Causes and Management of Scrotum Skin Rash

Rash in the scrotal area may occur due to many reasons. Causes such as jock itch, eczema and psoriasis should be considered under its differential diagnosis.

Jock Itch is the Commonest Cause

Scrotum rash

Jock Itch

Strictly speaking jock itch is not a scrotal lesion, at least initially. It usually starts in the groin area that may later involve scrotum. It tends to produce itch and redness in the affected area.

It’s due to a superficial fungal infection called dermatophytes, which is caused by Tinea Cruris. It is also known as ringworm, a misnomer since the causative agent is not actually a worm, but a fungus.

It is associated with heavy sweating and rubbing of inner thighs with scrotum in the groin area. That is why it’s commoner during conditions of excessive heat and moisture.

Tinea Cruris Microscopic

Fungus under microscope

The rash is raised, pinkish red, flaky, cracking, peeling and very itchy. It consists of circumscribed lesions consisting of small, multiple pinpoint-like pinkish bumps. Lesion’s central area may consist of dry scales with distinct edges.

Jock itch is associated with:

  • Excess moisture and sweating
  • Tight underwear
  • Rubbing and friction of skin
  • Fungal infection
  • Obesity
  • Diabetes
  • Long term use of corticosteroids
  • Improper hygiene of groin area

Management

Clotrimazole skin cream

clotrimazole cream

  1. Keep the groins clean and dry.
  2. Wash the area frequently with soap and water. This should be done after any work that involves sweating.
  3. Wear loose cotton underwear; avoid tight-fitting clothes.
  4. You may need a topical antifungal cream such as miconazole, clotrimazole or tolnoftate- as deemed suitable by your physician. If scrotum rash doesn’t go away in 2 weeks, then you may need to see a dermatologist.

Scrotum Eczema or Dermatitis

It is characterized by severe burning-like itching, redness, scaling and thickening of scrotal skin. The common causes of scrotum eczema or dermatitis include allergens, irritants and stress.

Contact dermatitis may be induced by variety of factors including chemicals such as topical antiseptics, clothing dyes, rubber of condom, spermicidal agents and grease etc. Other causes may include bug infestations such as pediculosis and scabies, generalized skin conditions such as psoriasis, atopic and seborrhoeic dermatitis and medical conditions like diabetes and systemic infections.

Treatment

  1. Remove the offending agent, if identified. This may include stopping any over-the-counter topical agent.
  2. Wear loose clothing to avoid moisture and friction.
  3. Manage stress with lifestyle modification and counseling.
  4. Medications such as oral antihistamines and mild topical steroids may be required for short duration.

Scrotal Psoriasis – a Generalized Skin Condition

Several areas of the body may be affected with psoriasis in addition to genital region. Psoriatic lesions are red and shiny with clear cut boundaries that are usually present on both sides symmetrically.

Treatment may include application of mild topical steroids for limited duration. Scrotum psoriasis may co-exist with fungal infection, hence topical antifungal creams may need to be applied.

Is Scrotal Skin Lesion an STD?

Many times people ask if rash on their balls an STD, that is, whether it spreads sexually.

Though jock itch fungal infection is not considered a sexually transmitted disease (STD), but direct skin-to-skin contact during sexual intercourse may cause it to transfer to the mating partner. Both the partners may need to be treated if direct skin transfer has occurred.

Scrotal eczema and psoriasis do not spread through sexual contact.

Rash in Infants and Toddlers

These rashes are noted on the skin beneath the baby’s diaper. These are seen among infants and sometimes early toddlers. Scrotal rash is bright red and scaly with pimples or blisters that may get bigger. Multiple patches may blend together to form larger lesions.

It is due to a yeast called candida which grows in warm and moist area under the diaper.

Diaper rashes may be seen in babies with:

  • Tight, ill fitting and friction-producing diapers.
  • Poor hygiene leading to unclean area with moisture.
  • Having diarrhea or more frequent bowel movements.
  • Taking antibiotics.
  • Reactions to soap or detergents used for cleaning cloth diapers.

Treatment

  1. Keep the affected area clean and dry. Try to provide diaper-free intervals to the baby by laying him on a soft cotton cloth or towel.
  2. Frequently change the diapers, especially if he has passed urine or stools.
  3. Clean the area with soft cloth and plain water gently, that is without rubbing, while changing diaper. Then allow it to dry for some time.
  4. Avoid alcohol and perfumed based wipes in order to avoid skin irritation.
  5. Use absorbent diapers and put them on the baby loosely.
  6. If you use cotton diapers, then avoid fabric softeners. Avoid putting rubber or plastic pants over the diaper as they do not allow adequate air flow.
  7. Avoid corn starch and talc.
  8. You may discuss about diaper rash creams with the baby’s medical care provider. Sometimes topical antifungals such as miconazole and clotrimazole may also be required.

