incontinence

  • Chronic diarrhea/ constipation, which leads to irritation of the anal orifice
  • Denervation, that is, injury to the nerve controlling the anal orifice. This may happen after some surgical procedure for hemorrhoids or childbirth.
  • Medical conditions involving the nerves, as diabetes or multiple sclerosis
  • Muscle problem, sphincter loosing
  • After parturition, especially when forceps have been used or episiotomy done.

Management

For all cases of fluid leaking from your bottoms, it is suggested that you see a doctor for proper evaluation about the cause behind it.

Meanwhile, keeping the area dry down there is essential. This is to prevent maceration of the skin there, and subsequent chances of any infections. Butterfly body pads or liners are available in the market. They may be used to keep the region clean.

I am a 43 yr old woman who has had no other illness other than asthma. I lately have had yellow oil coming out rather than stool a lot of the time. It’s strange and sometimes it’ll be all oil then I am done, stand up and a few min later I feel I need to use bathroom again so I try and more oil or yellow stuff. Sometimes it’s all oily stuff. It’s always bright yellow. Not clear almost as if the fat I eat is coming out while? If that makes sense. It’s awful. Feels weird too bloated like during these times. Like air also is there. What is happening? All tests show normal.

Could be undigested fat from your intestines. Cut down your fat intake. Include more of fruits and freshly stewed vegetables in your daily diet.

Cut down on all dairy, meat and egg. Take whole grain meals, as far as possible.

Do let us know if this helps.

You may be having slight inflammation in your lower bowels. This is the commonest reason why you get slimy secretions down there.

Slight changes in diet and lifestyle may solve the problem. Restrict your intake of tea, coffee and alcohol. Drink plenty of water. Include fresh fruits and veggies in your diet. Avoid excess of all spices.

This happens commonly. However, it’s better to get yourself examined to see if theer’s any evident injury or lesions back there, that’s giving you the pain.

    Hi,

I am wondering. this has only occurred the past couple of days. as everyone knows, the cornoa virus is going around and I did test last week and was negative but I don’t go anywhere, however, I have been trying to lose lots of weight, excercising a lot, and eathing just about no fat. I have had tons of anxiety . I will be 46 in less than a month. anyway, I never had this ever before and my stomach and everything was completely fine till basically yesterday. yesterday and today my stomach had felt I guess yo ucould say acidic and once in awhile my right side hurts too. but starting yesterday afternoon, and going through today, it feels like I have this feeling of very slight leaking. there doesn’t seem to be any color. just feels (wet) I guess. the stomach and right side seems to have gone down slightly. I was worried about the fact I ate some frozen berries I had for several months last week because I heard about hep A recalls, but I did microwave the food slightly. but in addition, and I don’t know if this may be part of it, but I have taken 5-8 x as much vitamin C over the past few weeks as I ever have before simply because I am trying to build my immune systtm, it is a harder pill. I have also taken 1.5 to 2x as much as all the other vitamins so I don’t know if anxiety and too much vitaimns could be casuing this? or if it was something I ate? like I said, I have never had this ever.

      May be anxiety.

If it pains again on the right side of your abdomen, anta acid pills may be tried. These would neutralize the acid produced due to spoiled food/ anxiety or any other factor. Likely to help.

    Hi
Have been having this problem for a while, I get a watery type fluid (probably leaking) from my anus which produces a foul smell down there and on my underwear. I’m very clean, shower 2x daily, clean clothes, use deodorant etc. I eat relatively healthy as my mum is a nutritionist though i do eat “junk foods” occasionally but I guess that’s normal. Thought I’d mention I trim my pubic hair as it’s cleaner so I doubt it’s a fungus? Recently had an intolerance test and I came back intolerant to yeast. Only started cutting out yeast a few days ago and have noticed no difference but thought that may play a part? I’m gaining the courage to go to the doctors and get checked out after the corona virus has calmed down but I’d rather not as it’s gross and uncomfortable for both me and the doctor. I only ever have sex with girls at parties as it’s only a one night thing and it’s usually intercourse rather than oral so there isn’t much risk of her noticing the smell. I’m fed up and want a meaningful relationship but this is holding me back because who wants a lad with a smelly butt. I’m just your average 17 year old who wants to feel normal again. Please help.

Could be related to some food you eat. You may need to cut down on spices, alcoholic beverages or caffeine; as excess of any of these may give you GI problems.

Else, this could be related to yeast intolerance. Any history of dairy intolerance in family?

Some mucous production there is normal. However, excess of mucous or it’s leakage needs to be checked. Often it is related to one’s diet. Excess of spices, chilies or garlic are common culprits.

Some medical conditions, causing inflammation of the lower gut, are also attributed to this condition. These are commonly grouped under Inflammatory Bowel Diseases. You’ll need to see a specialist to get diagnosed.

