Pouch of Douglas

Pouch of Douglas is a peritoneal fold present behind the uterus. It lies between the uterus and the rectum. It is present only in females.

How is it formed?

All our visceral organs are carefully packed inside and covered by a membranous layer, called the peritoneum. As this membranes covers the internal organs of the body, it may form folds as it spread.

One such fold is the pouch of douglas, which is formed when the peritoneum covers the uterus and then climbs upwards to cover the rectum behind.

Pouch of douglas is also called cul de sac.  This area is the bottom most portion of the peritoneal cavity, therefore, when any fluid oozes in the peritoneal cavity, it tends to get accumulated here.

Causes of Fluid in the Pouch of Douglas

Any cause of inflammation in one or more pelvic organs may lead to oozing of inflammatory fluid from it. This fluid may get collected in the pouch of douglas.

Below are listed the common conditions that cause it-

Pelvic Inflammatory Disease

This is the commonest cause of having fluid in POD. So, it’s important to get investigated and rule out the possibility of having PID.

What is PID?

This is a broad term used for any infection in the female reproductive tract. Commonly, the cervix, uterus or the fallopian tubes get infected.

The commonest infections causing it are chlamydia and gonorrhea. However, after childbirth, miscarriages, or any procedural instrumentation done over the uterus, the pelvic organs may get infected by many more bacteria residing in the vagina.

What Makes You Prone to Get PID After Procedures Done in the Vaginal Canal?

Normally, the cervix creates thick mucoid layer that serves as a protection for inner organs, not letting any bacteria or other organisms climb in. However, during any procedures done lower down, this barrier breaks up, allowing easy access for bugs to climb up and infect pelvic organs.

Signs

  • A dull, but constant, pain down there. This may be there for weeks before you finally note it.
  • Discharge from the vaginal canal may be there in some cases. This may be scanty to profuse. It may look like a menstrual bleed that’s more dirty brown and always foul smelling.
  • Pain and burning during urination.
  • Pain during sex. You may also bleed after sexual activities.
  • Low back ache is very common.
  • In severe cases, fever, with or without chills.
  • Abnormal uterine bleeding in between menses or after sex.
  • Increased levels of total white blood cell counts.

Your doctor may examine you well and order a couple of test, including cervical swab, to make the diagnosis.

Ultrasound Findings

  • Fluid present in Pouch of Douglas
  • Organs may show no change in early stages. Later stages, there may be an adnexal mass, involving the ovary and tube, which are often indistinguishable due to inflammatory changes.
  • In chronic cases, there maybe thickening of various ligaments holding the uterus.

Treatment is treating the infection with the help of appropriate antibiotics. It usually takes a long course of combination pills of the infection to resolve. The fluid in POD is absorbed back on its own with time.

Tubo Ovarian Abscess

Abscess is essentially a collection of pus. Following PID or chronic infections like Tuberculosis, a pus pocket may get formed in the Fallopian tubes.

There is pain in the lower abdomen of the affected side. The patient may have pain, fever, nausea and blood work may show an increased TLC .

Inflamed tube oozes out fluid that gets collected in the pouch of douglas. This may show up in ultrasound.

Hydatidiform Mole

This is a condition in which a mass consisting of many fluid filled cyst like pockets occupies the uterus, or rarely the tube. The structure may or may not accompany a fetus.

It is formed when after fertilization of the egg, proper implantation and placenta formation is not seen. Instead of placental tissue, we see abnormal cysts developing around the fertilized egg.

Since the structure is not normal, an inflammatory response is triggered against it. Inflammatory fluid oozes out and may be seen in the pouch of douglas.

Ectopic Pregnancy

Implantation of a fertilized ovum anywhere else than the normal uterine cavity may be termed as ectopic pregnancy.

The commonest location is the Fallopian tube.

Endometriosis

Here, the uterine lining that gives menstrual flow every month, may be present else where than the uterine cavity. This abnormally placed lining also bleeds when the women menstruates. Some blood may get collected in the cul de sac.

Endometriosis may occur in the pouch of douglas itself. In such conditions, apart from heavy bleeds every month, dyspareunia, that is, pain during sexual activity, is another common symptom.

Peritonitis

Inflammation of the peritoneum due to any underlying disease may cause it to ooze out fluid, which may get collected in the cul de sac.

Ruptured Ovarian Cyst

It is common for cysts to get formed in the follicles of the ovaries during reproductive ages. Any such cyst, which has grown big in size, may rupture spontaneously. Fluid may get collected in the pouch.

    Pls there seems to be a hard lump on my cervix and another just at the tip of my cervix. It is slightly painful to the touch. I have the urge to urinate frequently sometimes with a burning sensation. I feel severe pain s in my lower abdomen, lower back, my two ovary sides and the worst is I feel a movement in my lower abdomen especially when I run. I had an ultrasound scan today and it said “fluid collection seen in Pouch of Douglas”
    What could it be?
    Is there anything to worry about?

        Its been a 2 week since i have been active and i just last week i had abdominal pain , some bleeding and nausea. i went to the doctor and they checked my blood and urine they said my blood and urine are clean. They did a ct scan and found out I have a trace of fluid in my pelvis. Does this mean i have PID even though they examined my blood and urine and im clean ? They prescribed me famotidine and i been feeling much better. Thank youuu

          If it’s PID, you’ll continue having lower abdominal pain. Other symptoms, like discharge from vagina, foul smell there, bleeding etc may also persists.

          If you have no such symptoms, this is unlikely to be PID.

      I’m 30. I have having irregular period and diagnosed for PID which I have been on antiobiotics. I had pelvic scan and the result shows-
      that the uterus is pear shape anteverted and normal in size for nuliparous
      It measures about 39mm*32mm*33mm L*W*AP diameters
      The outline is smooth with homogeneous echopattern
      The endometrium is thin and hyperechoic
      No intra and/or extra uterine gestational sac seen
      No myoma is noted
      Pouch of Douglas consist of fluid collection.
      Pls can you explain this result to me and will the fluid in the POD not hinder me from conceiving.

        Fluid gets collected in the pouch when you have an infection.

        As you take antibiotics, your infection would heal up and the fluid would also dry up. Once complete treatment has been taken, pregnancy is likely to be safe.

        Hi….please I have been diagnosed with PID after having a pelvic scan…everything was normal apart from having scanty fluid in the pouch of Douglas… I had been injected with ceftriazone and gentamycin…I was later given doxycycline and cefuroxime 250mg for 5 days…
        Please do u think this is enough to cure the disease??

          Commonest cause of having fluid there is some pelvic infection. In this light, the antibiotics are likely to work. However, you need to follow up with a repeat scan to confirm the results.

          I was diagnosed for the PID. Was given same injection and antibiotics. I was having brown discharge before I began treatment but now it’s blood coming out since I started the drugs. Old what does this mean?

            Not a good sign. You need to report this to your gyne.

            There may be a mass inside that’s leaking out this blood. Needs to be examined.

        Please I have been suffering from lower abdomenal pain and valva pain with discharge in my vigina after scanning I was diagnosed of significant amount of free fluid in my pouch of Douglas does it mean I will not be able to conceive ?

          You are likely to be carrying some infection in your pelvic area. Conception would be difficult until you treat this infection.

    So I went to the gyno today because Ive had normal periods but in between periods I had very dark brown/black slimy discharge (lasting 5 days at a time and very heavy, like a second period but much darker nad thicker consistency) and cramping no matter if i have my period or not.this has been going on for 3 month now. My Gyno did a vaginal ultrasound and took a little swab to send to the lab. I haven’t had intercourse in over 6 month, and have a history of endometriosis. but she said it was not that. she did see fluid in my pelvis area, I assume in the pouch of Douglas. I am wondering should I go see my generalist as well to get blood work done (iron-deficiency because of heavy period?) might I have something more concerning that can not be detected with a quick vaginal ultrasound and a little swab? ive got cancer issues in my family (colon, intestine).
    trying to not be paranoid but ebut im a little worried it might be something to investigate further.

      Unlikely to be malignancy.

      There appears to be some issue in your uterus. If it’s not endometriosis, may be you have a fibroid or some other problem in the uterus. Your gyne would be able to find that out.

      However, it’s not a bad idea to see a generalist to correct anemia. You are very likely to be having that.

    I have pain in my lower abdomen,i did an ultrasound 2yrs back saying mildfluid in pod n submucosocal fibroid 24mm*26mm,was prescribed antibiotics but I haven’t conceived what could be d problem.

      To assess fertility, you need to be thoroughly examined by a gyne. There are many factors that may be contributing to difficulty in conception.

      Your hormonal levels, condition of tubes, present size of the fibroid, need to be checked.

      Mild fluid in POD is likely to have been resolved by the antibiotic course. The small fibroid may have grown in size. It’s location is also important. Where exactly in the uterus is it located?

      A big sized fibroid or fibroids interfering with common sites of conception may make it difficult for the egg to implant.

    My menses flow for just two days and no drop the third day while on fourth day few drops. The quantity starts decreasing right from the first day. I started noticing this from 2009. Recently I was diagnosed for having mild fluid in the pouch of Douglas and questionable pid. I urinate often Doctor did scan for me and said my endometrial isn’t smooth. Is there a cure for this and can i still have a regular menses

      Your age?

      You may be having some hormonal problem, giving you a rough endometrium as well, that’s interfering with proper menses. You need to see a gyne for full evaluation.

    I’ll be 30,I do experience itching too, abdominal pain, irregular menses, and sometimes I bloat from the vaginal. I have used clotrimazole insertion several times but no difference. I was diagnosed of mild fluid in the pouch of Douglas and questionable pelvic inflammatory disease

      You need to go for an ultrasound lower abdomen, to assess the condition of the ovaries.

      PID would require a course of antibiotics to go.

    I have severe backache during periods which increased over and above the usual pain. Continuous bleeding starts after sighting clots for 3-4 days though periods is regular every 28 days and have never missed periods. Went to gynecologist and did scan result of which is fluid in pouch of Douglas and to consider PCOD. I have never had PCOD and mother of two never missed my periods. So how is PCOD possible and how to treat the fluid accumulation

      PCOD patients usually have some menstrual irregularity, excess hair growth, acne, and obesity. Do you have any of them? Did they check your hormonal levels?

      This indicates an infection somewhere in your pelvic organs. It’s strongly suggested that you take antibiotics for this. Don’t delay. This would only given the infection a chance to spread.

    Hello I am 37 year old with a history of uterine fibroids. I also had a myomectomy about 5 years ago .I had unexplained infertility and after a failed IVF treatment I got pregnant and miscarried at 7 weeks and now have a 15month old who was born at 30 weeks gestation . For the past 4 months I experience pain outside of my menstrual cycle and have had in between bleeding for the last 2 months . My recent ultra sound scan shows a large fibroid occupying the whole pouch of Douglas .I was told that fibroids don’t cause bleeding in between periods ( although I am not convinced this is so ). I am in constant pain and have been given no explanation or reason for my constant pain . I was given Esmya 5mg tablets to start on for 3 months to help shrink the fibroids which I haven’t taken due to the long list of side effects and I’m breast feeding in addition to the fact that I was not given an explanation as to what is causing my pain and bleeding which is what worries me more than the fibroids . Is there any thing more I can get done to get to the bottom of this .

      Something needs to be done about that big fibroid. If you can’t go for the pills, you’ll need a surgical resection. Blood loss this way cannot be afforded.

        I had Experienced A-sharp pain and got a tranvaginal ultrasound. The doctor reviewed the images and said it was nothing to worry about… no treatment needed . That I was good to have children any day…. she just said a string of pearls … that it was not a concern to the point for treatment or infertility issues .

