Hemorrhagic Corpus Luteum Cyst

Q: My wife had a cyst on her uterus. It is 4 cm in size. She took ultrasound scanning from hospital. The report is- right adnexal hemorrhagic corpus luteal cyst. Is it a dangerous one? How can I treat this?

Let us first know what a hemorrhagic corpus luteal cyst is.

The ovaries release an egg every month. After the release of the egg from the ovary, the sac containing the egg forms a closed follicle called corpus luteum.

This corpus luteum stays in the ovary and secretes pregnancy hormones in anticipation of conception (pregnancy), while the egg travels through the fallopian tube. If conception does not occur, the corpus luteum disintegrates.

The failure of corpus luteum to disintegrate or get resolved in a normal course results in the formation of a cyst, called corpus luteal cyst.

Sometimes, when the egg gets released, it may injure some small blood vessel of the ovary. So, the resultant corpus luteal cyst is bloody or hemorrhagic in appearance.

Possible Complications

  • This hemorrhagic cyst may rupture leading to internal bleeding. This may give severe pain to the patient and result in considerable blood loss. The condition is termed as Hemoperitoneum, where the blood gets collected in various pockets formed by the peritoneum.
  • The cyst may grow in size to become big. Such a big structure may press upon the adjoining visceral organs and cause trouble.
  • A big cyst may sometimes twist the ovary. This condition is a painful medical emergency called torsion of the ovary.
  • The cyst may get infected.

Management and Treatment

If your wife is without any symptoms (complains), it would be better to wait watchfully. Such cysts commonly get resolved on their own within 2 to 3 months. They get absorbed and gradually shrink in size.

If she has pain, looks anemic, has nausea, vomiting or any other complain, it is advisable to intervene at the earliest and go to the ER. Surgical removal may be considered.

You’ll anyway need to keep observing the cyst by follow up ultrasounds. If the cyst grows in size, there is always a risk of rupture. In such cases, surgical removal is indicated.

      You need to treat the cyst first. Continue with the treatment your doctor gives.

      Once the cyst is treated, you may plan conception. There should be no problem.

    I went to the ER with severe pain 2 days ago,and they told me I have a hemorrhagic ovarian cyst in my left ovary. I’m confused…. does hemorrhagic mean it’s a ruptured cyst? They didnt use the word ruptured, but the name made me assume it was and now I’m confused? They sent me home with pain meds and told me to follow up with my Dr. How long does it take for the pain to go away? They told me it would get better each day, and so far I havent noticed any difference.

      Yes, it’s ruptured.

      Hemorrhage from a cyst is like profuse bleeding from it. Blood gets collected in spaces available nearby, including the pouch of douglas. Gradually, with time, the blood is absorbed and the ruptured cyst heals up on its own.

      In some cases, where the amount of bleeding is too much, surgical drainage may be needed.

    hi , I have a hemorrhagic cyst of 1.5 cm and junctional zone thickness of 14 mm on day 25 of mt last cycle. I have been trying to conceive for last 3 months with no positive outcome. I am 32 and My cycles are otherwise very regular of 28 days with no pain. Is there a issue ? whether i be able to conceive naturally or should take help?

      Hemorrhagic cyst alone is unlikely to be interfering with conception. Cause why you’re not conceiving needs to be investigated. It’s better to see a doctor for thorough check up.

      Both, you and your partner, need to see the doctor.

    Hello,
    I had a medical miscarriage (took cytotec at home for a missed miscarriage) and was given an ultra sound after several days of pain. The pain has finished and The ultra sound showed everything else was clear and normal but mentioned a 14 mm hemorrhagic corpus luteum on the left. Is this anything I should be concerned about or ask the doctor about? They didn’t mention it to me, I just saw it on the results.

      No, unless if gives you any pain or discomfort.

      Most of such cysts regress on their own with time. No medical treatment is required.