Depends upon what is causing you that rash. It may be a fungal, an irritative condition of the skin or some allergy. Bacterial infections are uncommon over the skin.

Is the affected area red or swollen? Since when do you have it? How big is the patch?

I work in a chemical factory for 3 years last month i felt itching, redness and scales on my scrotum i was scared and shaved that area and thought it will ease the pain but it gets worse i went to my GP she said i don’t what it is and refer me to someone at hospital but i didn’t go there and find out the next the my colleague had the same problem but his was gone within 2 days but mine lasted for 10 days ( maybe cuz i shaved and got 2 cut in the area ) i used some over the counter steroid cream. Now after a month i have the same condition.
I am pretty sure the last one was because of some allergic reaction to a chemical at our factory but don’t know about this time as i was cautious about touching that chemical again.
If it is allergic reaction what will help me heal fast because 10 days are tooooo long

    Reply
    1. Buddy M.D. Post author

Very likely to be an allergic reaction. It’s now important to avoid further exposure of the chemical.

Repeated washing with water would help. If it’s not itching, do nothing. Give it time to heal by itself. If it itches, you may occasionally use the steroid cream. Avoid too much use of the cream over the area.

It may be moisture collected in the area that’s giving you the itch.

Wash the area thoroughly with ample water. No soaps or gels to be used. Wear loose cotton underwears to let the region breathe throughout the day.

You may start using an antifungal powder over the scrotal skin. Zeasorb-AF powder or Miconazole powder may be used twice daily.

Likely to be a severe allergic reaction to some trigger. It may be a new food tried, laundry detergent or some other allergen around you.

Start taking anti histaminic pills at the earliest. If the reaction doesn’t lessen in a day or two, you must see a doctor.

So last fall, nearing the end of football season, I got this rash under my scrotum and the sides of my thighs and it was really excessively sweaty and itchy. I finally tried treating it like a month ago with lamisil cream for jock itch for like 3 weeks and it looked like it was working until like a week after the 3 weeks of treatment it grew incredibly itchy again and I ended up getting another rash this one is more dry than sweaty though. And I have a darker skin complexion so the rash is almost like white specs and the skin is shiny where the rash is.

    Dear doctor,

Last June I suffered from an extreme red and itchy rash on my scrotum. I thought it was a fungus infection, so I applied Dactarin cream for 2 weeks. The condition and itch worsened, so I went to my GP and he gave me Triamcinolone. It healed within 4-5 days and I stopped applying Triamcinolone after 9 days.

Last august I had to go to the GP against, because I had some irritation on my foreskin after rigorous sex. The GP prescribed Daktacort for the foreskin rash. It healed within 7 days.

Now this September I’ve been having a light reddish rash/irritation on my scrotum again. It’s not itchy like in June, but it feels slender and raw. I do shave my scrotum with a clipper once a week. I also have a very tiny/shiny (non itchy) light reddish patch on my foreskin. I’ve been applying Shea Butter and a Cold cream.. It’s been a week since my last shave, but the rash on my scrotum hasn’t healed. It’s not itchy, but the skin feels slender and raw.

I wear cotton underwear. I wash myself with water every evening. I don’t wash the area with soap and I don’t have a history of eczema or psoriasis. I am a bit worried because it’s the second time within a timespan of 3-4 months that I have irritation scrotum skin. I don’t want to apply another Corticosteroid cream, because I’ve heard that using those Corticosteroid too often might lead to chronic conditions like Red Scrotum Syndrome.

Difficult to comment without a physical exam.

This is unlikely to be fungal or any other infection. It’s some condition that’s coming from inside. It may be a local irritation to some product being used, or else, you need to be screened for eczema and psoriasis.

This is likely to be jock itch. This is a fungal infection that has stayed in that area since a while now.

As you understand, excess of sweat and moisture in that area has been promoting it. You need to keep the area clean and dry. No need to put soap there. Cleaning with plain water in enough. However, you need to clean it more often, 3 to 4 times a day.

Wear loose cotton underwears, that allow aeration there.

You may also start using an anti fungal powder in the affected area after wash. Apply it 3 times daily. Candid powder / Miconazole powder, any of these may be used.

    Dear Buddy M.D,

I am prone to dry skin on my scrotum.

I’ve experimented with different moisturisers. Coconut oil works well on my face and foreskin, but causes redness and a very bad itch on my scrotum. When I washed it off the itch disappeared, so I assumed it might have been an allergic reaction to the Coconut oil.