    Am 33Years, I went for scan my uterus is very bulky and anteverted in shape, with heterogeneous echo pattern and measures about 104mm*64mm*63mm in length, width and AP diameter. The endometrial plate is not visible.
    Palpable mass noted within uterus, no adnexae mass noted. Moderate free fluid found in the POD.
    Impression intrauterine myoma (ii) PID pls what antibiotics can I take to cure this? I want to conceive.

      You’ll be given antibiotics to treat PID. Apart from this, your doctor may suggest going for a DnC. This is a small operative procedure, which would scrape off the myoma.

      It is common to bleed for a few days consequent to spontaneous abortions.

      However, if you bleed heavy, you need to see your doctor. The dose of medicines being given may be adjusted.

    Had LSH 7 days ago, fibroids were causing abdominal pressure, pressing into my back on the right side and a lot of other things.
    Three days ago I began to have low back pain to the point I had to take medicine for it.. Would like to know if the pouch of Douglas would have anything to do with this.. I’m mobile not doing too much just a little at a time but the back pain is lingering around..

      Just give it time. Your ligaments might by pulling and giving you the pain, as they adjust themselves in the new space provided.

      A heating pad may be used to alleviate pain. However, do not over use.

      10 to 15 days, approximately. The fluid gradually reduces in amount as your infection gets treated.

    Am a lady of 30yrs, started having severe lower abdominal pain which radiates to the back and burning sensation, chills even when the weather is hot, temp BTW 36 -370c, no vagina discharge, nausea /vomiting, loss of appetite, bloating, breast tenderness, dizziness and feeling of fainting, all this started on June. I thought it was pregnancy because I have been trying to conceive after my wedding on March this Year, I had pelvic scan which shows Non gravid anteverted uterus measures 71 mm45 me, 55mm in long, Ap diameter $ transverse diameter. Normal endometrial echo is homogeneous and no sign of fibroid mass seen, both adnexa are free. Urinary bladder wall is normal with clear contents. MILD FLUID COLLECTION in the POUCH OF DOUGLAS, I was given Iv ceftrixone for 2days, levofloxacin 1dly for 10days, azithromycin 1g start then 250mg for 2days, flagyl 400mg for 7days, diclofenac 50mg 2times dly. I took on June when I had the symptoms, had normal 4days menses in a 28days cycle , but this July my menses lasted for just 2days in a 30days cycle still presenting with the same symptoms, carried another scan still the same result but very mild fluid collection. Please, what do I do to tackle this as I want to be pregnant soon, and how long will it take to conceive?

      Fluid in pouch of douglas indicates the presence of an infection there. It’s very important to treat it before you conceive.

      You have already taken antibiotics for around 10 days. May be your doctor gives you more of these pills.

      Fluid collected there may take time to resolve. However, you should be free from your symptoms. Any fever, chills or pain would mean you still have the infection.

      Your gyne is the best person to answer this. She’ll examine you to see if you have any symptoms of infection. If not, no treatment is required. Medicines, in early pregnancy, may harm the baby.

      Only if there’s fulminant infection, you’ll need treatment.

      The baby is securely packed inside the uterus, so don’t worry, he’ll not be affected by minor problems going outside.

    I did for scan 6/7/2017 because I was having this feeling that am pregnant but my scan result said this: ( A non gravid, anteverted , normal sized uterus measuring about 62mm x 44mm x 48mm ( longitudinal x Ap x transerve ) with myometrial echo texture. Uterine outline is consistent and of border regularity. Endometrial plate is thickened. No intra or extra uterine gestation seen. No evidence of retained product of conception. Within the cul- de- sac, indicative of pelvic inflammatory disease. Adnexae are relatively free. No obvious gross ovarian lesion noted. Both ovaries are normal in size, shape and echo texture) please what those this means.

          You may be having a condition called Endometriosis. Further investigations are required for diagnosis. Your doctor may suggest laproscopy.

      In general, fluid collected in this pouch suggests some infection in the pelvic area. However, there are other conditions too, where fluid may get collected in the pouch.

      Your clinical symptoms need to be correlated to reach a diagnosis.

    I am 49 and have had sciatic- like pain on right side that can radiate down to calf and pain at lateral edge of right sacrum for 2 1/2 years. Pain is usually worse before and during period. No previous history or endometriosis. Lumbar MRI negative. I had a pelvic MRI a year ago and radiologist indicated there was fluid in the cul de sac, most likely from a rupturned cyst. A few months later I was diagnosed with piriformis syndrome and received treatment which gave me little relief. One year later I had a 2nd MRI and same area is showing up and looks exactly the same as what was diagnosed as fluid. Is it possible for fluid in the cul de sac to remain there for a year?? I find it more likely that it was not fluid to begin with and may be something else. I have no abdominal or pelvic pain. I have no indication of an infection…. just sciatic pain and gluteal pain along the lateral right side of sacrum.

      It’s essential that you get your female parts examined by a gyne. Just a routine check up at the age of 49. This should include an internal exam, pap smear. Ultrasounds are already done.

    I have been having lower abdominal pain, pain during sex, and whenever I have sex with my partner the semen leaks and I have been finding it difficult to get pregnant and my menses last for three days but the dates always change.
    I went for a gynecological scan and was told I have normal uterus but some fluid found in the pouch of Douglas. Please I want to know what the problem is, but my doctor prescribed cefuroxime 500mg for me.

      You may take this antibiotic as prescribed. Also, an ultrasound lower abdomen needs to be done to rule out any chances of endometiosis or any other condition.

    Pelvic ultrasound reads:
    the uterus appears bulky in size, hypoehoic and heterogeneous in appearance large ill-defined calcified sub serosa fibroid with irregular borders extending upward towards the lower abdominal cavity measuring (97.1×99.0)mm in diameter. No pregnancy detected on ultrasound imaging. Pouch of Douglas seen with free fluid. Both ovaries were not visible due to enlargement of uterine fibroid growing within and outside the uterine walls. The endometrial echo was absent. There was volume of dilated serous fluid collection seen within the recto uterine cavity. Please this is the ultrasound results. I only have lower abdominal before and during menstrual period and I never hospitalized or treated PID before.

    I’ve been feeling pains in my lower right abdomen coupled with nausea, stomach pain and my heart beats fast. I went to the hospital next was told to go for an ultrasound. And these are the results
    Uterus: Retroverted in position and shows uniform myometrial echoes measuring (101.3 × 30.6 × 53.0)mm, and shows an intramural myoma measuring (38.8×35.9×33.9)mm. No defined gestational sac seen

    Ovaries; Both not Appreciated.
    Adnexa: scantly fluid seen in the POD

      Needs to be followed up by a gyne.

      You’ll be given a course of antibiotics for the fluid found there. Hormones may be given to reduce the myoma.

      Usually, women with PID conceive after they are treated for the infection.

      Situation where there’s a problem are ones where the fallopian tubes gets blocked due to infection. When you have PID, bacteria can get into the fallopian tubes or cause inflammation of the fallopian tubes. This can cause scarring in the tissue that makes up your fallopian tubes.

    Hello.
    I was diagnosed with PID 3 months ago and given multiple antibiotics. Was diagnosed with hydrosalphinx which later calmed down. All the while I had luliquid in the Douglas Pouch measured first at 12 cm, then 4×4 cm, and is now 3,8×12 cm. I no longer have signs of an infection and the abdominal pain is still there but gets better as time goes by. I am told that the red blood cells in my urine come from the liquid (culture is negative) and that the liquid can dissolve in time. Bloodwork shows no inflamation. Is it possible for the liquid to take so long to dry out (now on my 4th mont) or will it have to be removed? The symptoms are getting milder, but are several months too long or is it normal? Thank you very much for your assistance and advice.

      This really takes too much of time to resolve, 5 to 6 months. However, follow up scans may be done to actually see if the fluid is reducing or not. Also, has the hydrosalpinx resolved completely.

        Yes, I was told today by the ob gyn that the fluid is still there but is reduced, and that they would leave it to resolve itself over time. They also saw some “sand” in the bladder.

          May be you can give it some time to resolve by itself. Be observant. If you find some symptom, like ache lower down, discharge, burning during urination etc., you may inform your doctor.

    Morning sir,I had miscarriage last year August when my pregnancy was 4weeks and my doctor advice me to do evacuation which I agreed but before then I did a pelvic scan and the says “About 2 well defined echo penicillin solid masses measuring 48mm×44mm (antero-fundal,subserous)and 34mm×26mm (posterior, intramural)are noted within the corpus uteri. Features are suggestive of MULTIPLE UTERINE FIBROID. Both ovaries appear normal. The adnexae are free bilaterally. Seizeable echo free fluid collection is noted within the pouch of Douglas indicative of patient on menses /PID.
    Since after the evacuation have not been able to conceive .my doc said I should repeat the pelvic scan again bcos the previous pelvic scan was done September 2015.And here is present pelvic scan result:The uterus is bulky with smooth outline. There are fundo-anterior and fundo-posterior intramural fibroids. They measure30mm/34mm and 28mm/31mm respectively. Endometrial layers well defined. The ovaries are normal in sizes.No free pelvic fluid is seen. And this is also my Hvs and urine mcs result :urine culture (yielded no bacterial growth after 24hrs of incubator at 37°c),Hvs culture (yielded scanty growth of E.coli after 48hours of incubator at 37°c.senitivity :ciprofloxacin+++,streptomycin +++,sparfloxacin+++,ladicef+++,ofloxacin+++.My question is that can this infection be the cause of delay of pregnancy after the evacuation? Please I really need ur advice.

      Well, the fibroid may also be interfering with implantation of the fertilized egg.

      They are located at places where the fertilized egg may plan to get implanted. So, their presence may be hindering your pregnancy.

    Hello, I did a pelvic scan on the 12th September 2017 and it says;
    Nongravid anteverted uterus with uniform myometrial echopattern. It measures 56*35*27mm in L*W*AP Respectively. The endometrial layer is thickened. There is a left Adnexae hypoechoic cystic mass of about 23*18 mm in size with no septation or debris seen within. this is taken for a physiological ovarian cyst. The right Adnexae is free and normal in appearance. The general uterine outline is regular and consistent. The pouch of Douglas contains some fluid collections? PID
    Conclusion
    *Acyesis
    *left functional ovarian cysts.
    pls could there be anything wrong with me like pelvic inflammation. The last time I was supposed to see my menses there’s was alot of stretchy discharge like ovulation discharge instead of menses and two weeks later I spotted blood that lasted for 5days to me it was like my menses But it was too light. And could u please interpreted the result for me. Thanks

    Hi Doctor, I’m 25 years old and was diagnosed with PID in January this year 2017. I was having pain during sex, lower abdominal pain especially on my left side, backache, nausea and general body weakness. A pelvic ultrasound revealed that there was a moderate collection of free fluid in the POD. Since then I’ve been on different combinations of antibiotics to try and combat this but it comes back afterwards. It’s 8 months now, and I’m worried. What can I do to find a lasting solution?

    I only have this frequent milkish stretchy discharge with mild or no smell, its been more than a month now buh comes and go. I’ve no pain except that of 13/09/2017 which seemed like ovulation pain. I’ve not had sex since these symptoms began but I don’t av pain during intercourse.

      This may be an infection. However, we need to confirm. You gyne would like to examine you and order a few blood tests and a cervical swab, which may be sent for culture.

      Infection in the pelvis is a very common cause that interferes with pregnancy.

    I was diagnosed with PiD last year,after series of test it showed no infection. I did anothe sacn with showed the fluid in POD increased from 2cm to 11cm and blood test showed little bacteria in blood .. I was given antibiotics to treat both. Went for blood test and scan twice which den showed fluid has reduced and blood test came out clear.
    For the past two months my menstation is light and last 2days.
    I had itching down dere and increased amount of white
    cheesy discharge 2days ago with lil pain in lower abdomen., I ve done hvs test (result not out).
    Doc can PID reoccur… ? And is there a permanent solution. Am scared.
    I turned 30 this year.