Hello, I wake up with my underwear really a big portion of my butt is wet and it looks like I sat in a puddle. I smoke cannabis often, and I suspect this excessive butt sweating is happening due to the excess fatty acids/oils from smoking. It has only gotten worse over the months trying baby powder or other remedies and I do not know what else could be resulting in this.

    1. Buddy M.D. Post author

It is recommended that you gradually stop taking any such drugs. Many more unpleasant side effects are yet to come with continued use.

Night sweating is specifically common with cannabis use. To minimize, keep yourself well hydrated. Drink plenty of non alcoholic drinks through out the day. Take healthy diets, which includes plenty of fresh fruits and vegetables for detoxification.

Minimize your intake of tea and coffee. Caffeine in such drinks may add up to your troublesome symptoms.

    Dear doctor,

I have the following problem and I am indeed suffering very much from this condition or at least from the fear of this possibly happening:
I am a gay young male in his mid 20s. I used to have a very healthy life style, I hit the gym three to four times a week, I ate healthy wit only little fast food and I almost never drank alcohol. I always had some problems digesting lactose ever since I can remember. However, in my warly 20s my gastroenterologist also diagnosed an intolerance to fructose and sorbitol as well. Approximately 18 months ago I was diagnosed with metastatic thyroid cancer (which at the beginning my doctors deemed to be a Non-Hodgkin lymphoma) so I had to undergo major surgery and radioiodine therapy twice. Ever since my life has not been the same any more. I have constant bloating whatever I eat, I often have cramps in my belly. But life goes on. I am in my residency now and every day I feel awkward and uncomfortable in my skin.
However, what makes me suffer the most is the fact that as I am a gay male, I have engaged in some recepticr anal sex over the past 6 years, albeit something in-between 30 to 50 times cumulatively. So yes, I have obviously had receptive anal sex more often than my straight peers have, but I have had receptive anal sex in a much lower frequency than most of the gay guys of my age. Thr thing is that a have developed a real fear that – since I am always bloated since the two radioiodine therapies – I might pass wind without noticing it until I smell it. I really don’t know if this is indeed a thing but I am pretty sure it happens on longer car rides. And I feel really sorry for myself because I don’t know how to change this situatiom and if I should indees feel guilty because of having had receptive anal sex and thus being responsible for all this mess by myself. I should mention that I do not have fecal incontinence. Only flatus incontinence is what makes me struggle each day with myself 🙁

    Yours thankfully,
    a fellow young doctor

Your problem is likely to be a side effect of anti cancer treatments you received. Nothing much can be done about it. Though, it would gradually get improve with time.

For now, adding edible fibres to your diet may help. Avoid all frank sugars and dairy. Include as much fibre in your diet as possible. Fibre rich food may include leafy salads, citrus fruits, coarse grains etc.

Could be hemorrhoids. You’ll need to get a physical check up done to confirm the diagnosis.

Possible is not good for involuntary muscle sphincters of our body, be it the anal sphincter or the esophageal one. Limiting it’s intake would help.

Eating a healthy diet, comprising plenty of fruits and vegetables would benefit. Drink adequate amount of water as well.

Meanwhile, you may also use a portable bidet to clean down there, each time you get that fluid out. This would prevent irritation around the anus.

    Hello.

I am a 26 year old male with an embarrassing and anxiety inducing problem

When I was 21,I was diagnosed with a hemorrhoid but in the past five years it did not bother me at all, until around July of this year, one Day at work, I passed gas and felt an itchy and burning sensation so I went to wipe and some poop had come out, Just a little though. I went to my doctor and he gave me suppositories which I took for two weeks and I feel it did help as the pain and occasional bleeding went away.

Now, I am not constipated nor do I have diarrea. I eat healthy, drink over a liter of water a Day and use the restroom anywhere from one to three times a Day. I have no issues pooping and I can be in and out of the bathroom in five minutes, which I should mentón I use a prtable bidet to wash the área and afterwards I feel fine, although sometimes a little humid down there.

The thing is I have two Jobs, one where I am sitting all Day and the other where I walk a lot. At the sitting job, I am fine and don’t have any issues but once I start walking and start sweating, I sometimes need to go wipe and sometimes it’s Just sweat and other times it’s a brownish, yellowish líquid that doesn’t smell like anything. Could this be fecal remain that come out thanks to the sweat that causes it to come out? This only happens when I am walking where I need to go back and wipe and, it doesn’t happen everyday.

Also worth mentioning that I had blood test done and nothing malignant was found, nor do I have stomach problems. This is something I have dealt with almost all my life, where I walk and start sweating, and need to go wipe again. The bidet does help a lot in feeling cleaner but I still sometimes need to go back and wipe.

These past two or three times I used the restroom, I felt a stabbing pain in the anus like as If I were super constipated but Everything was normal and I was in and out of the bathroom as always, it was Just that stabbing pain that came and lasted maybe 20 minutes. I do also wipe more than I should and my anus is also pretty irritated from so much cleansing

Maximize your use of the portable bidet, you mentioned. This would prevent pain, burning and inflammation in the affected area.

