Scrotal Bumps, Cysts and Lumps

Bumps may appear on scrotum for variety of reasons. We may broadly divide them into 2 categories:

  • Bumps over the surface of the scrotal skin
  • Lumps inside the scrotum sac

Let’s start with the bumps that appear over the surface of scrotum. As we will see, many of these are part of normal skin texture and some are simply benign individual variations.

It is very important to know about the normal skin texture of the scrotal sack. Most of the hair bumps, that guys worry about, are the normal hair follicles. They look raised because the scrotal skin is thinner as compared to the skin elsewhere in the body.

These are normal, but frequently become a cause of worry just after puberty when hair start to appear on the genitals.

Folliculitis

Rarely one of these hair follicles may become inflamed, sometimes when one tries to pluck or shave the hair, leading to folliculitis. The follicle may also get infected with bacteria called staphylococcus aureus.

It appears as white-headed pimple around the hair follicle that may be itchy. It may later turn red and painful.

It may disappear on its own with proper hygiene, but sometimes require medical treatment. Warm compress, few times a day, may provide some relief. If it doesn’t improve with supportive measures or get worse, then an antibiotic treatment may be required.

These are visible oil-secreting sebaceous glands that are found in large percentage of population. These are considered normal structural variant rather than a disease. Sebaceous glands secrete oily substance called sebum, that keeps the skin smooth and lubricated.

Fordyce spots appear as small, red or white bumps on the scrotal skin, that are typically painless. They are considered benign and require no treatment.

A painless lump over the scrotal skin is very likely to be sebaceous cyst. It is also called epidermal cyst. These cysts are common in younger age group.

Skin that bears hair is kept soft by the oil (sebum) secreted by sebaceous glands. These glands open into the hair follicles.

When the ducts draining the glands gets blocked, they become distended by their own secretions. Eventually, enough sebum accumulates to form a smooth, firm nodule in the skin. This painless cystic swelling is called a sebaceous cyst.

Features

Such a swelling is discrete or separate from the underlying testicular tissue, that is, it’s not attached to the underlying surface. It is present on the skin surface over the scrotum. It is freely movable, smooth and soft to firm in consistency.

Sebaceous cysts are painless unless they become infected.

Complications

Inflammation

Even when the cyst is not infected, it may get irritated and inflamed due to friction with the trouser or scratching. The lump may become swollen and tender to touch.

Such a condition is treated by providing rest to the cyst. Avoid friction or any other irritation. Keep it clean and dry. Also, anti-inflammatory medicines may be given.

Infection

Numerous bacteria are present in the groin region. Also, the region remains moist. All this predisposes to infection. Scratching the cyst or breaching the skin by any other means may allow the bacteria to get inside and cause infection.

Infection is treated by antibiotics. Pus, if any, needs to be drained out.

Rupture

The cyst may rupture inside its capsule. After rupture, the chances of infection may increase. Such a cyst presents as a boil-like abscess.

Discomfort

Friction, as in movements, may cause discomfort. Sebaceous cysts may also cause difficulty while urinating or mating.

Malignancy

Sebaceous cysts are usually not cancerous. However, a small percentage may show malignant changes.

Treatment

If a sebaceous cyst is small and causing no problems, it may safely be left alone.

For inflamed cysts, a corticosteroid injection may be injected into the cyst to reduce inflammation. Triamcinolone acetonide is a synthetic steroid used for the purpose.

Sometimes these cysts become infected. This happens because bacteria are already present in the groin. Even little scratching or friction while walking may lead to infection. An infected cyst is red, swollen and painful. It may discharge a foul-smelling pus. Such a cyst is treated with antibiotics and then excised to prevent recurrent infections.

Larger cysts may cause friction and are unsightly, so are usually excised under a local anesthetic.

Surgical Management

The medium to big sized cyst, containing sebum or pus, needs to be drained. This may be done by making a small surgical nick on its surface and subsequent draining.

Alternatively, the cyst may be excised surgically along with its capsule. This method is particularly chosen when the cyst has a tendency of recurrence.

Other Possible Causes of Bumps on the Scrotum

A Pimple

These are extremely common in youth. They are painless, unless they get infected. They are small, firm to hard to touch.

Pimples are left as such unless they give some symptom. A painful pimple may be infected and may be treated with topical antibiotic creams.