    Please, I was having lower abdominal pain, vaginal discharge and severe lower back pain. I went for scan and the result was, the uterus measures, 6.1cm×3.9cm×3.9cm, the endometrial stripe is of uniform wall thickness, no focal mass, collection gestation seen. No adnexal mass seen. Minimal fluid in pod seen. The visualized urinary bladder is normal. No urinary stones, urinary bladder wall thickening.Impression was, minimal fluid in pod? Otherwise normal pelvic scan.

      You appear to be having an infection in some of your pelvic organ, may be the uterus or tubes. Need to be be on antibiotics as early as possible.

      Apart from antibiotic courses, lifestyle amendments would help. Avoid alcohol and smoking. These lower our immune powers.

      Boost up your immunity by taking plenty of fresh fruits, vegetables every day. Drink plenty of water, this would wash off all germs from your urinary tract on a regular basis.

      Also, cleaning well down there is very important. Always clean from front to back, never the other way round, to avoid contamination of the vaginal opening with fecal matter. Washing with ample water down there is a better way to keep the area germ free.

    I am 58 with a prior history of Endometriosis with the treatment of cauterization. I have had two pregnancies. I have been in menopause for 8 years. I get frequent bladder infections and have kept on top of these with scopes and antibiotics. I have suffered over the years with pain even after menopause. When I get this pain my bowels get messed up. I just had a cat scan and ultrasound of this area and all that comes up is I have a small amount of free fluid in the Pouch of Douglas and a few tiny peripheral echogenic foci are seen which may represent ovarian calcifications in both ovaries, normal arterial and venous flow seen within Free Fluid. Small simple amount seen within the posterior cul de sac.

    My question is, would this Free Fluid be enough to upset my bowels and keep this heavy feeling down in my groin area. Does endometriosis still flare after menopause and this could be my issue? Right now I feel like I’m going through ovulation. I do not get any discharge and everything else seems to be fine except for pain and bowel upset. I am so frustrated with all the years of pain and discomfort. Thanks so much in advance for your time.

      A small amount of fluid is not enough to explain your symptoms. It’s very likely to be a flare up of endometriosis.

      Yes, you may get Endometriosis after Menopause.

      Endometriosis. as you understand, is having inner uterine tissue outside the uterus. This may occur over the ovaries, tubes, intestine or else where.

      This abnormally placed tissue secrete just like your uterine tissue. However, while the uterine secretions escape out via the vaginal canal, secretions caused by endometrial tissues are trapped in the pelvic area and produce pain and inflammation.

      After menopause, female estrogen levels drop down, but doesn’t become zero. There’s still some estrogen in the body which helps this tissue to grow and secrete.

      You may have to go for complete surgical resection of the organ to get relieved.

    I’m 19. I’ve been having discharge from my vagina for years now. About 5 years ago I started my menstrual cycle. I was having the discharge but I began using a panty liner because I was made to believe vaginal discharge is normal due to ovulation. Since I wasn’t sure and I’ve been experiencing itching and discharge everytime when I’m not in my menses I visited my gynecologist yesterday. The ultrasound scan revealed a normal uterine size of (7.79×4.96×5.92)cm. Anteverted and homogeneous in echo pattern. Empty endometria cavity with thin lining. Normal ovaries. Significant volume of free fluid seen in Pouch of Douglas. His impression was Pelvic Inflammatory Disease. I was given drugs to take. Should everything be fine when I’m done with my medication? Or since it has been five years now I shoul be worried.

      Since all your pelvic organs look normal, as visualized on ultrasound scan, you’re likely to be alright after medications.

    Hi, I’m 26 years old. I treated PID 5 years ago but when I went for another scan, I was told it was an ovarian mass. The doctor said I should neglect it since I don’t have any symptoms. Presently, I missed a month period, I have being having cramps for about 2 weeks yet I’m not on my period and I haven’t been sexually active for about 3 months. I have never missed a period before, my discharge turned stringy and brownish and later returned to normal. I am yet to visit a gynaecologist though. Do you think it’s something to be worried about?

    I am 29 and I’ve been trying to conceive for 7 months now.I’ve been on depo for 5 years and I have an 8 year old daughter.To flush out depo from the system i’ve used some herbal concoction once in a while since Feb.I’ve been experiencing severe abdominal pain for 5 months now which come once or twice in a month after long call.This promptef me to seek medication and below are the findings:

    Urinalysis:No infection seen.urine clear.
    Pelvic ultra sound scan:uterus retroverted,non gravid,normal in size and echogenicity,no fibroids seen.
    Endometrium with normal wall thickness.
    Myometrium echo pattern appearing normal,no obvious mass lesion seen.
    Both ovaries seen an normal,no adrexial mass seen.
    No free fluid seen at POD.

    What could be the cause of me not conceiving now that everything seem okey?

    Hello doctor, on 20th of August, I had pain and bulging on the right side of my lower abdomen. I could feel the bulge at my navel and it lasted for over 30mins, then milder pains afterwards. I had a scan which says I have 2 subserous myoma and clear fluid in Douglas pouch. My Dr diagnosed fibroid degeneration and was placed on Augumentin, metronidazole and dictofenac. I got relieved, about 2days ago I started feeling pains on the same side of my lower abdomen and also a serious backpain. I want to know is it possible to ave fibroid degeneration twice within a month interval. Thanks.

    For a very long time I’ve been experiencing menstrual irregularities,sometimes I have cycles of 30 to 31 days and during each months I observed my period doesn’t flow freely (well) it last about 2-3days at times .on the onset it’s usually dark brownish in color before the day expires its changes to a bright red and i noticed there are traces of small clots in it. I’m just 29 and have never conceive for once,this has been plaguing my mind for a while. pls help me out,I don’t want to be a barren.
    About 2 years ago I went for a scan to find what is actually wrong with me.the result stated thus; The uterus is non gravis,anteverted,of normal size with uniform of myometrial echo-pattern. It measures about 65mm x 33mm x 41mm in LXTXAP diameter respectively. Endometrial plates appears normal,No uterine lesions noted,There is an anechoic spherical density noted in the right adnexa measuring about 35mmx33mm in size it’s not septated and has well defined border,Left adnexa is free.No fluid in POD and general uterine is consisted…. So I went for another one today to clarify the previous result and this is also what I came back with ; non gravid anteverted uterus measuring 88x68x32mm[LXTXAP],Endometrial echotexture is in proliferative phase. Adnexa bilaterally free.There is a non septated right ovarian cyst measuring 37x52mm and there is fluid in the pouch of Douglas….pls what I need to do and if there is any medication that can help me.thanks in anticipation

Impression : the features is suggestive of , Acyesis, pid, hvs and urine MCs is advice, am trying to get pregnant for 8 month now , pls what is wrong with me because am worried

          It may be due to the cyst that has gone away. However, other causes of having fluid there, like infection, need to be ruled out.

      You need ot follow up with a gyne for diagnosis.

      This could be a pelvic infection or there may be a mass in your left side adnexa.

      You’re likely to be having some infection in your pelvis. Follow up with a gyne. You need antibiotics at the earliest. Delay in treatment would give time to the infection to spread.

    I m 26 yrs old n 49kgs 2 years back i was diagnosed with pcos then i took POP for 6 months then I stopped medication n again menses didn’t come itself so again i took POP then COCPs given by doctor but due to severe nausea i stopped taking COCPs without informing doctor n then i did not get menses n now I m taking homeopathic medicines since 1 & half month for pcos n I didn’t get my menses after 24/7/2017.from 2 days i m having very mild pain in right side waist only.then i went for sonography n it showed minimal fluid in POD & rt ovary 19cc lt ovary 12 cc.do i need to be worried about minimal free fluid in POD? 1 year back sonography showed rt ovary 19cc lt ovary 23 cc n mild cervicitis and at that time doctor did not give any antibiotics for cervicitis

        Yes i m married n yes i m sexually active n I noticed pain in right side waist during intercourse prominent not too much but i can feel that its specific in right side n when i was taking POP n COCPs i was getting my menses but when i stopped taking them menses didn’t come after that n right now from 1 n half month i m taking homeopathic medicine to get my menses n still i didn’t get them n homeopathic doctor said it will take some time for me to get menses but right now i m concern about that fluid in POD…is that the reason of my pain??n i did sonography after i noticed pain…(pain is mild it doesn’t affect my routine but i m just worried about that)

          You could be having a mild infection in your doctor. An antibiotic course may be started.

    I am 30 years old. regular 29 days cycle. went for pelvic scan cos am ttc. the result7 says: uterus is normal size and anteverted with regular outline and homogenous myometrial echotexture. it has an AP diameter of about 3.5 cm. the endometrium shows a homogenous thickness. closed internal cervical is. the ovaries are normal in sizes and stromal echotexture. significance fluid collection to a depth of 1.1 cm was noted in the pod. Please what does it mean.

      You may be having some pelvic infection. It’s likely that the presence of infection interfering with your conception.

      Appropriate antibiotics need to be given.

    Iam 24 years old.. about a month ago i have done a sugery for removal of ectopic preg. without removal of the tube its size was 6cm that make sever internal bleeding and their was mild pain only which make doctors not notice it fast … now I still have the same pain ,pain during sexing and always have foul smelling however I have finished my medication course.. when I make scan it appear that there is minimal fluid in douglas pouch with normality of all internal organs and ovaries.. my doctor didn’t comment and said that is normal
    I don’t know if I still have a serous problem or infection and my conseve in the future will be normal or not?

      Apart from fluid in the pouch, you also have symptoms of pain and foul discharge. There are chances that you’re still carrying some infections and antibiotics may be needed.

    HI, I’M 27 N AV BEEN TRYING TO CONCEIVE FOR THE PAST 2 YRS. MY DOC ASKED ME TO DO AN ABDOMINO PELVIC ULTRASOUND. THE RESULT STATES NORMAL BUT THIS: PERITONEAL CAVITY-NORMAL GUTS, NO DISTENTION. NO ASCITES. ADNEXAE/POD: THERE IS SIGNIFICANT INFLAMMATORY TISSUE REACTION IN BOTH ADNEXAE AS WELL AS IN THE POD CAUSING POOR INTERFACE BTW PELVIC ORGANS. NO FREE FLUID COLLECTION IN THE POD. NO PELVIC MASS. FEATURES OF CHRONIC PID.I’M ALSO ASKED TO DO VAGINAL LAB TEST WHICH I STILL AWAIT. I DONT AV ABDOMINAL PAIN THOUGH I AV ITCHY VAGINAL WHICH GOES VERY RED AFTER ITCHING N PAINFUL IF I AV SEX AT SUCH TIMES. BUT NOT REALLY RED IF LEFT UNTOUCHED AND IT HAS BEEN A WHILE NOW, THOUGHT IT WAS YEAST INFECTION. ALSO, JUST FINISHED USING ANTIMALARIA N ANTIBIOTICS. NO ABNORMAL OR FISHY DISCHARGE.       There appears to be some infection in your pelvic region, that has affected your reproductive organs and is likely to be the cause why you’re not able to conceive.

      You may be prescribed antibiotics for this.

    Went for scan (PID) 2years ago. Am TTC Also feel pain during sex, deep Penetration. The result read.USS shows a normal sized uterus with AP diameter of 4.ocm. The uterine outline is regular. Myometrial texture and Endometrial thickness appear normal. Both ovaries appear normal in sizes and structure. Both Adnexa appear free. There is fluid in the pouch of Douglas with depth of 1.3ocm. No fibroid. No Cysts.