The stabbing pain, you mentioned, looks like Proctalgia. This occurs due to spasm of the pelvic floor muscles, the muscles of the anal sphincter, or the muscles of the rectum. You may talk to your health care physician about it.

Usually, medications are not required. Pelvic muscle retraining is practiced to manage the condition.

It’s likely to be from your anus. You may call it poop.

Looks like there’s some inflammation in your lower gut. This may have caused this inflammatory fluid to leak out. Also that bubbly sound from gut. Such issues are likely to subside on their own. It may be related to something spicy you ate or may be excess of alcohol.

However, if it recurs, you’ll need to see a doctor for physical exam. It could be blood mixed with fluid, oozing out from some lesion down there. Such possibilities need to be ruled out.

I am 71 years male, physically and mentally active suffering from chronic constipation. I eat and drink healthy, plenty of fruit and green veggies. Over the last few months, I have leaky anus. The leak isn’t foul smelling nor perhaps colored, nor slimy. Doesn’t happen when sitting but when I stand up and is accompanied by bubbly gas from the lower gut. My weight has been going down consistently. For my height of ~170 cm, from ~70 kg a few years ago, It reached 65 about 6 months ago and now 62 kg. It appears as if old age has befallen me much too early. From the readings on this forum, prima facie, the condition could possibly be: (i) mal-absorption of fat by the lower small intestine, (ii) inflammation, or (iii) the loss of elasticity of the anal sphinctre. Request advise.

Hi Doctor, Thanks for the reply. Yes; I got it done about 2 years ago. As per the GI specialist, it was absolutely normal. In the meantime, I have visited a Dentist, who prescribed antibiotic for the procedure he did. I am now on the 4th day of the course; there appears to be relief from the leakage and the associated bubbling etc. starting from the first pill. I also feel more energetic (I am using B complex as a supplement) and appear to be bouncing back to health with my defecation cycle becoming (is it temporary?) normal. Should I now observe for more time until after the antibiotic course is through? I have to keep vising the Dentist for 2 or 3 sessions more during the coming month. I shall in the meantime fish out for the old colonoscopy details. Should I get a fresh one done again? Thanks again and Best regards:

Too much of cocoa/ chocolate does make your sphincter lose and less competent. So, long term use may hamper their function to some extent.

Try trimming your coffee/ cocoa intake. You may see a difference.

If your problem doesn’t solve, further evaluation may be done.

I was 14 when i noticed i have hair in my butt and i tried to pluck some hair like 3-5. After i did that, my butt started to sweat and smell like poop. The smell started getting worse day by day and my butt is always sweaty and i have to wipe it frequently and find liquid poop on the tissue. This have been going for 3 years and i dont know what to do. My family’s not even bothering to take me to a hospital, not even once. The smell is not that worse now compared to 3 years ago but i have noticed that im always constipated. I go to the toilet for like 2x a week and sometimes none for a week.
I didnt know what to do, so i just started to think that maybe this is because of constipation but what’s the connection of plucking butt hair to that?
This greatly affected my life as a student and I don’t have anyone to relied on.

Likely to be due to constipation. Take plenty of fluids, dietary fibers, fruits and vegetables to correct the condition.

Avoid excess of tea or coffee, as they aid constipation.

Plucking hair can be dangerous and may instill infection at the site, condition called folliculitis. Better avoid it. Having hair over the area is not uncommon.

I feel like I need to use wet wipes every time after a bm to be sure I’ve cleaned it fully. It’s very difficult. I need to bring them everywhere. I really hope I’m not damaged. But if I don’t have the wipes the pain does be so bad the sting like an acidic sting. Tried anusol and that also causes the sting. So I don’t know what to do.

Using a regulated stool softener may help. Try using metamucil or any other brand on a daily basis.

Injuries out there take longer times to heal. However, they’re not permanent. Meanwhile, local area care is essential. Consider cleaning with splashes of plain water instead of wipes. Wiping there frequently in a source of unnecessary friction and may delay healing.

After cleaning with cool plain water, dab dry the area with a towel, instead of rubbing. Let it dry completely before you wear your bottom wears.

Wearing loose bottoms would be better. Also, keep applying any bland emollient over the affected area to avoid dryness. You may use it 2 to 3 times a day.

You need to manage your hemorrhoids first. Also, get rid of constipation completely. It’s bad for you. The best way to have regular formed stools is including plenty of fresh fruits and veggies in your diet. Restrain from all junk foods and sugary items.

If needed, a dietary fiber supplement may be added.