I’ve also tried Shea butter, but it gave me similar results.

Would you recommend Aloe Vera gel?

The very front of my balls itch very bad around the seam where the 2 balls join. They itch on both sides of the seam. When they itch, they’re slightly red and the skin in that spot looks sort of inflamed. I work in a very dusty environment and i sweat alot. I’m also bad about not wearing underwear. The spot that itches is where my pants rub my balls. I shaved it almost a year ago and no hair will grow back in that spot. Hair also won’t grow on my lower legs because of the friction of the pants combined with the dust. My balls don’t itch every day, and they’ll even itch when I’m off work. The itch is intense and feels like poison ivy. Scratching makes it worse. I’ve used jock itch spray, i even sprayed athlete’s foot cream on my balls. Anyway, I’m also sexually active with multiple partners. I’m just wondering if my problem is heat and irritation and maybe fungal, or if it’s an std. Also you may want to note that the pants i wear at work with no underwear are cleaned by a uniform cleaning company, they’re cleaned with some harsh chemicals. I started wearing boxer briefs but sometimes my balls itch, sometimes they don’t. Even when they don’t itch the skin on the balls and around the base of my penis feels real dry.

This looks like dermatitis. You may keep using Zincast. Wear loose hypoallergic cotton underwears only. Let the area be clean and dry. Avoid friction over the affected part.

If it doesn’t get better in a day or two, you may need to see a doctor for physical examination.

Redness and itching began on scrotum, has spread to underside of penis over course of 2/3 days, no raised bumps/ sores/blisters etc or burning just redness and itching but appears to have circlular spots but not raised.

This may be a fungal infection. Keep the affected area clean and dry. Wear loose cotton underwears to avoid moisture accumulating there.

You may start using an anti fungal powder thrice daily over the affected parts. Candid powder may be used.

I have small red like bumps slightly raised on my scrotum that are centralized on my right scrotum. I noticed them first about 3 to 4 months ago after running a few miles and it was near the spot that my underwear was causing friction with my scrotum.

The area on my scrotum(towards the front of my balls) started itching profusely (about 3 to 4 months ago). I went at began applying Lamisil to the location and it seemed to help with the itching. After about 4 to 5 days, the rash seemed to go away. I then began exercising again and shortly would notice the rash. This has been going on and off for about 3 months. It seems to go away when I put an over the counter antifungal cream on it but as soon as I stop applying the cream it comes back and will itch in the one spot. The rash does not spread at all.

I also wash regularly and dry off and use talc powder after I shower to try and keep everything dry down there. However, it seems like this rash will leave for a few days and then come back.

I’m wondering if my problem is STD, fungal or a bacterial infection. Please help!

Likely to be fungal. Such infection tend to come again whenever they get appropriate conditions to grow.

Keep the area clean and dry throughout the day. You may need to wash it with ample water after every exercise session. Little talc may be used to keep it dry. Wear loose cotton underwears to let the area breathe throughout the day. Avoid wearing tight bottoms for some time.

If you have itch, better keep using an anti fungal powder over the affected area thrice daily. It may take around 10 to 15 days for the infection to go completely.

I have been suffering from this since about a week. First there was only burning itching on the thigh area around my genital. When I checked it was red. I thought it may be due to some sweating. So I stopped scratching it and left it alone. After a few days I started feeling pain in skin of some areas on my ball sack. Now it really hurt and there was a stinging sensation even at the lightest touch on it. Sometimes even if I didn’t touch it, it would still burn and pain. There is no severe itching. The affected area’s skin has become dull and appears like dead skin. While washing today, it was hurting a lot. While I was washing my ball sack, the skin there peeled off and it was so painful like no pain I’ve ever experienced. It burned. Upon peeling off, it revealed moist red skin.. Which looked like fleshy…and burned like hell when touched or came in contact with water or anything at all. I had been using the candid dusting powder.

        As for your question..
Not on the scrotal skin.. No it didn’t work. But it worked on the inner thighs.
But it helped with the itching overall.

I’ve posted the same thing on the “Ask a question section”

The affected area looks inflamed. This may have happened due to some irritant like new soap, gel, or cream tried there. Such inflammations may also happen when you wear synthetic lower garments that rub against the skin.

For now, avoid all kinds of friction over the affected area. Avoid using hot water for washing. Wear loose cotton lower garments only.

Keep the area clean. Better wash the region with ample water, if you sweat too much or after every exercise session. You may start applying 1% hydrocortisone cream over it once daily for a week.

Let us know if it improves.

Is this likely to be jock itch?

Jock itch would give you intense itch there.

Could be due to collected sweat over the region. Keep the area well washed and dry. Wearing loose cotton undergarments would help. Avoid friction over the area. A little talcum powder may be used to aid this.