      There’s still fluid in the pouch of douglas. Needs investigations to know from where it’s coming.

      May be some infection still, or a condition called endometriosis.

      Do you have any of these-

      pain in lower abdomen,
      Fever
      Backache
      Any disturbance in menses
      Abnormal discharge or smell down there?

Got married last year and been trying to conceive.Sometimes I have irregular periods with my cycle sometimes being 59days.I went for a pelvic scan and the result I got is this ” A normal sized anteverted uterus with homogeneous my metrical echo pattern. The endome trial plate is intact.Significant fluid collected noted in the pouch of douglass”.So far I have no symptom.it was my irregular period that made me go for a scan. What does this mean and does it have an adverse effect on my reproductive organs?

      You need to be investigated further to know the cause of this fluid collected in your POD. This is definitely the cause why you’re not able to conceive.

      It could be an infection/ hormonal problem.

        Yes pain in back, vaginal discharge & very heavy periods with large clotting. The ultrasound was for the heavy periods & I should have added performed 6 months ago. Nothing diagnosed but was put on the pill to stop the heavy flow (which has reduced by a little but still very heavy).

          You may need to go for a DnC (dilation and curettage) procedure for diagnosis.

          Also, there appears to be some infection in the pelvis, that’s giving you the vaginal discharge and fluid in pelvis. Antibiotics may be required. Talk to your doctor about it.

    Pls there seems to be a hard lump on my cervix and another just at the tip of my cervix. It is slightly painful to the touch. it is had for me to go tiolet nd my fulse use to be very hard .I have the urge to urinate frequently sometimes with a burning sensation. I feel severe pain s in my lower abdomen, lower back, my two ovary sides and the worst is I feel a movement in my lower abdomen. I had an ultrasound scan today and it said “fluid collection seen in Pouch of Douglas” nd Increased bowel gas pattern seen at the epigastrice region.
    What could it be?
    Is there anything to worry about?

    Pls there seems to be a hard lump in my left side of my stomach and another in centre of my stomach. I feel bloated always and steady stomach pains. some time when i touch my stomach i feel pains. My Mensuration is not flowing well. It is slightly painful to the touch. I have the urge to urinate frequently sometimes with a burning sensation. I feel severe pain s in my lower abdomen, lower back, my two ovary sides and the worst is I feel a movement in my lower abdomen especially when I run. I had an ultrasound scan today and it said “fluid collection seen in Pouch of Douglas” and Also Increase bowel gas pattern seen at the epigastric region
    What could it be?
    Is there anything to worry about?

      There’s an infection in your pelvic region. You need to be on appropriate antibiotics at the earliest.

      Apart from this, there appears to be some mass in your uterus, may be a fibroid. Does your U/S say anything like this?

        No that is the reason why i go for scan but they said that they can see any thing. That i should not worry about it . But am still seeing it when ever i press my hand towards that place, especially early in the morning and i can’t lying down with my stomach as i use to do before

          Well, start with antibiotic medications first. Let’s see if it improves. May be something gets revealed in follow up evaluations.

    Hello Doctor,
    I usually have pain during sex,which causes my Vagina to swell and also be sore, I have gone for several test and my gynecologist said at that time there was no reason for such thing after the test I was referred to a psychologist and then a psychiatrist he felt it was something of the mind ,but it has persisted and when ever I go to complain d doctors don’t seem to understand what it is and this started in the year 2008 or there about, fast forward to 2017 my man and I tried getting pregnant it wasn’t forth coming so I went to the hospital complained and was asked to go for a pelvic and hvs test ,the hvs came back as negative that after 48hours of culture nothing was found but my pelvic scan came back with a normal size uterus seen,no ovarian cysts,both uterus free and fluid found in the pod and I was given ornilox, this was a month ago I went back today and fluid was still found in the pod but the radiologist wrote normal fluid found in the pod and my doctor is saying there is no problem. My question now is this ,is there anything like a normal fluid in the pod ? I thought the pod is suppose to be empty.thank u

      Most likely, you had that fluid due to the infection. Now, after the course of antibiotics you’ve had, the infection is likely to be controlled. The fluid in POD may take time to dry up on its own.

      Do you still have pain during sex? Let us know.

      Use enough lubrication during the act.

        Yes doc I do, and is not like i feel pain inside during sex but it is the act that causes my Virginia,clitoris and all the area down there to be swollen and sore thereby making the act painful Becoz my man organ will be touching those places and is always painful. But I noticed that sometimes while having my bath and washing down there I noticed that I my clitoris has peeled, whereas I wasn’t itching or doing anything down there.pls what do u think can be the cause of my Virginia getting sored during sex even when I am very lubricated

          Some women have semen allergy. Such symptoms may occur in those cases.
          Do you get pain when using a condom?.

        Hello Dr,
        I am 28 and was diagnosed with free fluid in POD, was given 5 days antibiotics dose.
        During the medication i was great and began getting the normal vaginal discharge. But a day after i completed the dose i began getting red and brownish discharge.
        should i be worried? or wait.
        today is the fourth day since i was done with the dose

          If your symptoms come back, it’s very likely that you’re not fully cured of the infection. More of antibiotics may be needed.

          Needs to be checked up.

      You appear to be having some abnormal growth in your uterus. This may be a submucosal fibroid or something else.

      Are you having any menstrual problems? Let us know all your symptoms.

    Yes since I started seeing my period at the age of 14 I have been having a painful menstruation, am always taking felvin later last year I stopped taking felvin and start taking parasitamol. But in my family all my sisters also had painful menstruation but it normally stop when dey give birth.
    My period last for 4-5 days, sometimes 3. Is always regular, I have not missed a period b4..
    Last year I noticed am always feeling feverish after ovulation for up to 5days it will stop….
    And I have been having waist pain for so many years after I got stressed up on duty, bending down always and lifting heavy loads…..
    For some months now I noticed I don’t see d egg stretchy discharge during ovulation.. But I will have one side cramping and see liquid mucous….. So I went for flocullometory, the scan said no follicle from both ovaries and but d ovaries are in a good shape and structure, no cyst , the uterus is normal..Myometrium is expanded by solid tissue mass with mixed echo texture and Ap diameter of 1.8cm submucous within the body of uterus. Exudate in the pouch of Douglass with depth of 2.0cm.. Plz doctor what does this mean… pls doctor I need good explanation..       What injections did he give?

      Your doctor is probably suspecting some infection in your pelvic region. he’s giving you antibiotics to cure it. Let’s see if you respond.

        I was injectected for 5days morning and night wit mixed antibiotics but I don’t know the name… And i was also given ciproflaxocin tablet,flagyl tablet and diflucan tablet….I have finish the treatment since November, and I went back for culture test this January dey said no bacteria growth..I went to scan and there is still fluid in d pod…am very confused as well as d doctor.

          If the culture says no bacteria, we can trust it. The fluid in POD may take time to get absorbed gradually in 10 to 15 days.

When I went for ultrasound scan I was told that there is free fluid in the pouch of Douglas.. So what is the best management for me to be relieved from such pricking pain?

    Good morning doc, early this year I was having a sharp pain in my lower abdomen and I went for diagnose. I was told that I have PID. and the doctor prescribed some antibiotics for me like zinnat,diclofenac, flagyl, and mycoten. I took all d medicine as prescribed but some months later,I started having pain in my pelvic area again.. and I went for scan and also endocervical swab. here is the ultrasound result . (the uterus is normal in size and outline,it measure 6.4 x3.7cm(LS xAP). the myometrial echotecture is preserved with intact endometrial plate echo. no adnexal mass seen , no free fluid in the POD. the liver, gallbladder ,pancreas, para aortic areas, spleen are normal. no ascites. normal bowel peristalsis demonstrated . ) and for the swab… couture yielded no growth. only yeast+ was dectected. and am unable to conceive . please what is hinder me from conceive. I can’t just wait to have my own baby

      Reports look normal.

      Do you still have any pelvic pain? Are your menses well synchronized?

    Hie Dr.
    I’ve gone through ivf and I am 8 weeks pregnant. It was an fet as I overstimulated on my cycle. Before transfer, I had a cyst but they said it wouldn’t affect implantation. I went for a scan and they found free fluid in the PoD. I was not given any advise if this affects the baby or not everything was normal with it.

        i have screened for all for all pelvic infection, the results is good i haven’t any infection but i have free fluid in the PoD. my the gynecologist prescribed me nothing !! gave me a period of 3 months to repeat the ultrasound.what do you advise me to do ?? especially 3 months for me will be long I can not pass without thinking a pod

          Well yes. If screened well, you may relax and repeat the scan after 3 months. Most likely, the fluid would get absorbed on its own in this time window.

    After I did a pelvic scan for ovulation test the result says “the adnexa are free and the pouch of Douglas is seen with significant free fluid collection” Also I urinate ofter and can’t even control the urine when coming and it come with a burning sensation. Please what could have happened?

    I usually have pain in the abdomen…sometimes burning sensation during urinating..back pain sometimes little discharge no fowel smell..Im 20 and never had sex b4…I was diagonise with chronic PID and have been given anti biotics.. orinzole plus..av use it for 5day buh Im still feeling pain in the abdomen… the fluid in the pod was measures 16.3 in depth…am worried abt what caused it

      Your symptoms clearly indicate an infection in your pelvic region. Continue with the antibiotics. As you complete the course, you’re likely to fell better.

      If not relived, your doctor may need to change the antibiotic.

    Hi, I have had several CT scans in the past, and in all of the CT scans it notes, “Fluid in the rectouterine cul de sac” and it says, “likely physiologic”. Last year my OB/GYN sent me to a general surgeon to have a biopsy of an inguinal lymph node, and I was sent to a hematologist/oncologist, lymphoma was ruled out and the biopsy came back as “inflammatory changes”. I was having low back pain at the time, and the low back pain has continued. The cause of the fluid was never investigated, and I am worrying that they have missed something.

      It’s not normal to have fluid in the cul de sac for a long time. The cause needs to be investigated.

      You might be carrying some chronic infection in your pelvic region. Any discharge or foul smell from the vagina? Any history of weight loss?

        No discharge or foul smell. No significant weight loss. I do have chronically elevated pancreatic enzymes that have remained elevated since a bout of acute pancreatitis in 2006. Apparently it’s just the new “normal” for me. Could this be a cause? I do have low back pain, aches most at night and in the morning, and have felt more easily fatigued.

          Also, on a CT scan they notes that a cholysystectomy clip was dropped during surgery and is in the paracolic gutter….could that be a cause of low grade chronic infection?

            May be, your symptoms, fluid in cul de sac and the swollen lymph node, all these are indicative of some inflammatory process going on in your pelvic region.

            This is most likely to be some infection or source of constant irritation.

    gudday i am 25yrs of age.i had a pelvicscan and d result read thus:the uterus is anteverted,nongravid,normal sized,it exhibits a uniform myometrial echo pattern.it measures about 72*49 mm in length and diameter.no uterine lesion seen.the endometrial thickness is normal.Thereis mild accumulation of fluid in the POD.
    OVARIES:normal in size and echotexture.
    IMPRESSION:mild pelvic inflammatorydisease
    CONCLUSION: Acyesis
    no uterin lesion seen,normal pelvis.
    but i dont knowtheantiobiotics to take..

      You need appropriate antibiotic to get rid of this infection. These are prescription drugs. So, need to visit a doctor for prescription.

    The right array appears slightly inflamed measuring 3.88cm x 3.87cm. Normal left array. There is mild fluid in the POD. ( pouch of Douglas). Suggestive of mild right oophoritis + mild PID.
    This is the diagnosis I was given after tests.
    What does it mean and what medication do I need to cure the same?

