Free Fluid in the Pelvis

Q: Follow up Question on the problem of free fluid in the pelvis and its management.

Is it required to get Ultrasound again? If so, how many days after should we get it redone after completing the medicines? If trauma is a reason, can it be found out by ultrasound?. Report says ‘USG Abdominal ultra sound scan- all parts are normal’. Is there any other special treatment required?

Reply:

If the ultrasound mentions ‘all parts normal’, there is nothing to worry. Trauma is usually ruled out by that.

Let us know the name of the medicine being taken, that would make the analysis easier.

In my ultrasoud report, its mentioned about findings of fluid in pouch of douglas. My age is 44 years. I was having heavy pain in lower abdominal region.
Dr. prescribed Ofloxacin + Ornidazole Tablet and Fluconazole 150 mg for 5 days.
Is it required to retest for ultrasound to check for presence of fluid?

Yes, that’s better. Get a scan after a month of treatment, as the fluid may take time to get absorbed.

Likely to be due to endometriosis. Treatment for this condition would solve the problem. The fluid would get absorbed on its own with time.

I have Crohns (last 3 year). My age is 28 years, female. Biologics did not work for me and I sm considering surgery for 2 strictures in (one passable on Rectum and the other in Sigmoid, is tighter, passable with difficulty. I have free fluid found with pain in the Cul de sac. Can Crohns cause that. Other organs are clear and unremarkable.

Chronic cases of crohns disease may show fluid in cul de sac.

Presence of fluid there suggests active inflammation in any of the surrounding organs. Inflammation in bowels may be one cause. Rarely, there may be some tiny perforation or leakage from the bowel walls, leading to fluid collection in cul de sac.
