Lump in Adnexa With Cervical Lymphadenitis

Q: I suffered from fatigue and weight loss with lymph nodes in my right neck. Excisional biopsy found Kikuchi disease. One lymph node in the groin developed and disappeared. That happened in August 2012. Lately, I had a CT scan done for full body. CT scan found 8 cm solid homogeneous mass in my right adnexa. I have suffered from pelvic pain before, for the last 7 years, doctor thought it was IBS. Another doctor thought it was ovulation pain. So I was given birth control pill to suppress the pain. I did not have pain after having those pills regularly for the last 4 years. Now, I am worried about the mass they found in my right adnexa. What could it mean? I would really appreciate your reply. If I have ovarian cancer, can a neck lymph node biopsy detect it?

Reply:

An ovarian cancer would rarely give a neck lymph node. Such malignancies spread to nearby regions in the pelvis. It may go to the paraaortic lymph nodes and further to the liver or lungs in advanced stages. Besides your neck lymph node biopsy has already showing features of Kikuchi disease.

However, it is recommended that you get your adnexal mass evaluated. It is important to know the nature and origin of this mass so that no doubt is left.

I’ve recently had a pelvic CT after an elevated ca125 test. The results said: Cystic lesions visualized in the adnexa, favor ovarian follicles. Dilated pelvic veins. What does this mean? Thank you.

      No evidence based link available.

However, epidemiological evidences do show that many ovarian disorders are linked with thyroid malfunctioning. As an example, low thyroid levels are associated with formation of ovarian cysts.