    Did my pelvix ultrasound yesterday after going to my docyor complaining pain and inflammation when sitting,pain in righy ovaries and right lower back that sometimes radiates to my leg. My results were:
    The uterus is normal in size and echogencity
    No obvious focal lesion seen on the iterus
    Both the ovaries are normal
    No adenexal mass seen
    No obvious intra or extra uterine products of conception seen.
    Small amount of free fluid is noted in the pouch of Douglas… (?) Secondary to PELVIC INFLAMOTORY DISEASE.
    My questions are:
    Is it the fluid in mu pouch causing thr pain when i sit? Because i read this pouch is located between rectum n vagina area n thats where i most feel the pain when sitting
    also does this mean i have pid? Why was there a ‘.. (?)’

      You are having the pain due to the presence of some infection in your pelvic region. The same is the cause for having fluid there.

      You need a long course of appropriate antibiotics.

            I have missed my periods last month i had all the pre pregnancy symptoms of pregnancy i had a ultrasound in the 1st week after getting 3 positive results from home pregnancy test kit they told me that they can’t find any sac but minimal pod collection is clearly visible. Have been having some pregnancy symptoms for a while now, my periods are lighter and pinkish in colour,heavy and full breast, urinating frequently,back pain and I have pimples all over my face,blood and urine test shows negative for the past three months and I did a transvarginal scan as the result is as follows; The uterus is normal in size and anteverted with a uniform myometrial echo pattern,the endometrial plate measures 14.9mm. Both ovaries appear normal in size and the outline. There is a cyst in the left ovary measuring 2.4×1.7cm. There is no fluid collection in the pouch of Douglas.. I don’t know what this means as I still feel very pregnant

    Good evening Doc,
    I am 37 years of ago, being married for about 9months. I had a miscarriage at 7weeks after about a month and half in marriage. Was told it was a blighted ovum. My period returned and since then no pregnancy.
    I did a pelvic scan recent, the result goes this:
    Clinical Indication: Abnormal uterine bleed.
    Scan reveal: Uterus is non bulky,nongravid and anteverted with homogeneous myometrial echo-pattern. A single fundalsubserous myoma is seen and it measures 29mm by 23mm
    The endometrial plate is intact
    Both adnexa are free of any cystic or solid lesion
    The ovaries appear normal and preserved
    Mild fluid collection in the pouch of Douglas
    Impression: Uterine myoma
    Fluid in POD.
    I need to note here that I am not bleeding so I do not know where the AUB result came from.
    Please I need your imput on this result. Considering the age and all, can all these affect conception? I need to have my babies please

    I went for a scan and it reads: uterus is normal sized anteverted non-gravid uterus . smooth and homogeneous myometrium. No fibroid seen.
    Endomentrial cavity is empty and intact.
    Adnexae: there is minimal free fluid collection noted within the POD
    Ovaries: bilaterally visualized and appears normal
    Urinary Bladder: Normal wall thickness. No pathology noted.

    I’m Fola, Good day doctor, I had an unexpected flow of my period on the 18th day into my cycle, I have never had irregular period, went for a test 3weeks bfr, was told I had E Coli, got it treated with Ciproflaxin tablet, went for a Scan the day I had the sudden flow and the result was given as follows
    Sonographic features shows a normal sized uterus, the Uterine outliner is regular, The myometrium and endometrium appear normal. There are pockets of Cystic mass in both Adnexea, it has a regular multi septate wall. This contains irregular Solid tissue as well as horizontal echogenic speckles. (USS feature suggestive of Inflammation). There are pockets of fluid in the POD with an AP diameter of 7.0cm. No cyesis or retained products. There is moderate tenderness to transducer probe pressure.
    I have been on Antibiotics drip and injections for 2days now, been trying to Conceive, should the Adnexea mass be a thing of worry??? Will be 29 and I’ve never had a record of pregnancy, though I do experience Sperm leakage after Sex. What should I do.

      There some sever infection in the pelvic region. You need to complete the antibiotic course to treat this infection fully before planning conception.

        Following up on the above explained, after my treatment my cycle stayed up to 27 days before another flow and I felt it has been normalized only for my next cycle after that to occur on day 19, I immediately went for a Scan and result was given as thus:
        The Uterine outliner is regular
        The myometrium and endometrium appear normal (Uss feature suggestive of inflammation)
        There are pockets of fluid in d pouch of Douglas
        No Cyst, No Cyesis or retained products.
        There is moderate tenderness to transducer probe pressure.
        I was told the pod is about 2/3.0CM as against the 7.0cm a month ago. Doh she said the sudden Cycle might be as a result of the Antibiotics treatment I just concluded. How true is this.
        Pls doctor, what could be the cause of my sudden/irregular cycle.
        What should I do. I’m still trying to Conceive.

            Infection is still lingering on there. In most cases, it takes time for it to go.

            This infection is the only thing you need to bother. Rest all appears fine. Your cycles would normalize as your treatment completes.

            Thank you Doctor, Should I start a new dose of Antibiotics or just increase my water intake and hope the previous treatment will clear it all as advised.
            If I need to repeat Antibiotics, which one will be advisable for me pls. I’m on Vit E, should I discontinue?????

            Just complete the prescribed course of antibiotics. Your doctor may suggests repeat testes after that to assess the condition of affected organs. If not healed, more antibiotics may be prescribed.

            You may leave vit E for now. Vitamin B and C are usually given to boost up immunity in most of the infective states.

            Good morning Doctor, Pls bear with me cos I’m a little disturbed, following up on the previous notes, my cycle for March dint come in 2weeks, like my previous cycle, it stayed full cycle, intact at day 30, then it refused to stop, though the blood wasn’t much but if I pee or have sex as at day 9&10 I see blood, then I noticed a lil swelling at my left side lower belly while doing self examination, I went to the clinic 2 days later and was asked to go for a different Scan, this includes pelvic and abdominal check, the result is given below:
            The first paper is 3 pictures of the swelling with some things written on each
            The 2ND paper reads
            USS shows a normal sized uterus with AP diameter of 3.8cm. The uterine outline is regular, myometrical texture and endomentrial thickness apears normal, Both Ovaries appear normal in sizes and structure. There is a complex cystic mass with septations measuring 10Cm*7.3cm in the left , also there is a cystic mass measuring 4.3Cm*4.2cm in the right adnexa. It has a regular wall and contain clear fluid.
            Pouch of Douglas is empty.
            No free pelvic collection.
            No Fibroid
            No cyesis or retained products of conception sighted.

            The other paper states the abdominal features appears normal. (Liver, kidney, GIT, Spleen, Pancreas, Urinary bladder,) normal upper abdominal Sonogram.
            Doctor Pls what’s wrong again, how did this happen and what should I do, though when I woke up his morning, I dint feel any pain there again, even my backpain relieved me, and the thickness felt in my left size feels really small compared to yesterday, I feel no pain in the right doh.
            Pls read through all my previous posts to helpcme better understand. Help me Pls, what should I do.

            There is no sign of infection now in your pelvis. All antibiotic courses are over.

            The adnexal masses may be due to hormonal disturbances. Your doctor may check your hormone levels are tract ovulation. If found disturbed, she may put you on pills for 3 to 6 months.

    Hey I’ve had laprotomy in Feb 2015 & laparoscopic cystectomy in Nov 2016. Both of the times for an ovarian cyst.
    I was on OCPs after the surgery, I had irregular period and then I’m overdue 16 days this time. I went for an ultrasound scan, my endometrial lining is 17mm thick, free fluid is seen in pouch of Douglas and ovarian cyst has been ruptured. I have severe abdominal pain, vaginal cramps, bloating and pain in my leg. Kindly tell me what to do

    I am 28 yr unmarried girl.l had a sever pain in lower abdomen in Dec 2016.when first time digonise in April 2017 I was found of having some cyst in both the ovaries.when in May 2017 I ws digonise it ws found to have some small POD collection.I too had increased levels of prolactin that tym .Now it is normal.I always have irregular periods.there is a delay of 15 to 20 days.Sometime there is also white discharge. Gyne prescribed me Novelin M1 for 6 months from the month of Aug 2017. in the month of Nov 2017 there ws a medicine allergy so I discontinued the medicine. When consult with gyne she asked to stop the medicine. I had the last period in 29Nov 2017.Is it matter of concern? what should I do now??plz suggest me..

      Findings suggest you have hormonal imbalances. This needs correction. Visit a gyne specialist for this.

    I am 31years old, had still birth in 2015 no pregnancy since then. But now blood test shows am pregnant, saw my period 26th Dec 2017 and lasted 4days normal. But the scan says am not pregnat, Simple Cyst in the left ovary, right ovary normal in size, Fluid in pouch of douglas. I do feel pains in right lower adomen pelvic area and sometimes its swells and go down on its own. Pls doc could i be pregnant and d scan didnt see it cos its early? And if i am pregnant with the fluid in pod affect my baby? When can i start treatment?. Thanks.

      It’s possible that the scan is missing the baby. May be a repeat scan would reveal it.

      Fluid in POD may be due to infection. This needs treatment. Talk to your doctor about it.

    I had a pelvic scan done a fluid was seen in my pod i was given medication and after a month i went back for the test and i was still javing the fluid in the pod and this time around there was a staph infection i was giving antibiotics which i took for ten days the doc said the drugs will work for both the fluid in the pod and the staph infection but some days back i noticed dat i have boil on my private part and is not with puss is begining to dry up so i googled the cause and i saw that is a symptom of staph pls what drugs can i take because i have been having unprotected sex for over six months now and no pregnancy my doc keep saying all is well but am really getting worried although i was pregnant in 2010 but i had an abortion and then after the abortion after two weeks i was still bleeding and i went back and the doc,said they were going to do a repair the D and C was done again i have had over three pelvic scan done and everything is said to be normal except for the fluid and infection which i have been treated for pls what other test can i do to accertain that all is well and what drugs can u recommend to clear this staph ones and for all because i am really trying to conceive. I am 30 years of age.

      Conception would be difficult till the infection gets eradicated from your pelvic cavity. You just need appropriate antibiotics.

      Eat a healthy diet. Include probiotics in your daily diet. This would help. Avoid alcohol.

    I am 31year old unmarried,few days back I had sever low abdomen and low back pain on right side .When I consulted doc , they told for scan in the scan report it was mentioned that the right ovary is enlarged.The doc said its PID ,I want to know what is the reason for PID,(as I googled in net PID is caused due to sexually transmitted infection,after childbirth)
    I do get breast pain during my periods.
    Can you please help me in this regard

      It’s an infection. You may get it via unprotected sex, after child birth or any other instrumentation in the vaginal tract, infected tampons.

    I did an abortion and saw blood for 8days and stopped after a week I went for scan and I was told within the endometrial cavity is seen a thin film if fluid, suggestive of haematometrial. There is significant fluid collection seen in the pouch of Douglas with a depth of 1.22cm. I was given injection and place on antibiotics Amclavin -625. The next scan shows 17*8mm POD collection. Please hope this will heal up soon though just finished the drugs.

          There are chances you have some infective focus in your right side adnexa. Needs treatment.

    I was diagnosed to have massive collection of fluid in the POD.
    it causes me lower back pain and heaviness down there.
    I was given Ceftazidine and genticin injection for five days and some antibiotics after the injection.
    before the injection I was having smelly discharge of which cleared up afterwards.
    I went back for a scan and the fluid was still there.
    pls could it stop me from conceiving later?
    cos in not ready now.

      You appear to be having some severe infection in your pelvic area. It’s important to eradicate it before conception. The infection would not let you conceive.

      Once eradicated, conception should not be a problem.