Some people have localized darker colored patches over the skin since childhood. Such patches are considered normal. They may arise due to abnormal distribution of melanocytes there or because those areas remained folded (less exposed to open) for a longer time during the growing years.

I have a burning sensation on the right side of my scrotum. If I stretch my scrotum skin surface, little tiny bumps are present. My GP responded to the reddish coloration. My GP prescribed Clotrimazole and Betamethasone Dipropionate cream. The burning sensation still present.

    Dear doctor,

Last June I was diagnosed with itchy scrotal eczema and GP prescribed Triamcinolone for up to 9 days.
Last August I was diagnosed with an inflamed rash on my foreskin and another GP prescribed miconazol/hydrocortison cream for up to 7 days.
Last September the yeast type of rash on my scrotum returned and I was diagnosed by my GP with a yeast infection on my foreskin and glans and he prescribed Ketoconazole for up to 4 weeks.

Today I got diagnosed with a yeast infection on my scrotum (again) and my new doctor prescribed miconazol/hydrocortison cream for up to 7 days.
The doctor also advised me to go to the local hospital for a blood test. (which I did) I am kinda worried now.
I don’t eat a lot of sugar or junk food and I am certainly not obese.

Do you have an educated guess about what it could be?

Are you taking any other medications, like steroids, for some other medical condition?

Getting frequent yeast could also be related to your daily hygiene. Factors like not bathing daily, wearing tight lowers, sweating too much there, eating more of junk food, may contribute.

I got an allergic reaction (severe itching) from the miconazole nitrate/hydocortison cream. So the doctor prescribed Fluconazol tablets 1 a week for a 4 week period. Still awaiting the results from the blood test.

    Hello Doc,
I’m having skin of my scrotum crack profusely, from extreme dryness/flaking skin. It seems to come and go starting with a mild dryness with mild itch from the sheading skin. But the mild dry skin on scrotum will peel or flake away. Then after 3-4 days the new skin will be dry/flaking away but increasing in dryness and severity, with each cycle. This time is the worst ive had of it though. It looks like the skin is tight, glassy, and intense sensation to peal it away. But I can no longer do this as there are fissures/cracks in the “acordianed” non exposed surfaces of the skin (scrotal sac is in a similar state as exposer to frigid temperatures-tight, tucked up) so im “crawling in my skin” to peal it away, but to painful.
My general skin type is very dry. Its like my body can’t produce oil to keep the skin healthy. At any given time threw out the year I can rub a leg, and it will be snowing skin. lol So i am very accustomed to dealing/tolerating dry skin. But ive never had dryness in this area. And never had dryness this sever anywhere on my body including cracked heals. Theres no rash or irritation. No bumps/oozing of new skin. There’s no rash or irritation, but I have had spouts of intense itching in the past sometimes so intense “eyes roll in the back of my head-euphoric”. I was sexually active then (2.5yrs ago) but not sexually active since. The intensity of sensation to itch has not been often, many months go by normal as anyone else, and occasionally a spout of moderate to intense itchiness would developed but only on a 1 time itch occurance. Never a persistent itch .
I avoid chemicals like fabric softeners, anything that has perfumes or dyes, and all that stuff that would irritate dry sensitive skin. So I’m thinking its not a reaction to a chemical/substance. Also I don’t have any allergies so I’ll rule out a allergic reaction, unless its a autoimmune reaction…
Long story short and if you have not guessed it, I have a issue about seeing doctors.
I’m not to fond of it, but the autoimmune possibility (if you think there is one??)
Would make seeing a doctor the only chance of fixing the issue..

    Other wise…
Have any good remedies, or your thoughts on 1-2 of the very very best remedies that come to mind lol

    Daily joke:
I was told at an early age, that I would NOT make a good doctor…
Because I don’t have any Patience……. “Patients” XD

My condition began when one evening my scrotum was red, flacks and the skin quite hard. I applied Anti-Fungal cream and the condition improved, after traveling a week later it returned. Subsequently oral Anti-Fungal and More cream resulted in a rash appearing on my inner thighs and disappearing after a few days. My scrotum was still red however no longer itchy. Since then my scrotum has been intermittently red and prickly/slightly itching. During the day it can almost entirely clear up but later become red and with a noticeable sensation. Steroid creams seem to offer slight improvement but redness and prickliness at times persists.

Likely to be fungal. It’s better to restrict on using the steroid cream. Use it not more than once daily.

You need to keep applying the anti fungal ointment. Such infections may take time to go. However, you should be able to see improvement.

