Cyst in Right Adnexa and Conception Problems

Q: Hello Doctor, I took my wife to a gynecologist to know about a problem, as to why my wife is not getting pregnant? At that time the doctor took some blood test for both of us and a sperm test for me only.

There is no problem with blood report and my sperm count. Other reports are normal. After that, the doctor took an ultrasound, that reports-
Uterus: enlarged in size -10.2 * 3.2 * 4.2 cm normal outlines and echo texture.
Adnexa: a cyst is seen in the Rt. adnexa, measuring -1.2 * 1.2 cm.
Minimal fluid in POD.
Impression: Bulky uterus.

My question is-
Is it a problem getting pregnant in presence of a cyst? How much time will it take to be normal? Is this a main problem for not getting pregnant?

Prior to getting pregnant, your wife needs to get treated for the gynecological problem she has.

An adnexal cyst may be the cause of infertility. Even if she conceives with the untreated cyst, it would be unsafe to proceed along with this adnexal mass. It may interfere with pregnancy.

What Is an Adnexal Cyst?

The structures on the side of the uterus are called adnexa. This include:

  • Ovary
  • Fallopian tube
  • Ligaments on the sides of the uterus

Any cystic (fluid filled) swelling in any of these structures can be called an adnexal mass.

A right adnexal mass may be a mass in the right ovary, tube or anywhere else in adnexa.

There are a few conditions where a bulky uterus is associated with a cystic mass in adnexa. Infertility may be a symptom.

One such condition is endometriosis.

Endometriosis

Uterus is normally lined by a layer of cells called the endometrium. Sometimes, this endometrium grows in areas other than the uterus. This condition is called endometriosis.

It may happen in the ovary to give a cystic swelling.

Symptoms

Symptoms a patient experiences are as follows:

  • Pain in the pelvic area
  • Disturbed menses, heavy bleeds
  • Infertility
  • Pain during intercourse
  • Constipation
  • Weakness and tiredness all the time

Does your wife have any other symptom? Your wife needs further evaluation by a gynecologist to conclude a diagnosis.

Other Causes

There may be other gynecological causes also, of having a cystic swelling with infertility. They include:

  • Tumors of the ovary
  • Polycystic ovarian disease

She requires a full gynecology checkup. After that, the treatment would begin. The mass in adnexa needs to be treated before getting pregnant.

Mostly, such masses are treated by medicines (various hormones). It may take 3-4 months to treat it. Sometimes, surgical removal of the cyst is required.

She would require a lot of care and compassion from your side throughout the treatment.

    There is right cystic adnexal mass measuring 3.3 x 2.7cm. There is pelvic fluid in the pouch of Douglas.

    Conclusion: Right cystic adnexal mass.

    What is the meaning of this report? Does it prevent one from getting pregnant?

      As you can read above, there are many reasons of having an adnexal mass. It is important to find out the nature of this mass and its origin. Is it from the ovary or the tube and so on. Your doctor would investigate you to assess this.

      Not all masses of this type interfere with conception. Masses from the ovary may interfere with conception. So, such lesions are treated first before planning a pregnancy.

    Hello Dr. I’ve missed my period for 3 weeks now with negative pregnancy test. I went for scan 2 day and doctor said I have adnexal mass on my left ovary measuring 4.2 and has said I have to come back tomorrow for TVS for further examination. Am so scared, I don’t know what to do now.

      TVS or transvaginal scan is a safe and simple procedure. You need not worry at all.

      It is suggested that you go for further evaluation by your doctor and know the cause of your missed period.

    I had an abortion by manual vacuum aspiration and 3 weeks later I went for ultrasound and doctor said I have left adnexal cystic mass of 9mm. What is the meaning of this and can this prevent me from conceiving subsequently. Will this mass increase in size later on.

      Give yourself some time and wait for a regular pattern of menstrual cycle to establish itself.

      Only after you have at least 2 consequent normal periods, go for another conception.

      If the mass interferes with menses, it is likely to disturb your conception too. However, such masses usually do not interfere with conception.

      You need to keep an eye over the adnexal mass to see if it is growing in size. Bigger masses need to be removed.

    My name is Charles and I am writing you to confirm about my wife health condition- the reason why she is not getting pregnant. I took her to a gynecology and he told us that my wife have a fibroid and the meter is 110mm. But according to him, we should go for follicula tracking and when the result came out we found out that is was 110mm. There are two follicles measuring 13mm and 11mm located in the right ovary. Left ovary is normal in volume and presently unremarkable. Uterus is anteverted and bulky with a heterogeneous myometrium.