Over the past 10 years I have often felt dampness in my rear end and often needing to wipe. This was especially bad after drinking too much alcohol. I always figured it was random sweat or caused from drinking too much.

about 9 months ago I completely stopped drinking. The dampness continued where I would have the feeling a few times a day especially in the morning. About 3 months ago the feeling started to happen all of the time. It feels like water is constantly leaking from my anus. It’s a clear liquid that does not smell bad. It isn’t a large amount of liquid, it’s just enough to make my rear end feel damp and if I don’t constantly wipe to keep it dry my butt and the back of my legs feel wet and itchy. Not enough liquid to be seen in my underwear or clothes

I met with a surgeon. He found that I had 3 internal hemorrhoids that didn’t appear to be inflamed. We first tried a fiber supplement, stopped washing the area with soap, decreasing the small amount of caffeine I drink. That did not make a difference. Next he banded the 3 hemorrhoids one at a time. I just had the last one done 10 days ago. Still the same symptom. He said to wait 3 weeks total from the last banding and then we would order a colonoscopy.

Any advice is appreciated. I am constantly asking in my head if it is a discharge problem or a sweat problem. Dampness seems to start right at the anus. I have no other symptoms and I have been able to deal with it, but problem is a real nuisance Thank you for your help

Do you have excessive sweating else where over your body?

If not, this is likely to be discharge problem. Eating a fiber rich diet, abstaining from alcohol, limiting caffeine would definitely benefit in a long run. Also, limit to excess of spices like garlic and pepper.

But these lifestyle changes take time to show results. You’ll need to be patient.

You may go for colonoscopy to see if we’re missing any thing else.

I’m a 47 year old male and pretty active. I’ve been living with this problem for almost 2 years. During the summer of 2020, I’ve noticed excess mucus on the toilet paper when I wipe. Also during that time I experienced pretty bad constipation. Ever since the constipation, I have not had consistent bowel movements. Lately, I experience bleeding when I wipe. I went to my dr who then referred me to a G.I. The G.I. Recommended a colonoscopy. The colonoscopy was completed and everything was fine with my colon. The G.I. Said j have small hemorrhoids that did not need need any banding or other procedures done to them. He suggested I use a medicated cream and he said the mucus leaking is due to IBS. The colonoscopy was done last June 2021. I continue to have anal leakage. It is usually clear and kind of liquid. When I wipe I will see clear mucus in the toilet paper. This leaking gets so bad that I have to wear 2 pairs of underwear and there will still be a spot in the back of my pants. I have tried everything – increase fiber, Metamucil, stop coffee, stop soda, no alcohol, more fruits and vegetables, more exercise, sitz baths, etc. nothing seems to help. Always mucus when I wipe, and always wet underpants and pants.

Continue with your efforts- increase fiber, Metamucil, stop coffee, stop soda, no alcohol, more fruits and vegetables, more exercise. Add whole grains and complete abstinence from junk food. Apart from this, you need to take a probiotic on a daily basis. This could be home made curd/ yogurts or you may purchase any from the market.

Since IBS is a chronic problem, it would be better to learn managing it by conservative methods at home. It basically means that your gut it irritated and needs to be soothed.

    Hello,
For many tears now i have been battling with yeast and bv. Sometimes tests come back negative snd i still experience symptoms this always irritates the anal area and the skin surrounding. I use treatments but ut always returns. The itching is unbearable and often its all in the anal area. Is there any chance it could be originating from there? Sometimes its quite damp down there and its hard to know where its coming from. There is also an odour. Any idea what it might be? Im so tired if this snd just long to be normal!

Though this is not a diagnostic sign of cancer, it would be better to do a screening colonoscopy, in your age.

If you have accompanying fatigue, weakness, blood in stool, abdominal discomfort, chances of malignancy may be higher.

Hi, I’m 22 y/o and had been having bowel movements with ‘sharp’ things in it for a few months. Every time this happened I would see blood; I assumed that the blood was coming from fissures caused by whatever the sharp things were, but have also had hemorrhoids in the past.
A few weeks ago I was told that most likely I am chronically dehydrated, and that’s why my bowel movements feel ‘sharp’, so I’ve been making an effort to drink more water and have seen a significant (positive) change since.
Today, I had a bowel movement with no ‘sharp’ things, but saw there was blood, a small blood clot, and now I’ve been excessively leaking clear, scentless liquid from my bum for a few hours now. I’ve showered & washed the area with soap but the leakage isn’t stopping.
I can’t get in with my doctor until 2 weeks from now and it’s really starting to worry me, what do I do??

Be a little more patient dear. Continue rehydrating yourself with plenty of water. Also, add fresh green vegetables and seasonal fruits to your daily diet. Restrict to alcohol, smoking and limit your intake of tea and coffee. All these chemicals are dehydrating and irritant for the GI mucosa.

You’re likely to gradually improve with each passing day. It may take around a month before you see visible results.