Three weeks ago I noticed a red coloration on the right front side of my scrotum. I thought it was trapped while sitting for too long and it would go away. But then I noticed days later that it didn’t. I ignored it, then I noticed that it’s not only the right but also the left. So I have like an upside down heart shaped redness on my front scrotum, slightly blending to the sides but the border is blurry yet distinguishable.

I had been itchy down there for months, but never attributed anything to it. Until I had a firy instance, I decided to look closer.

I noticed the skin was really shiny. At one point it would start to flake, but it was stubborn to come off. I also started getting patches of this shiny skin on the shaft.

Admittedly at times, I would wear the same underwear for days, even after working out. Gross but that habit has changed.

I went to the doctor and she said I had a Candida fungal infection. I was prescribed seven days once a day of Fluconazole 100, and twice a day of topical Ketoconazole 2%. I have also been airing things out, keeping things clean. I have finished the pills and still using the cream.

The flaming of the skin seems to have subsided, the itching seems to have decreased. The shiny skin seems to have decreased about 75% but isn’t entirely gone.

But my main concern is, that the redness didn’t change at all. The redness exists strictly on the wrinkly area of the scrotum. It’s more noticeable when the scrotum is crumbled together than when it’s warm and lose. I didn’t have this a month ago.

    Hi Doc,

I was diagnosed with a yeast infection on my scrotum early December. The GP prescribed hydrocortisone/micanozole cream, but it seemed to make my symptoms worse. I went to the local hospital and the GP over there prescribed fluconazole 150mg capsules for up to four weeks. I’ve finished her course January 1th.

A couple of days ago I’ve noticed a light reddish and slightly raw and burning sensation on my whole scrotum. It’s not itchy or scaling, but it does feel raw and burning almost like a sunburn. I tried to sooth the skin with petrol jelly, but to no avail. The results from a recent bloodiest showed no diabetes or thyroid problems.

    Any thoughts?

Better not use any medicines now. The yeast is likely to have got cured with all those tablets and medicines.

Just give the area some rest and time to heal up. Avoid friction over it. You may keep applying any emollient/ petroleum jelly over it.

My testicles are red and itchy. The skin on the sides of the testicles is flaky and Peeling off in some parts there are red spots where the skin has peeled off. My inner thighs are also red. My testicles leave wet areas on the sides of my underwear also some white stuff which I assume is my skin. My testicles and the wet areas on my underwear also have a strange smell, which seems very familiar but I can’t figure out what it reminds me of. It may be related to excesive moisture. Is the redness a side effect? It doesn’t itch but my penis (glans) looks wrinkly/leathery from time to time. In the evening my scrotum looks a little better, especially when the skin stretches. Looks like a local irritation there, not a disease.

Commonest cause of irritation may be dryness. Wash the skin well with ample water during bathing. Avoid using any soap or gel over it. Dab dry and apply a thick, non perfumed emollient/ moisturizer over it. Avoid using very warm water fer bathing.

Another reason could be rapid temperature fluctuations. To prevent this, where loose cotton lowers during your sleep. Avoid too much heat in your bed room.

Hi doc, my rash started in my toes, spread to back of hands, chin and scrotum. Scrotum by far the worst part, when contracted in cold the skin is horribly wrinkled and feels thick and scaly, very uncomfortable, in warm water or environment it’s not as bad but still very red and raw (can see clear colour change boundary) I freaked out googling it and got checked for STD as result (clear) 3 GP doctors have just prescribed antihistamines and steroid cream and although the tablets help with itching cream not doing much good (been6weeks now). Since started in toes I obviously thought athlete’s foot but no joy with anti fungals. I was using lots of cleaning products with methylisodazonolone (spelling from memory here but hope you know what chemical I mean) so I’ve kind of diagnosed myself with contact dermatitis as a result, does it sound plausible? The folds of elbows and knees have also become affected but perhaps 2 weeks after the initial outbreak and eyes are also itchy when irritated at different times of day. I’ve been avoiding this chemical but it was in my bleach spray, washing up liquid, laundry detergent and surface wipes. I’ve applied numerous types of moisturisers emollients etc but they seem to annoy the head of my penis driving me crazy with uncomfortableness. Last point worth mentioning I got an impacted wisdom tooth out 48hours prior to first symptoms. Also for clarification I cleaned my house barefoot several times which could explain toes as initial point of display? Sorry for long post but I just don’t recognise my body anymore from intimate areas to my hands and face and I’m always uncomfortable so hard to even forget about it!! Thanks for any help

No oozing, (I do occasionally see fluid on toes I take to be sweat) if I squeeze my ballsack so the skin contracts there are very tiny intermittent red specs I take to be broken skin from scratching, quite itchy, I’ve noticed more dandruff in my hair but I think I’ve maybe kept skin elsewhere from peeling due to the shear amount of specialist moisturisers I’ve been using (aveno, oilatum, epaderm etc) think I’m going to need to see a dermatologist?