    I am 34years old my period hasnt been regular for a while now my virginal was hitching so i went see d doc i was ask to go for a pelvic scan and and a hvs i was diagonosed with subseral fibroid which was said to be 18cm and infection i was given an antibiotic called A Day.I had my last period 25th December 2017 and am still yet to get it, i noticed i started ovulating on the 8th of january2018 which lasted for about a week but on the 31 of january i noticed my discharge was slippery as if i was still ovulating . Pls can this be the reason why my period isnt stable

          This is unlikely to be the cause of having unstable periods. May be you need to check up your thyroid profile.

    I’m just 17.
    some years back I used to notice severe pain during my ovulation.
    I didn’t take it really serious.
    but recently,it was hurting me more often than before.
    I was scanned and the result was…The uterus is normal sized anteflexed,non gravid with secretory endometrium of 12mm
    normal myometrial echo
    no myoma nodules seen.
    Right adnexae cyst seen of 4.3×3.2cm with thickened wall and ground glass appearance, it has diastal acoustic enhancement.
    left ovary normal.
    no significant fliud in the POD.
    I was given antibiotics and was advised to come for subsequent scans
    pls could this Prevent me from getting pregnant in the future?
    and how could the situation be cured?

    but last month I discovered it was 27 days.
    I’m yet to menstruate this month.
    and I feel heavy down there and also to the right part down I feel so much pain.

    Good evening doctor. Please I’d like you to advice me.. It’s a long read, kindly bear with me. I’m 33, have two children, the 2nd is about 2yrs old. Before I got married, I used to have moderately painful menstruation but after childbirth, it stopped. Also, I have had few incidents of itchy vulva before getting married and had them treated. My menstruation comes every month, flows quite heavy for 4-6 days but doesn’t have a time table meaning that it can be 30 days this cycle, then 29 the next and 33 the cycle after that- that’s how it’s always been. I didn’t have difficulty conceiving though I was diagnosed of high prolactin because fluid was coming out of both my breasts. Anyways, the main reason I’m asking for help is because I menstruated 3 times last month. I’m also having pain in my lower abdomen – I feel it when I wear tight trousers. Initially, I do have brown /dark red spotting between periods and a sharp pain in the middle of my lower abdomen (I don’t know when I ovulate) but I was surprised to see real bleeding with lots of clots and the blood usually comes after sex and flows like menstruation for 3-4 days before stopping. Prior to this blood flow, I had serious vagina itch which I treated with clotrimazole vaginal cream. Saw my gynae yesterday, he sent me for a scan and here’s the result :
    Uterus appear normal and anteverted in position,
    Endometrial cavity appears intact and empty,
    Myometrium is homogeneous in outline,
    No ovarian mass seen,
    Adnexa are free bilaterally,
    There is moderate fluid collection demonstrated in the posterior cul-de-sac in keeping with pelvic inflammatory disease.
    Urinary bladder appears distended and harbours cloudy and echogenic urine? UTI? Calcium crystals. No stone or mass seen.
    Impression-PID, UTI? Calcium crystal. Urine mcs and hvs is advised.
    I’ve been given doxycycline, flagyl, ciprofloxacin to use and Ecs and urine culture taken. I’m also adding fluconazole to the medications. Please I’d appreciate your explanation on what these mean? Scan results and unexplainable bleeding. Does my husband also need to get treated? How can I stop getting these infection because it’s been recurrent for a while now. Thank you and God bless.

        Scan shows clearly that there’s no abnormal growth or mass in your pelvic region. So, the only reason that comes in mind to explain those symptoms is infection there.

        Infections are usually accompanied by low abdomen pain, itchiness, frequency or urgency while urinating, mild fever. How many of these symptoms do you seem to have?

        Yes, recurrent infections requires your partner to be checked thoroughly and get treated.

            You surely have an infection. Since your infection has well spread in your pelvic area, thorough treatment to treat it is required.

            Treatment appear alright. Have you self added fluconazole to the regimen? If yes, you may discuss with your doctor about replacing it with an anti fungal ointments.

            Too many antibiotics, as in your case, may deplete the population of friendly bacteria dwelling down there. This may disturb the pH of the vaginal/ vulvar area and cause itch. You solve this problem, you may include any probiotic supplement to your daily diet. Home made curd, taken twice daily, may also be OK.

            OK doctor, thank you. Yes I added fluconazole. I was at the hospital today for the lab test results and both the urine and ECS culture yielded no bacterial growth after 48hrs. However there were numerous pus and epithelial cells in the result. I was given abf topical cream as I complained of the itching still present. I noticed when I scratch a lot, down there peels and I get stuff like bumps on the skin. Please can you explain what having those cells means and what is home made curd? Thank you.

            It’s alright to have those cells in urine. Just indicates past infection, that is getting cured.

            Curd may be made fresh at home from milk. If not possible, any yogurt, probiotic supplement would do.

    I had DNC on DEC for a molar pregnancy. I got my period at 5weeks.The problem is I am having stomachache that comes and goes and brownish discharge mixed with blood.And my period lasted for 7days instead of 3days.
    My scan result this morning says:The uterus is bulky and anteverted measuring 120mm(L)×60mm(Ap)×75mm(W).There is a fundal suberous fibroid measuring 48×44.5mm.
    An amorphous endometrial cavity mass heterogeneous echoes measuring about 37.8cm3 by volume indicative of retained product of conception.
    The right ovary appears normal, there is a thin walled left ovarian cyst measuring 36.7×26.2mm with thin internal septations indicative of hemorrhagic cyst .
    No fluid at the pouch of Douglas. What does all this means.I don’t want another DNC pls help

      You may need to go for another DNC.

      Previous one was not done completely. The retained products of conception cannot be left as such. They would get infected and complicate the situation further.

    I’m 25, no history of pain during urination or other signs of infection or UTI. I only noticed increase in length of days of menstrual flow over a period of abt 5month and at a particular month flow lasted for 10days. Saw a doctor, went for ultrasound and it suggested PID cause of fluid in PoD. I treated with a dose of cefiximine and finished with doxycycline for 10days(though I missed one dose). So after a month, I did a repeat scan and still found fluid in pouch.
    What might be the problem? Thought its supposed to have resolved.

        My period for the month is here again and has refused to stop, again!!! I still use a panty liner for the drops of blood and this is the 12th day. Went for lab test and I’ve just received the result of the HVS m/c/s today and it shows staphy + candida??.
        I’m just sad right now and don’t even know what to do anymore. Is rocephine the best treatment plan? The pharmacist also said I’m to take ulcer drugs, said its because I’ve been using caffeine containing pain relief for my dysmenorrhea , is it necessary?

          You still have the infection and it’s very important to treat it. Such infection may give you problems, like infertility, later on in life.

          Rocephine is the drug of choice in most cases. Your doctor needs to check up if you’re responding well to it or not.

          If your pain killer contains caffeine, it’s better to take anti ulcer pills. These pills are just strong anta acids and would protect your stomach lining while you take your medications.

So we were advised to go for pelvic scan cos I explained to my doc dat I had an infection with painful urinations and sm blood coming out after urinating 3years ago which I treated.( I got dis from my ex I know) came back last year with just painful urination which affected my husband erection( soo sad ?because my husband didn’t have any infection wen I met him because he did test after I met him which stated dat he is infection free n he is a faithful man.. And I have been faithful too.. we treated it and it went.. But last month I noticed a lot of white discharge with an unpleasant smell which last for 5days n later saw a brownish discharge for 5days.. Now I went to scan and this is the result Doc:NORMAL SIZED(65/41/32MM) NON-BULKY UTERUS IN ANTEVERTED ORIENTATION.. THE ENDOMETRIAL CAVITY APPEARS THIN, EMPTY AND INTACT.. NO INTRA OR EXTRA UTERINE GESTATIONAL SAC IS SEEN PRESENTLY….THE MYOMETRIAL ECHITEXXTURE APEARS HOMOGENEOUS.. NO FIBROID IS NOTED… THE ADNEXA ARE FREE.. BOTH OVARIES ARE INTACT… THE POUCH OF DOUGLAS HARBOUR SIGNIFICANT FLUID COLLECTION…….
    my period is regular and its last for 2.5 days… 28-30 days cycle…. Pls I need u to help me interpret all dis to my understanding sir.. Will prescribed drugs cure it prominently and will I conceive as soon as I treat it plsss.. Help me out Doc!

    Right dominanat folicle.

      Need to be treatment urgently and completely with appropriate antibiotics. You appear to have an infection in your pelvic region, affected the urinary bladder and the reproductive tract.

      If not treated well, conceiving would be difficult.

          A repeat scan may be done to visualize whether the fluid collected has reduced or not. Also, you should be free from any pain, discharge of back ache.

      You’re having some infection in your pelvic area. This needs urgent treatment with antibiotics. Your doctor have rightly proscribed you with pills.

      Drink plenty of water. Avoid alcohol and limit to tea, coffee and excess of spices. Consume plenty citrus fruits.

      If you still any burning or pain down there or have an urgency to urinate, you may need to get investigated again.

      You should be completely free from that pain. Also, get a repeat ultrasound to see if the fluid is still there.

      If any sign of infection is left, more antibiotics may be needed.

    I have been having white discharge and lower abdominal pain for sometime now. I also missed my menses for the month of February. (25) I went to see the Dr and was asked to do scan and this is the outcome:
    The uterus meauteru, 7.8 cm x 6.9 cm x 5.9 cm. The enendometrial stripe is of uniform wall thickness minimal fluid. No focal mass, collection or intrauterine gestation seen.
    No adnexal mass seen. Minimal fluid in POD seen.
    The visualized urinary bladder is normal. No urinary stones, urinary bladder wall thickening or trabeculations, ureterocoels or urinary bladder wall diverticula’s were noted.
    IMPRESSION: Minimal fluid in POD.? Paid otherwise normal pelvic scan. What is the meaning of this Dr.? Will there be complications in case I am pregnant? Pls help

      You are likely to be having some infection in your reproductive organs, may be the tubes, cervix or else where. This is giving you the pain, discharge and also the fluid in POD.

      Need to start off with appropriate antibiotics at the earliest. Complete treatment is required, otherwise, the infection may interfere with future pregnancies.

      Though the vaginal swab test shows no growth of bacteria, it’s very likely that you have some infection in your pelvic area.

      Appropriate antibiotics needed at the earliest.

    Pelvis: USS shows a normal sized uterus with an AP diameter of 4.0cm. The uterine outline is regular. The echo pattern is homogenous. The myometrail texture is normal. The endometrial thickness is normal. Both ovaries appears normal in sizes and structures. Both Adnexae were bilaterally free. There is fluid with a depth of 15mm in the pouch of douglass. No cyct. No fibriod. No cyesis or retained products sighted.
    Please help what should I do…

    My doctor prescribe levofloxacine 500mg 1daily, Azithromycine 1g for start, 500mg 1daily, flagy 400mg 1daily , diclofenac 50mg 2daily.
    I used the drugs yesterday night, but I see my menstruation today evening, but the pain is too much, I have to stop the drugs due my menstruation because I don’t know maybe I can use the drugs with my menstruation or not, because am unable to reach my doctor, but am having much pains, it will come and relief me for some minutes and come again, I used buscopan. The pains always coming from above the navel even before the scan.

    Hi doctor, I’m 30 and not married I saw my period on Tuesday which was 13-03-2018 and ended on Friday 16-03-2018,and it was a normal but It started again on monday morning and was really heavy,thick and red then on wednesday i noticed big clots coming out with the blood, and also the bleeding comes out heavily every morning the stop in the afternoon everyday.. then I went to the hospital on Thursday and I was asked to go for pelvic scan which i did and this the result….

    USS shows the uterus with an AP diameter of 4.5cm. The myometrium is largely homogenous, save for a solid tissue mass with missed echo texture measuring 2.5cm by 2.3cm within the body of the uterus(submucous).
    Follicular activity noted in the right ovary.
    There is fluid with the dept of 14mm in the pouch of doauglass.
    No thesis or retained product.