After a bowel movement I do need to wipe a lot. Often a shower of the area helps a lot. I feel I can’t get it all out. But always regardless of a shower and proper cleaning, I have a light brown tinted liquid seeps out and causes extreme irritation stinging and itch. I’ve been to the doctor she didn’t find any haemoroids and put me
On sachets for constipation and told
    Me
To do pelvic floor exercises. I’ve had no improvement it’s been a month. Is this faecal incontinence? I’m
    So worried. I’m 36, female.

Constipation is the first one to rule out. Take a healthy diet, full of seasonal fruits and vegetables to combat it. Also, drink adequate amount of water.

This would help you get rid of constipation. Also, a healthy diet, rich in fruits and veggies, would give you enough anti oxidants to heal any inflammations going on it your alimentary canal.

Apart from this, see if your getting enough probiotics in your diet. These are good bacteria, that are needed for a healthy gut. The best way to get them is consuming home made curd/ yogurt on a daily basis.

Restrict on food that damage anal sphincters, like excess of chilies, garlic, alcohol, chocolates, smoking etc.

Do continue with pelvic floor exercises.

Your symptoms are likely to get corrected gradually. It may take some time.

I cleaned and dried very good (carefully and softly using a finger and a sheet of TP after wiping stuck the tissue about 3inches in or so. I then thoroughly washed both hands and air dried them. I then got my middle finger wet and inserted it inside my rectum, id say about 5-6 inches in I felt nothing until I moved it around completely to which I came across something hard with an odd shape. It wouldn’t move as I thought it could very well be poop. Pretty sure I was feeling the edge, it was like a wall of something. any idea? also it did not smell like poop at all!!
Will be starting to implement more fiber in my diet moving forward. from time to time I do sweat from down there and sometimes its sweat other times its not! even though I do my best to clean. I’ve heard of procedures where they use water or some kind of fluid to fill your insides up to flush out your system. is that something most hospitals do? is that an expensive procedure? would it be recommended for the average person to have something like that done every say 5-10 years?
not looking forward to the doctor visit that’s for sure! 25yo I try to eat healthy with little to no junk food.

Likely to be your normal rectal wall you felt while self examination. However, you confirm what it is, you’ll need to see a doctor for physical exam.

Such washes are not a routine work. They are done post surgeries, or other pathological indications.

I am an 80 years old man. Lately (around 3 weeks or so) after going to bed and getting up in the night to urinate I sometimes involuntarily fart out very small lumps of poo (which fall down my pyjama trousers onto the floor, messy and embarrassing) and sometimes a slight slimy mucus. I also now seem to be going to the toilet a lot more to excrete although only small lumps come out, which did not used to happen.
I do have urinary problems (frequent visits to the loo) and haemorrhoids which I treat with an anal cream when necessary but cannot think that they are the cause. My eating habits have not changed. I am taking medications for hypothyroidism, atrial fibrillation, anemia, iron deficiency and prostate as well as cholesterol tablets and injections for blood thinning.
    Likely cause?

may be your peristalsis has slowed down due to lack of activities. Increasing the amount of dietary fiber in your daily diet may help. Or else, any stool forming medication may be tried.

Your medicines may also be playing a role in causing these symptoms.

Hello, I’m a 26 year old female, about 115 pounds, and I live a moderately active life as I’m chasing two toddlers around all day as a homemaker. I noticed I had a hemorrhoid when I was about 19 or 20, I was young and didn’t think much about it and was honestly quite embarrassed so never sought professional help. Since then I’ve had two children, the hemorrhoid seemed to get worse after having my second child. I notice blood after wiping a few times after a bowel movement so I have to use wipes and I’m considering using a peri bottle now to ensure I’m completely clean. I have had a copper iud for almost a year now and I do notice that since then I have some random pain in my uterus and lower stomach, never enough to be completely uncomfortable but it’s there. Recently though I’ve noticed a clear fluid on the back of my underwear and it does have smell like poop. It’s pretty strong and I have to change underwear or liners a few times a day now. It’s really picked up in the past couple of weeks. I do drink 1-2 cups of coffee a day and eat up to two bowls of whole fat yogurt with dark chocolate chips and almond butter everyday. I do drink water and take a prenatal (I breastfeed), 1 garlic pill, 2 cranberry pills, and 1 activated charcoal pill a day as well. Besides that I do consumer dairy, gluten, and try to get my veggies and fruit in my diet. I am uninsured so it’s a little hard to seek medical help at this time so if there’s anything I can do to manage this a little that would be helpful. Thank you!

Stick to the healthy diet you’re already taking.Just trim those chocolate chips, as chocolate tends to worsen the musculature of various sphinctures in our body.

So, need to avoid being constipated at any cost.

Just start doing Kegels Exercises to strengthen your pelvic floor on a daily basis. Exercises strengthening your anal sphincter may be started. Also, avoid lifting heavy weights for few weeks. Let your core muscles and pelvic floor gain strength first.

Give it a month or two and let us know.

Sciatica would give you pain behind your thigh which may extend downwards or upwards to cover larger areas. Leaking from sphincters are likely to be due to sciatica alone.