I have since begun doing some hygeine in the morning and I ditched wearing sweatpants at night to sleep in and I either wear boxers or nothing at all the redness seems to have subsided some some of the skin is returning to normal color so idk if it’s contact dermatitis or something?

I moved into a new apartment with a radiator that is very overactive, that which I’m not sure if it correlates or not. I have tried a lot of things under the sun and eventually went to a dermatologist, he said it was ezcema. I was prescribed elidel and it did seem to work however I was only told to use it for 2 weeks, since I stopped it came back just as bad. I should note I noticed a formation of tiny white bumps on my sac, not blisters but bumps that also sometimes itch. Other things I have tried are aloe, gold bond powder, almond oil, etc. please let me know what I can do to rid this, also, I have a new GF is this transferrable? I’m not 100% sold it’s eczema.

Avoid over heating of your bedroom during sleep. Also, weal lose cotton lowers during night.

If you sweat a lot, fungal inflections need to be ruled out.

      Eczema is not transferable.

Very likely to be a fungus, may be trichomycosis, where the fungus affects the hair over the affected area. Over the counter anti fungals may not work. Oral dose may be needed.

You need to see a dermatologist for diagnosis and treatment.

Meanwhile, keep the area clean and dry. Avoid using soaps or gel there. Clean it with ample plain water only. Dab dry, avoid rubbing. You may clean with rubbing alcohol, as you did, once in a while. Avoid friction over the affected part while walking.

Redness of the affected area – Yes
Swollen – Between 8 to 10 little(pimple-like) bumps when counted.
Any pain – Yes (burning sensation) when touched but no discharge.
Is the affected area expanding with time? – Not expanding because I have been washing with diluted Apple cidar vinegar and application of Penicillin ointment.
Since when do you have it?- 8days
Have you had a similar problem before? No.
NB: Whenever I stop the application of Vinegar and penicillin ointment, its start to expand and itch.

Start using an anti fungal powder over the affected region. These are available over the counter. Miconazole/ Ketoconazole may be used twice daily, including once at night time.

    Hi Dr,

I got diagnosed with intertrigo/yeast infection a while ago. I’ve used Ketoconazole cream for six weeks on my scrotum and penis. I’ve also used Diflucan for three months. I’ve stopped the topical application of Ketoconazol and I’m approaching the end of my Diflucan course. My scrotum is less red but still slightly reddish corners. My doctor said it’s most likely erythrasma of the scrotum and presribed two claritromycine capsules. The glans of my penis looks very wrinkly and a bit irritated or dry. Any clues why it became wrinkly all of a sudden?

Likely to be due to the long treatment over the area, which usually alters the pH of the region.

It change would gradually revert back to normal. Take a good diet, consuming fresh fruits, vegetables and any probiotic supplement. This would help in early recovery.

Doc can u tell me the difference between inverse psoriasis and eczema .I have a rash which is red and changes to pink and dulls but if I scratch becomes red again and itches, it’s like a vicious cycle of scratch and itch but it develops a white layer some times .it’s mostly moist to touch but not wet ,there’s no dryness and the white layer has a foul smell. when I wash it the white layer goes away and forms after some time the tag is warmer compared to the test of the body but I guess it’s natural add it’s in my groin . Oh and whenever it itches, it is warmer than usual and more moist and a bit sticky but when it dosen’t it it is dry and cold what could it be

This sounds like inverse psoriasis due to the characteristic white layer that forms over it, however, diagnosis requires a physical examination by a dermatologist.

    Hi Dr. Buddy,

I’ve been dealing with a chronic scrotal itch — dry, scaly, flaky, itchy. I notice that the the top layer of skin is all broken up, i.e. not a continuous layer but a many different scales that upon itching flake off. For the most part, this only occurs during the night time.

Interestingly, during the day, my skin is intact and does not scale or peel even if I itch. As night comes, the skin just all of a sudden loses its integrity? and when I itch the scales and flakes form.

Sometimes the skin looks like it forms fissures or grooves in the skin. This frequently bleeds when I itch. No sexual history and not sexually active. I’ve also noticed some darker round pigmentation around the scrotum. About 1-2 mm spots that are not raised. Is this concerning?

sorry for the multiple questions and thank you in advanced if you take the time to take a look at this.