      Since she has a fibroid in her uterus and the myometrium is also heterogeneous, it is better to get treated before conceiving.

      All her problems reflect some hormonal disturbance. Levels of female hormones may be measured. A follicular tracking may be done to assess the overall hormonal status.

      Usually, such cases respond well to hormonal pills, given for 6 months to a year.

However, again I have started to get abdominal pains and my ultrasound shows multiple cyst in the right ovary largest measuring 7cm. Now, I don’t know what to do?       You need treatment before you plan to conceive. There is some hormonal disturbance in your body.

      Your doctor is likely to try hormonal treatments for around 3 to 6 months. This would make the cyst regress and restore normal menstrual cycles.

      Once the cyst start regressing, you may conceive.

    I’m 29. I’ve no child yet. Recently, I went to a gynecologist for irregular menstrual cycle. Previously, I took 2 emergency contraceptive pills for 2 different occasions. Dr. gave me ultra sound of pelvic organs. Report said: “Uterus is normal in size, shape and position. However, there is a cystic mass seen in the left adnexa containing homogeneous low level internal echoes with thin septum measuring about 5.6 x 5.7 cm.” Now, I want to conceive. Can I do so? Will it interfere with fetus in future?

      Conceiving now is not a good idea. It is advisable to get treated first for your menstrual problems.

      Plan conception after you get at least 3 normal menstrual cycles in a series.

      From what you have detailed, looks like you have some hormonal problem. This is likely to be linked with the adnexal lesion.

      Talk to your doctor about treating your menstrual irregularities and the adnexal mass. Hormonal pills may be considered for treatment first.

      Are you having normal menstrual cycles? If yes, you may conceive, there is no problem.

      Are you getting any other symptom, like abnormal bleeding, pain abdomen etc. Let us know for a better insight in to your problem.

      Yes, the cyst may appear in virgin girls also.

      It is common for girls to get functional cysts in your age. Usually such cysts regress on their own in 3 to 4 cycles. No medical intervention is required till you get any symptom.

      Follicular cysts commonly occur in your age group. They usually regress on their own with time. Read more on follicular cysts.

      No intervention is required unless you have any symptom. Let us know if you have any.

    Hi! I’m 33 yrs old woman married for 13 yrs. My Roentgenological findings…the uterus is anteverted and measures 7.8×5.4×5.0 cm.cervix is intact. The right ovary is unremarkable measuring about 3.1×1.4 cm while the left is poorly visualized. There is a cystic structure in the left adnexa measuring about 4.4×3.1cm.. No posterior cul-de-sac fluid, no ascites. IMPRESSION. left adnexal cystic structure, consider ovarian etiology. Normal sonogram of the uterus and right ovary. Can I possibly pregnant? We are longing to have a siblings.

      It is recommended that you get yourself treated first, for this cystic structure, and then plan conception.

      Your chances of conception are dim till you have this cyst.

      Moreover, if you conceive, this cystic structure may interfere and complicate your pregnancy.

      You need to take antibiotics. It may take around 10 to 15 days to get cured.

      If you are getting normal and regular menstrual cycles, you may go for pregnancy.

    Hi, actually problem is with my friend that she has an extra development of adnexa wall in her right ovary. It is about 10*15 cm. Due to this in her scanning right ovary is not clearly seen. Is there any treatment or operation for this and after her operation can she conceive?

      She has to get the adnexal lesion treated first, to assure safe conception.

      Such lesions are treatable. She needs to see a gynecologist. The doctor may try treating her with hormonal pills first. The adnexal mass may respond to hormones and regress gradually.

      If it fails, surgical treatments are available.

    I had a chocolate cyst of right ovary but I remove that cyst by laparoscopy for 2 months. Right fallopian tube is good. Dye test also good. My left ovary is good but left fallopian tube is blocked. Can I get pregnant?

      Yes you can get pregnant. Your right ovary and its tube appear to be functional. They are enough to make you pregnant.

    My wifes uterus measuring 87×38mm shows a hypoechoic mass in Lt Adnexa not getting separated from uterus measuring 50× 55 mm. It is a subserosal fibroid. The age of my wife is 36yrs. Is she able to conceive after getting treated? How much time it would take to get treated or to be normal?

      Fibroid are treatable. You need to see a gyne for this. The treatment is usually for 3 to 6 months. These are hormonal pills.

      Difficult to say. First get the condition treated. All would depend upon the results of treatment. You need one patent tube for conception.