Wart or Herpes Infection

Such conditions are almost always painful. You need not worry about them, unless you’ve had recent sex with somebody infected.

They may present as an ulcer or a bump. The lesion has a tendency to spread to adjoining areas.

Jock Itch

This fungal infection presents with circumscribed itchy rash consisting of multiple, pinpoint pink bumps. The rash is raised, reddish pink, flaky and very itchy. You may read more about jock itch rash.


Causes of Lumps Inside the Scrotum

Till now, we have discussed the possible causes of bumps present over the scrotal skin surface. Now, we will discuss the causes of lumps inside the scrotal sac. These include:

Spermatocele

Spermatocele is an abnormal cystic growth of the epididymis, a coiled tube on the upper side of testicle, that transports and stores sperms. This abnormal sac or spermatic cyst contains dead sperm cells and fluid. This usually does not cause pain.

Spermatocele doesn’t usually require treatment. But sometimes it may grow large and tense. Then it may require surgery.

Varicocele

Varicocele is an abnormally swollen or knotty varicose vein present along the spermatic cord. Essentially, it’s an enlargement of the veins that drain the testicles.

Usually it doesn’t produce any symptoms. Rarely it may produce pain that may be perceived as dull discomfort or heaviness. It may increase on prolonged standing or later in a day after exertion. It may get relieved after rest.

Mostly varicocele doesn’t require any treatment. If the pain is mild then it may get relieved by resting with lying on your back. You may also take pain medications. If the pain is severe or accompanied by swelling or infertility problem, then you may need to undergo a surgery.

Hematocele

Hematocele is a collection of blood in the pouch. It occurs after surgery or injury involving this area. It may also get consolidated and become hard with time. There may be some discomfort as well.

If it is small and painless, then foot elevation and bed rest may be sufficient. Otherwise surgery is performed to drain the accumulated blood. Ultrasound is done to diagnose the type of swelling.

Hydrocele

Hydrocele is a sac filled with fluid around the testicle. It usually leads to smooth and uniform enlargement of the scrotum.

Epididymitis

Epididymitis is a pain and inflammation of a curved and coiled structure at the back of testicle called epididymis. It normally stores and carries sperms.

Inguinal Hernia

An inguinal hernia occurs when there is a protrusion of abdominal contents through a weak area in the abdominal wall that passes via inguinal canal (groin) into the scrotum.

Testicular Torsion

Testicular torsion occurs when testicle rotates and twists the spermatic cord through which it is suspended. This results in cutting off the blood supply to the testes and consequent necrosis.

Injury to the Scrotum

An injury may also lead to the swelling due to oozing of fluid and blood at the site of trauma.

Tumors

Testes sometimes is affected with an abnormal growth of tissue due to tumor or cancer.

Further Evaluation

For further evaluation, the following may be asked on examination:

  • Does the lump hurt when touched or squeezed?
  • How does the swelling feel- liquid, firm, or solid?
  • Is it smooth, twisted, or irregular in looks?
  • Does it seem attached to underlying tissues like testicles or the glands?
  • Self examine your groins to see if there are any enlarged lymph glands.

Call for Attention

A testicular mass is always a cause for attention and should be evaluated right away. The primary concern in such cases is testicular cancer. Hence, if the lump seems to arise from testicle or attached to it, get it examined by a doctor immediately.

Making a Diagnosis

The doctor will examine the lump. The following tests may be done to help diagnose a scrotal mass:

  • Ultrasound of the scrotum
  • Biopsy of the lump

      If it can be felt distinctly from underlying testis, and can be felt almost as part of skin, then the chances of sebaceous cyst are higher. But other causes (as explained above) should be considered as well.

            I discovered a lump red in color very hard a few days ago now it’s doubled in size irritating It feels squishy to the touch now like fluid the skin is peeling off the bump.
            This however is on the shaft area I haven’t noticed leakage yet but I feel feverish and I have no energy I did go to my local er and he said he didn’t know and to see a specialist.
            The increase in size and irritation is what is freaking me out any help is much appreciated.

            May be a cyst that has got infected now. You’ll need prescription antibiotics for it. Also, since this is a big one, your doctor may need to drain it at his clinic.

            Avoid friction over it. Any antibiotic ointment may me applied over it, till you get to see your doctor.

            Likely to be a sebaceous cyst, filled with thickened sebum inside.