Yes, it is not, i’m sure… I think it happens when i pass out gas, i suspect that has to do with the feces being so loose, i feel like i have some type of internal inflammation… And even consuming fiber (even if it helps a little) doesn’t solve it. I don’t know what is causing it, but it’s being going on for like 5 months

But is this something that i will have to deal with forever ? Or is it curable?… honestly, it’s giving me a lot of anxiety.

This is surely curable. Just give it time and keep eating healthy. Also, it’s advisable to add some probiotic in your diet on a daily basis. This would restore intestinal microbiome, which is likely to be disturbed due to repeated antibiotic courses.

You could take anything like home made curd/ yogurt etc.

We may first try managing your symptoms by dietary alterations only. Eat freshly cooked light meals containing plenty of fibers. This may include stewed vegetables, lentil soups, breads, rice etc. Avoid all junk food, heavy calorie laden food, fried items, frozen food stuff. Avoid excess of spices. Mildly spiced food in alright.

Also include any probiotic supplement to your daily meals. This could be home made curd/ yogurt. Let’s give it some time before investigating further.

In the last few months I have two episodes of feeling a lot of pressure , and urgent to do a bowel movement but all that comes out is mucus…and a fluid that looks like water and gushes out.
I did see a gastroenterologist and he wanted to do a colonoscopy as I was due was as had a precancerous polyp removed 6 years ago . However I have to take care of my son with type 1 diabetes. And being stuck in the bathroom for hours during the prep ljust seems too stressful to do . Oh I see em to alternate between constipation and greasy stools. Thank you.

Your symptoms hint strongly towards Inflammatory Bowel Syndrome, a disease condition where the lower bowel are chronically inflamed. You need to get the diagnosis confirmed first, and then the treatment may be started.

Your GP may order some tests for that.

Inflammatory Bowel Syndrome is basically a lifestyle disorder. Patients suffering are mostly those who have lived stress for long periods of time. So, stress management remains an important part of disease management.

    Hello,

I’m a 27 year old female. I’ve had no kids and I’ve never been pregnant. I’ve had stomach issues my whole life, even as a baby. I was diagnosed with IBS early on. I suffer from chronic constipation, and it’s bad. I also have very severe mental illness and I’m on over 10 different medications, some including pain medications. My doctors tell me this makes me constipation worse. The longest I went without a bowel movement is 18 days, which was a few years ago. I have had some external hemorrhoids but nothing else. I was put on a bowel regiment over a year ago which has helped me poop almost daily.

A little over 4 months ago I started leaking a clear, watery, and sticky fluid from my rectum. I got Covid the same week.I didn’t take my bowel regiment during Covid because I was too weak. Covid went away. 10 days after the start, leaking went away for 2 days and then came back. Doctors upped my bowel regiment. Didn’t help.went to urgent care, x-ray showed that I was full of stool. More bowel regiment. No help. Told to do a colon cleanse with miralax, it went away for 2 days and came back way worse. Before, if I showered, I might be dry for a few hours before my next bowel movement. Now, it was 24/7 and leaked way worse. It’s been like that ever since.

A month after it started, I’m now in a diaper daily I got tested for infection and had C-diff. Primary told me it was the cause. Stopped bowel regiment. Still leaking. Had c-diff for another 2 months. I still leaked when it went away. During c-diff, my GI realized my leaking probably wasn’t related said it was internal hemorrhoids. Took prep H and witch hazel wipes. Prep h cream didn’t work, tried their suppositories, didn’t work. Went to Mayo Clinic in Rochester, GI said it was pelvic floor dysfunction. Did manometry with balloon test and test to see if I felt the need to defecate. All normal. Did x-ray, full of stool. I told him it was probably because my c-diff was going away and my constipation was returning. Told him I was on a bowel regiment when it started and I didn’t think it was constipation. He disagreed and said it was constipation. Refused to text me for anything else. Went to a PT who specializes in Incontinence and pelvic floor. They do a rectal exam and feel what they say is a level 2 rectocele. Starts some exercises. Says no kegels because I’m already tight enough as it is. Said it might make it worse. Went to OB/Gyn confirm rectocele and also a cystocele, but they say it’s a level 1.