After running a big distance out of the blue, I fell ill and couldn’t chane the clothes in which I ran for a day and half. After feeling a slight burning sensation, I checked my scrotum and found that vast red patches had developed all through the right side and some on the left too. Assuming it to be a funal infection, I applied Clobetasol Propionate cream in the area. Now some of the patches have dried up, but there are gash-like wounds on the lower right and upper left portions of my scrotum. It burns so much that I can’t even move. I have been applying anti fungal cream for a day now, but the patches haven’t dried. What do I do?

Are you sure it’s fungal. Fungal gives you intense itch. If its just burning, may be local irritation of the skin in response to excess of sweat and secretions produced there.

Clobetasol Propionate cream is a steroid. To be used with caution.

You may use an anti fungal like, ketoconazole cream.

    Hi Doc,

I have a red rash underneath my scrotum which has been there for about a year now. I’ve tried all sort of remedies including anti-fungals (Lamisil), anti-itch creams with hydrocortisone. I have applied them to the affected area for the prescribed amount of time, but no change in the area. I have also tried home remedies such as tea tree oil, coconut oil, aloe-vera plant to try to soothe the rash. I have been wearing cotton underwear and washing the area with soap/water and then completely drying it with a fan before applying any ointment. Also, during the night I usually put the fan on, so the area is getting plenty of air. With the amount of effort I’ve put, this rash should be gone by now. However, the rash is not going away.

The rash is red, roundish in shape, and just underneath the scrotum. The rash is very sensitive to touch. For the most part, the rash only itches when it is rubbed up against something like when I walk or sit. However, It is near impossible to avoid friction in the area, because it’s just underneath the scrotum. Whenever I walk, the movement causes friction against my skin, and when I sit the rash rubs against my skin/underwear.

It is important to note – I am currently 22 years old, but when I was younger sometimes I would not wear underwear and I guess the underneath of my scrotum would rub up against my clothing when I would sit. Overtime, this may have caused this rash. So a possibility is that the area might be chaffed skin via continuous friction. If so what type of medication would I used to heal chaffed skin?

Any suggestions would be much appreciated.

Just not wearing an underwear is unlikely to cause any pathology.

These may be some skin condition that needs to be diagnosed, may be localized dermatitis. Have you seen a dermatologist for this?

Avoid cleaning it with soaps or gels. These may render the area dry and weaken the skin. Just plain water washing is enough.

For the last 8-10 months I’ve dealt with issues ranging from what looked like jock itch early on to the most current symptom which now acts like a soap allergy.
During this time, my wife has also battled yeast infections – we’re sexually active and exclusive but only a couple times a month due to her condition.
Mine doesn’t seem that bad but won’t go away. Sweat doesn’t seem to bother me much, but if I use soap in the shower I get reddish scrotum with discomfort – when at its worst it’s a little itchy but more so raw feeling and can take on a shiny characteristic. I’ve tried everything from men’s body wash to dove to tea tree wash – even my wife’s Vagisil, but the best seems to be not washing. I’ve done a couple hydrocortisone creams but nothing seems to work long term. Also, I used to regularly use a powder like gold bond, but that is also irritating now. Thanks you.

    Hi,

For a long time I always had an itch near the bottom of my scrotum where the crease is. It started years ago when somehow it bled a little turned into a scab. I mistakenly scratch the scab, and long story short it forever became itchy and occasionally dry up and I would peel the dry skin, sometimes with a minor scab. Now for past few years the scab is no longer there, but the exact same spot on the crease gets itchy from time to time and I scratch.

I usually put hydrocordizone to remove the itch, usually at night when it keeps me from sleeping. I have tried Lotrimin 1% clotrimazole before, but no success.

A week and a half ago I decided to put 1% clotrimazole again, this time with an off-brand ‘Family Care’ from Amazon. Immediately it reacted with my entire genital area being very itchy, and one side of penis skin being swollen. I tried for 2 more days hoping it would get better but it did not. I switched to putting hydrocortizone to remove the itch (which I’ve read in your past comments its NOT recommended in this situation), but now I have this pain in that area around the scrotum which feels like needles poking and is intense enough to wake me up in the middle of the night.

I have now switched to not using and cream, and now take Advil/Tylenol to alleviate pain until I find another solution.

From your past comments, would the correct action be to continue using the clotrimazole cream (maybe brand name Lotrimin this time?) while taking painkillers until the issue is resolved?

    Thanks.

    Hi doc,

I have a red and sometimes brown rash on my scrotum with light borders. I have no pain, itch or burning sensations, but it feels sticky and clammy. I’ve been on antifungals, so that shouldn’t be the cause. The discoloration and irritation gets worse when I wear red or orange cotton pants or pants that were dry cleaned. My penis is unaffected, but I do get irritation and cuts after dry cleaner, colored pants. I never had issues with wearing pants from the dry cleaner before tho.