            It needs to be excised out aseptically and completely. A surgeon would be the best person to handle it.

        If no, these bumps are likely to be normal. They may be Fordyce’s spots. You may read about them in the above written article.

    A white small bump appeared out of nowhere. Does masturbation has to do anything with this? How can I get it removed? Will it go on it’s own?

      It’s unlikely to go on its own, but if uncomplicated (that is not infected and not growing) then it is usually left alone. If it feels unsightly, then the surgeon can easily remove it under local anesthesia.

      As such masturbation does not have much to do with it, but it may contribute if it leads to moisture or unhygienic area of skin.

      It sounds like a cystic lump, may be a sebaceous cyst or any other. But you need to be watchful.

      Though testicular cancers are very rare, it would be better to examine your scrotum for any suspicious change. Self examine during or after a bath, when the scrotal skin is relaxed and can be easily moved.

      Self examination of the testicle

      Gently roll the testes and try to see if there is any change in its consistency. A normal testicle is smooth in texture. Any roughness, if present, should be noted. You will feel the epididymis. That is a normal structure often confused as being a lump.

      The skin over the testes should be easily movable. If you feel that it is adherent to the underlying structures at any place, again note it. Also look for any gland enlargement in the groin region.

      It would be advisable to observe this lump at regular interval. If it is a simple cyst, nothing needs to be done till it pains or shows any other symptom.

    I have a soft bump on the upper part of my thigh. Initially I thought it was a pimple, so i did nothing but scrub it. Now it seems to have gotten a bit larger. Its soft almost balloon-like.

    I’m extremely nervous about it, not to mention it’s not pleasant for any girl to look at. I’ve had it for about a week and a half. I was hoping it was just disappear but it hasn’t.

      It’s not a good idea to scrub or pull it. You may leave it as such, undisturbed. There are pretty good chances that it is a pimple, like anywhere else on the body. It will go away on its own.

      If it grows, pains or leaks out, intervention would be required.

    I been having this lump under my left armpit and it’s swollen and it only hurts when I squeeze it. Sometimes the lump looks big and then it reduces its inflammation.

    I went to the doctor a couple of months ago and the doctor said it was an infected axillary cyst and needed a surgical excision. I wanted to know if this should be taken care of it right away since I been having it since 4 years ago. Also when I squeeze it this gray liquid comes out.

      It is recommended to go doctor at the earliest. Though the infection seems to be arrested and so you could keep it for 4 years, it is better to get rid of such lesions from the body as soon as possible. It is unlikely that it will resolve on its own.

      Such lumps may grow in size with time and may press upon important underlying structures like nerves and vessels of the region.

      The gray liquid appears to be a mixture of infected material, dead cells and tissue debris.

      Just check if it is freely movable over the underlying surface.

      If yes, it’s likely to be a sebaceous cyst. You may read about it above.

      However, if it is hard or fixed to structures below it, you need to go for an examination.

    I have had a lump on my scrotum for almost 5 years. It is kind of hard, white in color is movable and not connected to testicles. It has grown in size over the years and does not have any pain when I touch it. Can these sebaceous cysts continue to grow over time?

      Though these cysts are slow to grow, they may grow to give bigger swellings.

      As they grow, the chances of them getting traumatized during walking or getting infected increases. The cyst may become painful. Such cysts are drained or excised.

      You need to get it examined.

      It may be a swollen lymph node. This happens secondary to some infective cause. Such swellings are usually painful.

      It may be a lipoma. This is basically fatty tissue getting deposited underneath the skin.

    I have a grouping of these small whitish bumps on my scrotum. They are no bigger than the head on a ball point pen, movable and not attached the the testicles themselves, just on skin. There are about 15-20 of them total. What do you think they are?

      Likely to be clogged sebaceous glands.

      Just leave them as such. Wear loose cotton underwear. Drink lots of water.

      Avoid fatty stuff and have plenty of fresh fruits and vegetables.

      If you notice that they are increasing in number or developing some symptom, it is recommended to visit your doctor for examination.

    Good day! I’m Karen, I got sebaceous cyst removal last November 2013. And now it’s June 2014, again at my left armpit, there is this red with white dot thing same look like my sebaceous before.       It may be a sebaceous cyst again.