Go to rectal and colon associates. They do rectal exam and say rectocele is too small to cause issue. I’m devastated. They say they can’t help me. Say they can try defecography but thinks it won’t help. Even more devastated. Have colonoscopy. In so much pain during first part I go to ER. Go home and finish. Turns out I have IBD. Unsure what kind right now. Colonoscopy doctor thinks crohns, new GI who specializes in IBD says maybe ulcerative colitis. Waiting to get meds. Return to ER because of severe stomach pain for over 24 hours. Barely able to move. In ER for 12 hours. Return home in pain. This is the history of my leaking. I’m still in a lot pain and feel very weak. Stools are bloody looking and wet. Gurgling and tingling, I’m assuming from IBD. Mental health is horrible. Suicidal ideation daily because of leaking. I don’t leave the house. I wear diapers but it leaks out of the sides. It doesn’t fill it ups but it sticks close to my bottom and seeps out under the edges of the diaper. I’ve tried many techniques to stop leaking. Nothing works. Using cotton balls now, but it’s still bad. Diet has always been awful. I have a bad relationship with food and it’s related to mental health issues+trauma. It’s hard for me to eat healthy foods. I may have a panic attack. I still try. Drinking enough water is hard because of newly worsened ADHD. Have barely eaten in the last 2-3 weeks because of depression and stomach pain. Not doing well mentally, physically, or emotionally. I don’t know what else to do. If it was hemorrhoids, proctitis, fissures, or fistulas then colonoscopy would show it. Pelvic muscles are fine, maybe even too tight. I’ve been working on relaxing them and breathing. Nothing. Absolutely nothing. I’m at my wits end and my husband and I won’t be able to go on a vacation I’ve been waiting over a year for because of this. What do I do????

I’ve also developed multiple intertrigo rashes that won’t go way with both cream and anti-fungal meds. Don’t know why. Makes suffering even worse. Laundry is piling and I can barely get out of bed. Struggling and need help and answers.

Also, showering used to help but now it makes it run more. I do use soap to clean the area and try to gently rinse the area. Seems to make it run worse. This only started a month and a half ago, so I don’t know why this is happening when it didn’t cause issues before. Wiping, sitting on the toilet even just to pee, and other things, can make it worse. Please help me.

As for the first ER visit, I mean I was in so much pain during my bowel prep. Mine had two parts because I have very mild CKD. Also, I am obese, at 226 and 5.4 ft. tall. I used to be 258 but have lost a lot of weight over all of this. Stomach has been crazy bloated this whole time despite weight loss. I look pregnant even though I’m definitely not. I also have symptoms of either UTI or vaginitis with excess discharge and fishy smell but tests were mostly fine at ER. He said the parts that weren’t are probably due to IBD. I disagree and may call ER back about it.

It’s an IBD picture largely. Managing this condition is very difficult in a long run. You’ll keep getting symptoms as you taper your dietary regimen. And this dietary regimen is not advisable for long periods. It’s for short runs only.

The key is to control your mental health. You’ll need to be strong. Try to meditate some minutes each day. This would ease up your mind and reset the bowel functioning as well.

For now, dietary regimen may be needed to ease symptoms.

I’m a relatively fit 46 year old male who works as a server. For as long as I can remember, I’ve had issues where I need to go to the restroom after a hike, a shift, or a workout just to wipe, even without a bowel movement. Sometimes I need to do this mid shift. There is usually a small but noticeable amount of feces on the toilet paper, as if I didn’t wipe after going to the bathroom earlier (but I always wipe thoroughly). I get really insecure if I can’t go wipe at least every 4 hours, and I certainly wipe before a workout so I don’t smell in the gym. The smell can get bad for anyone hiking behind me if I haven’t wiped in a while. I sometimes have skid marks on my underwear, and after a few years of wear, holes appear around that area because of the acidic material that inevitably winds up there.

In the past year or two, it’s gotten worse. In the past year alone, I’ve had bloody stools at least five times. Four brighter red on the paper and in the bowl, and one that was dark and tarry. Sometimes, stools are a bit rough and crumbly, but most of the time are ok and smooth.

I also have lower abdominal pain/pressure on my right side that happens when i bend over to pick something up off the floor, as if someone is poking the eraser side of a pencil into my lower abdomen. This has been going on for five months.

I got covid two months ago and have not felt the same, and fatigue has gotten worse.

I’m concerned about what might be going on. Any ideas?

I should also add that at least once, the bloody stool was the day after leg day at the gym

Two other times were after a slight strain during BM

This calls for investigations to reach a diagnosis.

What is the color of blood that you see? If it’s bright red, it may be due to some lesion near the anal orifice. May be piles. However, if the color of blood is dark, it’s likely to be coming from deeper intestines. You also have some abdominal complains, again pointing towards some intestinal inflammation.

You need to get checked up by a GI specialist. He’ll order some investigations to reach a diagnosis.

42 y/o old female type 2 Diabetes which was originally gestational & never went away. 3 children who were born vaginally. Youngest is 16. Had a hemorrhoid ectomy done 7 yrs ago due to internal & external hemorrhoids. Other than going back into surgery about a week later bc my stitches burst & caused lots of bleeding, I have been fine. I also did iron infusion treatments last month. My iron saturation was a -4 I take stool softener from time to time. About a week ago I noticed that I have mucus coming from rectum & I have no control over it. It happens multiple times a day. Sometimes it’s a lot, it appears to be clearish, slimy & smell bad. What is happening? This is very uncomfortable & embarrassing.