    ASS:
    demographic features suggested* SUBMUCOUS UTERINE FIBROID
    POUCH OF DOUGLASS FLUID
    R/O PELVIC INFLAMMATORY DISEASE

    And the doctor gave me tranexamic acid 500mg and cefuroxime axetil 500mg to taken for 5days
    And i feel pain at my lower right abdomen whenever I use the cefuroxime. It’s also swollen
    Pls I dont understand anything becos my doctor didn’t explain much but only said i need to operate it and that’s all
    Does it mean i can’t get pregnant or what
    And I want everything to go away. I need your advise pls doctor.

      You have a small growth is your uterine cavity. Medically, we cal it a fibroid.

      The fibroid is giving you the pain, swelling over the lower abdomen and the heavy bleed. It needs to be scraped out by a small surgical procedure.

      This should not hinder future pregnancies in any way.

    Hi Doc, have been having the feeling that am pregnant for 3 months now have been nauseated, fatigue, extreme hunger ,whitish discharge. But the pregnancy test were negative both the urine and blood serum test. I did scan this is the result- The uterus is normal in size and outline. It measure 6.5 cm * 4. 4 cm ( L* AP). Myometrial echotexture is homogeneous and the endometrial plate echo is thickened and measures 15.0 mm. There is significant fluid collection in the pouch of douglas. Both adnexae are free. The impression is thickened endometrium plate.

      The condition is medically called Endometrial Hyperplasia. What’s your age? Any kids?

      You may need to start with a short term Progesterone therapy to treat the condition. This is a hormonal treatment, to be prescribed by a gynecologist.

      If overweight, shedding weight would help a great deal.

      You appear to be harboring some bugs in your pelvic area that gave you the pain and discomfort. Medicines were rightly taken. You may also need to take some probiotic supplement after this long course of antibiotics.

      Symptoms may take a few more days to go completely. However, you shall now feel less pain in back or abdomen. Let us know, has the pain reduced?

          14 days of these pills are usually enough.

          Pain may take time to go on its own. However, you may need a follow up check up, with U/S, to assess the inside condition.

    Hi Dr. I’m 28. 2weeks ago I had terrible abdominal pain. Couldn’t stand upright. A nurse neaby gave me some tabs to relief the pain. It actually subsided but I wanted to be sure what was wrong so I went to see a doctor. We ran some test and results showed I had typhoid. His scan revealed my organs were ok. No fluid in POD. He advised I go 4 an appendectomy. To be doubly sure I went to another hospital after 2 days. The scan result from the 2nd hospital showed mild fluid in POD.2nd Dr is suspecting PID and placed me on treatment involving gentamicin iv for 3 days and doxycyclin. the pain has reduced though its still there. I feel it when I walk, i can’t lie tummy down or on my right side.its also painful when touched.I pee often. I dont understand why both scan say something different. I’m confused. Is it typhoid, appendicitis or PID

      It’s difficult to not be able to differentiate between an inflamed appendix and PID. The person who is doing the sonography would be able to easily tell you.

We have been trying to conceive since January this year. I went to the hospital and was scheduled for a TVS because of the following symptoms I had; nausea, fatigue lower abdominal cramp and night fever. These were the following results, BLADDER is smooth- walled and normal. UTERUS is anterveted. It measures; 86mmls, 49mmAP, 60mmTS. FIBROIDS – nil. ENDOMETRIUM is 16mm & regular. CERVIX is normal. OVARIES are normal. ADNEXA has no masses. POD has significant echo rich fluid collection at the posterior cul- de- sac. IMPRESSION: Endometrial thickness. ? Proliferative phase. ?Early cyesis R/o PID. If this (early cyesis) happens to be what looks likes early stage of pregnancy formation. What should I do about my POD having an ‘echo rich fluid collection’. Warm regards. Joan

      Your doctor may put you on antibiotics and repeat the scan after some time. It’s important to get rid of the infection, if its there. Antibiotics which are safe in pregnancy are given in such cases.

    Hi there

    I have had test in UK and they say I have fluid in POD 23×20 cystic lesion with irregular internal walls (that looks like papillary projections) seen in the right ovary.
    No ultrasonic evidence of high lesion seen.
    What does this mean?

    Years ago I was told I might have PID I was given one course of oral antibiotics but the STI clinic changed hands and they lost my records, so I was never able to discover if I did or not

    I did a scan that showed fluid in the p.o.d because of a missing period of about 18days.staph aureus was shown on urine m/c/s and hvs.I was given a combination of metronidazole and clindamycin.I would like to know when the period will go back to normal and when its safe to do another test

      You’re being treated for the infection first. These pills would reduce the fluid in POD and treat the infection.

      After the infection gets treated, your menses may revert back to normal by themselves in 2 to 3 cycles. If not, you need to consult a gyne. This issue may required separate medical treatment.

      Ultrasonography suggests that the patient has polycystic ovarian syndrome. This needs to be correlated with clinical findings.

    The endometrium is thickened with a depth of l5. 7mm.
    Pod – pouch of Douglas contains mild fluid.

    Good evening, I can’t sleep with my tommy, I do have cramps on my left or most times right pelvic. I started spotting and seeing a brownish discharge whenever I stress myself. I saw my period last April, I ran a pregnancy test and it was negative. Few days ago I noticed pain in my Breast. I went to the clinic and I was asked to go for a pelvic scan which the result States.
    Uterus is normal size ante-verted and empty.
    Outline is well defined. Echo pattern is normal.
    No myoma or adenomyosis seen.
    Endometrium is normal. No gestational sac seen.
    No ovarian mass seen.
    Pouch of Douglas harbors small clear fluid.
    No evidence of salpingitis.
    Could my hcg be low or should I be worried. Thanks

      Well, even if HCG levels were low, a gestational sac is usually visible. So, it’s less likely that you were pregnant.

    I’m 32 years and i have been having abdominal pain. Some blood tests were done and i had hormonal imbalance. After completing the medication the pain did not stop. An ultrasound was done everything was normal except for the free fluid which was seen in the Pouch of Douglas 3,36cmx0,95cm. I was given some antibiotics but after completing the course i still feel the pain. I now have abdominal and back pains. My stomach is now swollen. What may be the cause? Should I go for another ultrasound scan?

      Better to go for another scan. Your symptoms cannot be due to the free fluid alone. What is the situation of the ovaries/ uterus?

          So, the only abnormality seen is free fluid in the pouch.

          The commonest reason for this is pelvic infections. A repeat scan would reveal if the fluid is still there. Vaginal swab cultures may be performed to check infections.

    I have been pain in my right lower abdomen most especially at night that last for 3-4 hours and I have gone to do series of test which says I have a free fluid collection in my pouch of Douglas which is causing PID and I also have a moderate size of cyst.i have taken several antibiotics injections and drips and the symptoms hasn’t stop but I am now on rocephine for 3 days and I have taken it for two days now and the pain hasn’t stopped.am so confused.pls do they do surgery for fluid in the POD?

      Surgeries are not done routinely. Long courses of antibiotics are run.

      Rocephine is appropriate for this. However, may be supplemented by doxycycline type pills. You need to discuss about appropriate antibiotics in your case, with your doctor.

          Fluid in POD is commonly due to chronic infections in the pelvic. This may give you chronic abdominal pain or backache.

The right ovary appears sonographically normal measuring 3.32*2.05*2.30cm with volume of 8.18cm3. The left ovary measures 4.65*3.08*3.93cm with volume of 23cm3 with areas of irregular cystic collection. There is large fluid in the pouch of Douglas.
    Please what does this mean and how will it affect conception?

    pls I am 30 years old went for a pelvic scan everything normal except for fluid collection with depth of 2.0cm in the POD. went for hvs and states heavy growth of staph and strep. I have used different types of IV antibiotics from rocephin to gentamycin to ciprofloflaxin complete dosage also tablet like doxycine to flagyn. even when the test shows by body is sensitive to this. I am so confused don’t know wat else to do. even my doc. is confused with my case . plz what else do I need to do. I am trying to get pregnant and the sperm keeps pouring out ..

      Such infections may take time to resolve. Sometimes, surgical interventions, like drainage, may be needed.

      Sometimes, surgical drainage of infected fluid speeds up recovery. Your doctor is the best person to decide whether you need a surgery or not.

    I am 18 years old and my health has been rather fine before i had the problems that i do now. My periods have always been every month on the 17th. 2-3 days late at the latest and last 5-6 days. I have had cystic acne since 5th grade on my face, neck, back and chest. At the end of 2015 my face and back and neck cleared up (minus the scars on my back) I am 4’10 and have been 100-105lbs. 110lbs was my heaviest before all this and it was very easy for me to lose the weight. My diet has never changed. I eat two meals a day (800-1000 cal per day). I had thick and long blonde hair and my health was great. After my period of June 17th,2016 i stopped having my period for 4 months. I took pregnancy tests all throughout that time and they all came back negative with one faint positive test but a week later i took another and it came back negative. I’m assuming it was faulty or i just had a hormone surge. My acne came back but only on the lower half of my face and my neck and my back. Not cystic or as bad as it used to be but they are stubborn break outs. I put on 60 pounds in two years with no change in my diet. The weight is so stubborn and it happened so fast the stretch marks on my stomach broke open and bled. I have a brown patch of skin that grew in the middle of my chest and never went away. My hair turned brown in a week and began falling out in handfuls and my hair became very oily. I have the worst pelvic pain and back pain. It feels stiff and painful. My legs sometimes lock up and have even given out on me. When i bend down, i can not stand back up quickly. I have to slowly work my way there. I cramp in my thighs very easy. My periods have been skipping multiple months ever since that 4 month break. I had two times where i felt i always had to pee and when i peed there was a burning feeling and my area where urine comes out from felt like it was pulsating and eventually a small clot came out from my urine tract. I had a period in April of this year where i was only lightly spotting and some of what came out of me was black and transferred black. This lasted two weeks and i have never had period cramps but i had one that caused me to throw up on myself. My pelvic and back pain is primarily on my left side but my right side still has pain. I was excessivley bloated and now when i sneeze or cough i pee a little. This last period lasted four days and was nothing but blood clots. My last one before it was two months ago. I get a tingly pins and needle pain in my left lung and heart area and it lasts 30min-1hr and is once or twice a way and it’s painful. Now i have not been able to sleep at night even when i am tired and genuinley try to fall asleep. Ia have been doing some testing and my DHEA levels were flagged for being high. They are supposed to be less than 6.6 but they are exactly 6.6. My cholesterol and tryglcerides are very high and they did an ultrasound and found follicles in my ovaries and a mild amount of fluid in my posterior cul de sac. My follicles are small and they are less than 25 so they can not confidently say that it is PCOS and if my cysts are small and IN my ovaries, they couldn’t rupture and fluid leak all the way over to my Pouch of Douglas, could they? If so, they couldn’t leak enough fluid to be detected on Ultrasound, right? I am so stressed. I don’t know what could be wrong with me.

      This is clearly an hormonal issue which needs immediate treatment. Have you been put on any pills?

      Fluid in POD is unlikely to be from the ovarian follicles. There has to be some other reason.

        I have not been on any medication, I have never been on birth control either. My follow up is on the 21st but I feel like my pcp is going to try to tell me PCOS. Which is what I thought it was before but if it doesn’t reach the requirements for PCOS, I don’t want to settle on a diagnosis that’s not accurate and let these problems grow. I forgot to add that I was also tested positive for a genetic mutation (polymorphism) when I was seven and I don’t know what kind but the doctors had told my mom it could very well cause me to have health problems. My dad was supposed to be a twin and absorbed his brother or sister in the womb and had two blood types because of it. He also had a hereditary leukemia so I’m thinking it could have been passed on from him. He passed away from it at the age of 39. I also do not have the facial hair that comes with PCOS either. Are there any requests or questions I should ask my pcp? Any extra tests to have done? Should I have some of the fluid extracted and tested? What are your thoughts?