      It is common for these cysts to recur again and again after drainage. This is so because the capsule enclosing the cyst is still in place.

      The capsule cells regenerate the cyst with time.

      You may require an operation where the cyst is removed along with the capsule enclosing it. This is called total excision of the cyst.

    I’ve been checking my scrotum recently again and I’ve still got this small white lump. It’s not attached to the testis, it’s just under the scrotal skin. I’ve had it for about 2 years maybe more and when I squeeze, it hurts a bit but then it goes away and that’s the end and it hasn’t been infected and hasn’t gotten bigger. I was asking if it’s something I need to check up on or if it’s fine as long as it doesn’t get infected or bigger?

      This sounds like a cyst, may be a sebaceous cyst. It is benign in nature. You may ignore it and let it be if it doesn’t bother you.

      Intervention is required only if it grows in size, or becomes painful (infected) or gets attached to the underlying structures.

      You may still visit a dermatologist, if you want, and get it checked up to be sure.

    On my scrotum there is a sort of red lump. I’ve tried to get rid of it but there seems to be no liquid leaking from it. It feels smooth, and sometimes stings depending on how I squeeze it. I’m not old enough for any sexual activity so it can’t be anything to do with that. I’ve tried adding some spot cream to the area but it hasn’t seemed to work. I don’t know what it is, or where it’s come from. I shower often also. It doesn’t feel round when I touch it, it sort of feels long and it’s underneath the skin. I’d just like to know if there’s any sort of treatment needed.

      This appears to be a sebaceous cyst. You need to do nothing about it. It is likely to go away on its own with time. It may take 15 days or even a month, nothing to worry.

      Do not squeeze or try to pop it. All this would infect the cyst and complicate it.

        I’m starting to think I also have this I have a hard white lump under my penis. It’s freely move able but if move the right way it will dis charge white pus and the lump will shrink in size and I’ve had this lump for about 3 to 4 weeks but it seems to be getting smaller now on its own.           Likely to be a sebaceous cyst. It would regress in size with time on its own. Nothing needs to be done.

          You may try warm compress over it to soften its surface. This may also help in draining out sebum from inside it. However, do not force drainage at any point.

    Hello, I am concerned about some bumps on my scrotum. I have had multiple whitehead looking bumps on my scrotum for years now. I have tried to pop them, and a solid white pus comes out, but this is then proceeded by the bump growing in size. Two of these bumps have grown substantially due to me trying to pop them. But since, I have stopped messing with them. Until recently (years later), the largest of the bumps started turning red, and feels like liquid to the touch (unlike the others which are generally hard). I took a small needle to it, slightly punctured it, and squeezed all contents out of it. Tons of pus and blood came out, similar to a very large pimple. The bump still appears to be there, but it has reduced in size greatly. I would believe that these are pimples, however, I have had them for about 5-7 years, and they haven’t gone away.

    What are these bumps, and what course of action should I take with them?

      The white stuff which comes out of it is likely to be sebum. This is semi solid and pasty in consistency.

      You need not mess with the bumps. Popping them is likely to infect them, making them turn red and painful.

      Just let them be. Practice good hygiene in the groin region. Bathe daily, assuring that you wash the groin area thoroughly with plenty of water. Soaps or gels are not required.

      This would cleanse the area on a daily basis, washing away any extra sebum being produced.

      Wear loose cotton underwear only. Make sure that the area is properly ventilated throughout the day. Do not let moisture get collected there.

      Cut down on fatty food and fried stuff. Avoid junk food, they predispose to the formation of pimples.

      Instead, take plenty of fruits and vegetables in your meals. Drink lots of water.

      All these steps would lessen your tendency to get those bumps on the scrotal region. The bumps which are already there are likely to regress gradually with time.

      Likely to be an infected pimples, or infected hair follicle.

      You need to apply a topical antibiotic cream over it. You may use mupirocin (bactroban) over it, twice daily for a week.

      Avoid friction over these bumps. Wear loose lowers to assure this.

      Likely to be a sebaceous cyst. Don’t fiddle with it much. Just let it be. It’s likely to regress on its own with time.

      If it grows in size or becomes attached to the underlying structures, you’ll need to see a doctor.

      The ache is unlikely to be due to it. It is more likely to be muscular, due to some pull or wrong posture.