I have been having leakage from my anus I assume for about 2-3 years it’s not a lot and it’s not everyday mostly notice damp or wet like feeling between buttcheeks after walking a lot. Sometimes it’s just sweat other times it’s a yellow/ clear liquor not mucus like more like water or pus. Using the bathroom is not a problem and I usually go pretty fast. It’s never enough to stain underwear but it’s enough to notice it. I’m 25 years of age and a male I don’t really have any other symptoms of anything. When I do notice it if not cleaned fast it gets a little itchy but goes away after cleaning. Occasionally it happens in small amounts when I pass gas or if I’m holding a poop in but that’s only occasionally.

No I have not, sometimes when I clean my butt fast and ruffly it might get irritated and might have a small speck of blood but like something so small like a pen needle point ( this dosnt happen often at all) only when I clean myself ruffly or with like cheap toilet paper

No no swelling or pain at all. Just leakage sometimes, if I clean myself to rough or with cheap toilet paper after a while it might have a small speck of blood like pen point small but that’s it and that’s in extreme rarity I can usually feel when my butt is irritated from that but no pain just like itching a little but only when irritated ( leakage dosnt happen all the time)

It’s unlikely to be cancer, from whatever symptoms you’ve listed.

Looks like your lower bowels are inflamed from inside. They kept oozing out some fluid throughout the day. This may be due to some inflammatory condition affecting them or else may be linked to some irritant you’re consuming in your diet.

Common irritants include excess alcohol, tea/coffee or other sources of caffeine, excess spices, garlic etc.

Try switching to a blander diet for some time. Consume plenty of ripe seasonal fruits and cooked vegetables. Drink adequate water throughout the day. Avoid pungent spices like garlic, etc.

    I’m a 14yo male

I have a clear watery liquid coming out of my anus… it doesn’t happen all the time and there are no other symptoms with it. I was just concerned because I saw a lot of bad things it could be.

Okay, let me explain this better. I know that the anus produces a fluid to allow poop to come out easier, I think for me I’m just producing to much and at weird times.

Appear to be excess of normal anal secretions. There may be something in your diet that’s stimulating it. Avoid excess tea, coffee or any other source of caffeine, excess spices, garlic etc.

Likely to resolve on its own with time.

Could be nothing in your diet that’s coming out undigested. Worms are usually mobile and also give symptoms like itchiness, redness etc.

      Stool analysis may reveal it.

If it’s only black specks in toilet paper no itching or redness just specks will a stool analysis be the only way to find out what it is

Yes, that’s the only way to confirm.

It’s likely to be some undigested tiny seeds of some berry or some other fruit you are taking. Else, it could be fungal spores.

If this is recurrent, better to check it out.

I have pain in the area by and in my anus when trying to defecate, when I move my legs in certain ways, when I urinate if it happens to touch that area… then today I randomly felt as if I was leaking out of my anus, I rushed to the restroom and it was a yellowish liquid.
I wash regularly as well as changing undergarments regularly. My diet isn’t the best, but most of the time I eat healthily. Sometimes the pain will go away for a week or so and then return a little while later.

I have a history of constipation dating back to a really young age and I am 18 years old. I don’t have any digestive issues as far as I know, though my mother has IBS.

Though it’s not troubling you at the moment, constipation is not a nice condition. It makes you prone to conditions like piles etc with increasing age.

Better to get rid of it gradually by natural methods.

Avoid junk foods. Take plenty of seasonal fruits and vegetables in your diet. Whole grains and fibers are a must. Plenty of water throughout the day.

hi, I am a 15 year old female. I recently fasted for two days and last night I was watching TV and felt like I had to pass gas but a neon yellow liquid came out? This has never happened but I am wondering if it’s from not eating enough?

Could be some temporary inflammation in your lower bowels. If there’s no other problem, you may ignore this sign for now. Likely to subside with time on its own.

26yo male, recently I find some sort of leakage in crotch area of underwear and I suspect the leakage is from rectum and I am addicted to anal itching (like lower part of the butt).
The leakage looks transparent and smells little like semen leakage. I once didn’t took bath for three days and the leakage area turned to green. Is this temporary or should I visit a doctor? or is there something I can do from home.

Persistent itching in the crotch area may be fungal. Are you using any medications to manage itch there?

Hello, I have a liquid coming out, I have never seen it before, but suddenly I feel wet or humid, and it evaporates but there is a strong smell, like poop, sweat or like a garbage can…the first time my stomach sounded like diarrhea and when I went alone it was clear thick mucus with a very bad smell… since then, my anus sweats I have grade 2 hemorrhoids the proctologist says they are fine but I am sure I have a fistula or fissure because it hurts… when I feel like When I fart or have stomach pain, that penetrating smell escapes me, I don’t even realize that something is coming out, just because I feel wet and in pain.

You need a physical exam by a doctor for diagnosis.

Fissures are usually very painful. However, a fistula may be present with discomfort only.