    I went to complain in the hospital that my menstration days reduced from 3days to 2days and it’s not coming the way I want, So I go for Ultrasound which shows : There is a thick walled cyst in my right adnexum measuring 4.5cm*3.2cm.the right cyst has internal echoes. the left is free.But doctor told me I can’t be attend to now because I don’t Hv any symptoms yet..But am concerned about my menstration reduction,and my menstral cycle is accurate what can I do?     Dr my scan shows rt ovary 28 multiply 19mm
    And my left ovary not separately visualised however anechoic cyst measuring 40*32*30mm in left adnexa.
    No free fluid in cal de sac.
    What does it means.does this cyst dangerous or can create problem in conceiving???

      It may delay conception.

      However, usually one healthy ovary is enough for conceiving.

      You need to be evaluated by a gyne. There may be some hormonal problem which needs to be checked. An ultrasound may be required.

    I went for a abdominal scan in October due to some symptoms I have been having. The results read thus;
    The uterus is enlarged measuring 62mm in AP diameter. A fibroid mass is seen in the fundal aspect measuring 46mmx41mm in size. The endometrial plate is intact. An oval shaped, thick walled cystic lesion is seen in the right adnexium measuring 40mmx45mm in size. No calcification or solid component seen within it. The left adnexium and POD are free. Conclusion
    1.Uterine fibroid
    2. Right adnexal cyst.
    Please Doc, what is the way forward. Can I get pregnant?

      Conception would be difficult before getting these problems treated. Your doctor may try hormonal pills for some time. These pills are likely to make the fibroid regress.

    I’m not getting pregnancy from past 3years recently I had my pelvis ultra sonography right ovary is 3.0*2.8*2.2cm 9cc and left overy:4.3*2.2*1.7cm(8.2cc).largest follicle measuring 7mm in right ovary
    bilateral ovaries appears mildly enlarged in size with subcentimeter sized peripherally arranged follicles and relatively echogenic stroma
    a wlldefined anechoic cystic lesion measuring 2.0*1.7 cmin left adnexa which is separately seen from left ovary….. left adnexal simple cystic lesion -? paraovarian cyst. polycystic ovaries

      You’ll need treatment before you plan conception.

      This is likely to be a hormonal problem. Your gyne may give you pills to eat for 6 to 18 months.

      You need to first get treated for PID. Scan shows that the infection is widespread, affecting your vital reproductive organs.

      After treatment, you may try conceiving.

    I was put on dose for 1 week n later went back to check n it was no more

    Hello doctor! I want to concieve but i couldn’t so i went for an ultrasound on 12 day of my period..
    And this is my report says:
    Transvaginal sonogram done
    UTERUS :
    Uterus is anteverted normal in size measuring,
    L.S = 7.3 cms.
    T.R = 4.4 cms.
    A.P = 3.1 cms.

    Midline echo central.
    Thickness of Endometrium = 0.5 cm.
    No mass seen.

    BOTH OVARIES :
    Size of right ovary = 3.7 * 2.2 cms.
    Size of left ovary = 3.4 * 2.1 cms.

    Multiple small sized follicles are seen in both ovaries.
    A cystic structure measuring 2.2 * 2.0 cm is seen in left adnexa adjacent to left ovary. This could be left paraovarian cyst.

    Please answer me i could still get pregnant?
    Is my egg ovulating?
    What is the cure?

    Doc please I need an answer I had miscarriage last 5 months and since then I was unable to achieve pregnancy, I took clomid and paludel and my menses came out twice just the period of 11 days and am havhaving irregular cycle and I want to conceive so I went toscan and it shows that I have complex adnexa cyst about 8.6*4.5. Pls I want to conceive and what can cause it

    Hi,

    I had a missed miscarriage last year and ultimately had a d&c. I have been unable to conceive since and I just had an ultrasound, the report says Left adnexa-prominent venous lake. What does this mean and what is usually the treatment for this? The only “symptom” I have is right lower abdomen pain during ovulation. Cycles have been completely normal.

      Venous lake just means that they see a dilated vessel near your left ovary. The cause of this venous congestion needs to be evaluated.

        Normal sized anteverted non Travis uterus with normal middle endometrial plate measuring 9.4cm5.0cm3.9cm.there is no myometrial mass

    Hell Doc, l went for a pelvic scan and the result read thus:uterus is normal measuring 8.72cm*4.28cm
    Oval shaped mixed echogenic mass seen in the right adnexa measuring 5.15cm*6.19cm,shows multiple septae with thick wall and solid cystic.
    I ve been trying to conceive for 1year now, am 36yrs old, have never missed my period, always regular and had never any symptoms except mild pain in my mid section.