          These are the findings they said for my ultrasounds:
          Uterus: The uterus measures 5x3x4cm
          Endometrium: The endometrial echo complex measures 9mm which is within its normal limits
          Ovaries: The ovaries are of normal size, shape and configuration. While there are multiple small peripheral follicles in each ovary, there does not appear to be the requisite 25 per ovary to meet the sonographic criteria for a diagnosis of PCOS. There is no sign of an adnexal mass on either side.
          Doppler: There is in tact color flow to each ovary. No high resistance spectral waveforms seen.
          Cul-De-Sec: There is a mild quantity of free fluid identified within the posterior cul-de-sec.

            Well, polymorphism is a very general term, and it’s difficult to comment over it. What genes are involved and what expressions to they give.

            However, for your symptoms, you may be given hormonal pills, which would benefit in alleviating your symptoms.

            You may request for a full hormonal assay. Some set up provide genetic studies, which may be opted for.

    Hi Doc have been experiencing pain in my lower abdomen I don’t experience any pain when I’m with my partner my periods changed from three days to two days and are not painful,I went for a abdominal pelvic ultrasound today and the report shows that there are no aortic lymphadenopathy,abdominal ascites or appendicitis seen uterus is anteverted nongravid and normal in size and shape outline and echo pattern there are no endometrial/myometrial masses seen the endometrial stipe is thin both adnexa are clear there is free fluid seen in the pod the doc gave me doxy for 14 days and secnidazole pkiz doc could it be the reason I’m not able to conceive.

      You seem to be carrying a n infection in your pelvic region. This may be a likely reason why you,re not conceiving.

      Complete the antibiotic course. Your symptoms are likely to go. The fluid in POD would also get absorbed.

    Hey doc! I recently had 2 pelvic scans, two weeks apart because i noticed i had thick blood clots during my period and a heavy flow. First test revealed i have a fibroid, however every other thing was ok. Second test also saw a fibroid but with the following diagnosis:
    The pouch of Douglas shows moderate fluid collection. POD fluid may suggest PID.

    Please what could this mean, i was advised to have a hcs and urine mcs. Could you please help me shed some more light on this diagnosis.

      Fluid in POD may suggest that you’re carrying some infection in your pelvis. Further investigations suggested would confirm this.

      This means you’re carrying an infection in your pelvis, likely to be due to the tissue retained inside you.

      For now, an antibiotic course is required to eradicate it completely. Acyesis just means you’re not pregnant right now.

      There appear to be some infection in your reproductive tract, which needs prompt treatment. You may be given a long course of antibiotic treatment. Before that, your doctor may suggest draining out infective fluid from your pelvis.

      Left ovary appears cystic. This is usually a hormonal issue.

By doctor’s diagnosis she is found mild fluid collection seen in cul-de-sac -mild PID through ultrasonogram of Kub with pvr. Doctor gives her entibiotic neofloxin (ciprofloxacin) and tablet ucol 2 (tolterodine tartrate 2mg).is it right drugs to recover her from this infection? And one more question is how can it possible at this age? Doctor also surprised that it is not seen at this age. Will she get any side effects in future when she will be adult because of this infection?

    Hello doctor am glad I found this page, am sure you can help me too, my name is Caroline am from Tanzania I am 22 years I got married in2016 but untill today I don’t have a child me and my husband have been trying to get a child but Everytime I sex with him i feel pain in some parts inside me.. I went to see a doctor and I was found with PID infection and I was given the antibiotics dose I finished the dose within seven days and went back to see a doctor again and the investigation showed the my ESR is 26 so again I was given another two types of dose which are NOR-T which I was supposed to finish within 10days and DUPHASTONE which I was supposed to finish within 14 days after I finished I went back to see the doctor and the ESR was 4 but this time because I was scared and I wanted to know more why do I not get the pregnancy so I took the ultrasound and the report were as follow free fluid collection seen in the pod, uterus is normal in size, shape and echoes. Endogenic endometrial strip are of normal thickness. Both ovaries are identified with multiple small cysts. The cervical region is normal. Urinary bladder looks normal outline conclusion sonographic feature suggestive non gravid uterus with free fruid in pod ddx PID but the doctor said I should not worry cause that is normal.. my menstruation cycle was sometime go to 30 days sometime to 26 my last day I entered to menstruation was 7th August this year.. the doctor said if I don’t see my days for a month I should go back for another checkup am so afraid and I really need to have a child what is my problem please help me doctor

      You appear to be having some hormonal problem, that’s interfering with conception. You need hormonal pills for this. DUPHASTONE would alone not solve this problem.

      A specialist gynecologist needs to see you. Pills may be given for 3 to 6 months to solve those cysts in your ovaries.

          Those cysts are likely to be the reason why you’re not able to conceive. They indicate some hormonal problem. Treating this problem would resolve those cyst gradually, and you’ll be able to conceive.

    Hello doc,
    Have being trying for a baby for about four months now, I was to have my period August 9, I saw it August 12, and before it even started very small particles of blood was coming out, about four of it before the normal blood started coming out which was very light then the deep blood color follows. Then September too, instead of seeing it September 6, I saw it September 9.
    I do have smelly discharge from my vaginal, and it also smells bad sometimes when having sex.
    Then, I went for a pelvic scan yesterday thinking I was pregnant because of some symptoms I do have, the result came out this way
    It reveals of about 27mm in diameter normal shape and size.
    Endometrial plate not seen
    Bilateral ovaries and tube not seen
    Nil sign of pregnancy
    Nil fibroid seen
    Associated fluid in the pouch of Douglas
    Then the doctor said I should go for HVS that I have PID.
    Please, I don’t know what this means.

      You certainly have an infection in your pelvic organs, that’s preventing you from getting pregnant. You need to be on appropriate antibiotics at the earliest.

    Hello doc am glad I found this page I was feeling some pains down my stomach so I went for scan today so the result was the uterus is anteverted and normal in size measuring 57/44/37 mm. The outline is smooth with uniform echopattern. The endometrial layer is thickened. No extra or intra-uterine gestational sac is seen .No myoma is noted . There are two smooth wall septated anechoic cyst at the right and the left adnexa measuring 38/34mm and 44/33mm respectively. There is mild fluid collection at the pouch of the Douglas. Please sir what does it mean I can’t give conceive or will the pain continue like this sir please explain better

      This may indicate some hormonal disturbance in your body, which has given rise to those cysts. You’ll need to get them treated before conception.

      Such cysts interfere with conception.

      Your doctor may prescribe simple hormonal pills for some time to correct this issue.

      PID is a very common cause of not being able to conceive. You’ll need to take appropriate antibiotics to treat it.

      You may need to get a surgical cleaning done. Usually, a procedure called DnC is done. Also, antibiotics may be prescribed, as fluid collected in you pelvis suggests some infection.

    My menstrual flow was getting light lasting for just two days so the doctor advised I had an ultrasound scan done, in the ultrasound it was noted that there is 14*11mm myometrial nodule and intact endometrial plate, POD collection along side ?well rounded left adnexal? Functional cysts ( larger =17*19mm) are seen but no probe tenderness or abdominal lesions…

    PID was concluded but I am a virgin and have never had penetrative sex, is it possible for virgins to have PID? and what do you think is the cause of my suddenly light menstrual flow

      Virgin female may get PID, probably from sanitary tampons, pads, infected commodes etc. You may need to take antibiotics to resolve this.

      Reduction in flow may be a hormonal issue. It may have also caused you the myometrial nodule.

            You may continue with the antibiotics, as prescribed. A follow up ultrasound may be needed at the end of treatment for objective assessment.

            Your doctor may prescribe you hormonal pills for a short duration.

    Should I be worried about the size of the nodule 14*11mm..
    And the cysts as well, I really would like to have a child, does any of the findings affect my fertility

    Hi Dr, I went to the clinic and an ultrasound was done, Dr said my ovaries are enlarged due to presence of cysts, the left one measuring 48.5*32.6*48.6 and the right measuring 45.0*46.7*49.9. This test was done on day2 of my menstrual cycle, could it be enlarged due to menstruation? Or does this findings affect my chances of having a baby?

      Unlikely to be due to your menses.

      Cysts in ovaries are usually due to some hormonal issue and may interfere with conception. You’ll need to get them treated before planning pregnancy.

        Thanks doctor, I’m so worried.. After the scan the doctors gave me some antibiotics and asked me to come back after a month. Do you think things might go worse after a month? Because I had a scan done like two weeks before this one and cysts were only seen on the left one now they are present in both.. I’m so confused and scared that if I should wait things might get worse..patiently waiting for your response

          You may take antibiotics to combat infection in your pelvic region. If, however, you don’t conceive, you may need a hormonal treatment to correct this problem.

    Please doctor, I have been having frequent discharge in menstrual form, I did a scan and the result read (moderate fluid collection is seen in the pouch of douglas, there is nidular sub serous mass noted at the fundal region of the uterus measuring 23mm x 22mm). Please what does it mean and what do I do?

      It’s a non cancerous mass or tumor in your uterus. Such growths are common, and may arise due to hormonal fluctuations in females.

      Nothing to worry. Many of such mases are hormone responsive. You may be given pills to help it regress on its own.

    Hello Doctor,
    I usually feel a sharp pain on the left side of my stomach and another in centre of my stomach. I feel bloated always and steady stomach pains. some time when i touch my stomach i feel pains. And also I missed my period for last month before finally seen my period this month a day after going for U/S Scan. It is slightly painful to the touch. I have the urge to urinate frequently sometimes with a burning sensation. I feel severe pain s in my lower abdomen, lower back and the worst is I feel a movement in my lower abdomen especially when I just had a meal.
    The result of the Scan is as below:

    Scan Report:
    USS shows a normal sized uterus with AP diameter of 3.8cm. The uterine outline is regular. Myometrial texture appear normal. Endometrial thickness is normal in sizes and structure. Both adnexa appear free. Pouch of Douglas is empty. No free Pelvic collection. No fibroid. No Cysts. No cyesis or retained products of conception sighted. Pls kindly advice as appropriate. Thanks

        Hello Buddy MD,
        Thanks for your feedback.
        No they didn’t rule it out Urinary Tract infection at all. But have been placed on an antibiotics already and am feeling much better than before now after about 1/2 way through the full dosage.

    I was having heavy pain at my right lower abdomen, and heavy discharge, I went for pelvic scan today.result says ,uterus normal size anteverted utreus is seen with normal myometrial echotexure, measureing about 55mm in AP.There is no evidence of uterine fibroid, no cyst, endometrial appears normal, both ovaries appear normal, Adnexae free, pouch of Douglas there is fluid in pod . Impression : the features is suggestive of , Acyesis, pid, hvs and urine MCs is advice, am trying to get pregnant for 8 month now , pls what is wrong with me because am worried

    I went for d pelvic scan and d result shows early cyesis at 9 weeks with seeding of uterine fibroid and fluid with depth of 15.6 mm in POD….need you to explain this to me though I know I am pregnant..thank you

      You’re pregnant. However, the scan also shows a small growth inside your uterine cavity. This growth is not cancerous and is likely to remain as such throughout pregnancy.

      However, your gyne may ask you to take certain precautions and call you at regular intervals for check ups to monitor for pregnancy and check if this growth is not growing along with pregnancy.

      Fluid in POD is minimal. Nothing needs to be done unless your doctor detects any infection in your pelvic area.

      Did they do a urine culture/ vaginal swab culture? Can you share the reports here.