My doctor thinks it’s due to sweating, but my underwear is never soaked. Yesterday at 3 pm it was red, slightly swollen, but around 11 pm it was gone. I’ve tried vaseline and an antifungal with hydrocortison, but this made it worse. I am not overweight and in overall good shape.

    Any thoughts?

    Hi Dr,

I have a mild red rash on my foreskin and shaft after a masterbation session. It feels slightly burning. I am applying petroleum jelly.

I still have a tube of triamcinolon in my home. Can you give me instructions how to use the triamcinolon?

Just apply the ointment over the lesion, with out rubbing it. Use it twice daily.

Avoid using it for a long duration.

Better to wash it daily with ample water. Ensures better hygiene, as it washes away all secretions from over these glands. This is particularly essential in early teens and young adulthood.

    Hi doctor,

Is it normal for a 30 year old guy to have 6-10 drops of urine leaking after urinating? I usually let it leak onto some toiletpaper, but I wonder if this makes my skin irritation on my scrotum (they think due to sweat) even worse. I have an appointment with my dermatologist soon, should I discuss this with him or is it normal?

That’s normal. However, do report if you have any burning during urination, itching there or redness over the penile tip.

    Good evening,

I am having mild eczema on my scrotum and I am applying mometasone 0.1% ointment.

Do you think the schedule below is effective?

    Week 1 – daily, once a day
    Week 2 – 4 times, once a day and 3 days nothing
Week 3 3 times, once a day and 4 days nothing
Week 4 2 times, once a day and 5 days nothing
Week 5 1 time, once a day and 6 days nothing
    Week 6 stopping

You’ll need to follow the prescribed schedule and see if or not it’s effective for your case. Mild eczema is usually controlled by single daily application. People usually need a month or two to over come it.

        Thanks doctor,

Is it safe to use mometasone 0.1% ointment for two months? I’ve read something about skin thinning.

It does thins down the skin in prolonged use. However, applying a thin layer over it for a month would be safe.

    Hi,

I have noticed two very tiny red dots on my shaft and the skin of my glans looks wrinkly a couple of hours after applying petroleum jelly. It’s not itchy and the burning sensation is mostly gone. Does this indicate an allergic reaction?

Likely to be a pimple out there. Avoid squeezing it.
You may start applying any topical/ antibiotic cream over it, twice daily. Likely to regress with time.

    Good morning,
For six months now I’ve had the area around my testicle all red and itchy, it stops on the legs folds and it goes down to the perineum, but no inflammation on the actual testicles.
I’ve tried all kinds of antifungal creams thinking it was jock itch, but it didn’t work, I’m very clean and don’t sweat a lot, I don’t know what to do now.
    Thanks

    Good day,
I have the same problem but it didn’t itch, my concern now is that the redness and some markings/spots are showing and growing, is there a way for it to go back to normal? it started last October and slowly spreading this November.

hi, so I’ve been dealing with this for a few years and embarrassed to go to the dock, i work as a cook in a kitchen.
I often find when I get home the underside of my balls are sweating and very moist and itchy, so I will scratch and look and will see extremely dry and flaky white skin. They don’t really bother me throughout the day except when I get home from work and can typically resist itching until I get home.
Its is also accompanied with a musky smell when i drop my briefs to itch

Have you tried any anti fungal powder over the area?

This area tends to be moist. And since you work throughout your day, not getting a chance to aerate the region, its possible that moist keeps building up there. This may aid fungal growth in the area, giving you itch.

My scrotum stays sticky all the time. Minor itching. Have tried all the antifungal, including the oral capsules. Use corn starch several times daily. Stickiness is the main problem.

Unlikely to be fungal. Looks like a skin condition. You need to see a dermatologist fol physical exam.

Good morning. I’m a dark skinned man. I had jock itch but I ve treated it using Fluconazole capsule and Antifungal cream. My problem now is that part of my scrotal region is discolored. It fair colored and partly reddish. It doesn’t itch me that often

The area is likely to blend back in same color with the adjoining skin. This may take some time.

Hello, I’m a 38 year old man…woke up today with itchy scrotum. The wife and I had fairly vigorous sex two days prior and I had showered before then, did not shower yesterday and went on a longer walk with the kids. I’m not overweight and I’m not sure if its chafing or not. Scrotum is very red, itchiness subsided almost entirely after a thorough shower this AM.

To follow up, doesnt seem like jock itch as theres not a ringworm like rash, just red scrotum, seems a bit warmer than normal as well.

This could either be some allergy or jock itch. Unlikely to be an infection.

Since the onset was sudden, it could be some allergy from lubricating jelly, condoms or something else used during the act. Excess of sweating could also be a probable cause.