    Hey there, I have a small bump on my scrotum skin on the left hand side. I just discovered it it’s non painful and a little hard. I was wondering is this a reason for concern or is this a normal thing. Please reply, as I am quite worried. The bump seems to be under the skin and it is not at all painful or discomforting.

      This bump is very likely to be a sebaceous cyst. You may leave it as such. Do not press it or fiddle much with it. It’s likely to regress on its own with time. Take care to avoid friction over the bump. Wear loose garments to avoid it.

      You need to be concerned if the bump becomes attached to the underlying tissue, that is you are not able to move it freely. Other signs of concern are any pain or discomfort in the bump.

        In the beginning I pressed it a bit. Now if I do try and press it, it does tend to pain very slightly but if I don’t touch it, it remains without any pain at all. Is there any kind of warm compression that I may use to get rid of it faster, although I feel no discomfort at all or do I leave it as it is. Note, it only pains when I press it.

    A slight pain began a couple days ago. I felt very small lump that felt more or less like a bead of rice. It’s on top of my scrotum on the right side. It was a slight dull ache, but now it only hurts when I press on it. It seems to be attached to the testicle. I have no nausea nor other symptoms. Just pain on palpation.

    Hello, I experience some pain in the downer part of my left testicle when I touch it. And it’s like a soft lump in the downer part, in the region of the epididymis. I think it’s related issue to the epididymis but I’m very worried if it’s testicular cancer, when I’m not touching it, there no pain at all. Just when I squeeze it, I feel a lot of pain. Please if there is any other condition that causes a such problem in that region or it’s a cancer.

      This is very unlikely to be cancer. Either you have got some minor trauma there, or some infection.

      Don’t touch it often. Wait for a day or two. If it doesn’t improve, you may visit a doctor for examination. You may require antibiotics.

    I am 19 years. I recently masturbated when wearing tight boxer and since then a small lump has formed on top of my right testes inside my scrotum. It moves from side to side.       May be a cyst or a pimple. It’s not due to masturbation.

      Wearing a loose boxer would help. Tight undergarments predispose to pimple formation.

      You may leave it as such. Just avoid friction over the bump while moving and other activities. The bump is likely to go away on its own.

    I noticed a lump on my scrotum sack for about 3 years ago. It was hard and painless. 3 months ago, I noticed that it was dry ( like a dried pimple), so I tried pressing the whitish lump out but I couldn’t. I began to notice little pain on the lump since afterwards (though the pain is not regular because it comes and goes). However, I observed from last week that the lump has increased a little in size with red patches on top of it. It has also become little painful at intervals as well. Please what do you suggest I do?       Appears to be infected. Do not delay. You need to see a doctor and start appropriate antibiotics at the earliest.

      Very likely to be a sebaceous cyst, with yellow sebum filled in it.

      If small in size, such cysts may be left as such. They usually regress on their own. However, if big in size, they have a chance to get infected. So, they require surgical drainage.

      Also, your male hormones may be checked.

      No this is not normal. You need to get it treated. The fluid filled sac need to be drained. Such sac are prone to get infected.

    I noticed I had a series of small whitish bumps under the skin of my penis shaft, and I was hoping they would go away on their own but they haven’t. I noticed I also had the bumps on the other side of my shaft too, so they are on the left and right, but nowhere else. They vary in size, and they don’t hurt but I definetely want them to be gone. I also later noticed I have about 4 white pimples I believe on my ball sack, spread out. They seem pretty hard and not able to pop. Any ideas what all these bumps could be? And if so is it treatable, what do I need to do? And last but not least, just the other day I noticed I also have like 5 or so skin colored bumps at the back of my tongue, and I have had bad breathe and I believe it’s caused from that.

      Oh one more thing I noticed is that my penis also shrank in size it seems, not so much when I’m erect, but when I’m soft it seems that it’s much smaller then used to be and there’s a slight darker patch of skin now on my shaft that I don’t think was always there.

      Are these white bumps painful?

      If not, they are likely to be sebaceous cysts.

      Do not pop them. Keep the area clean and dry. Wear cotton based loose undergarments only. This is to keep the area well ventilated.

      If any of the bump becomes painful, you may need to apply antibotics over it.

        No the bumps arent painful, but I’ve had them for awhile. I’m worried they won’t go away on their own, and I really don’t want them getting worse or causing any problems for me such as making me sterile or making my penis nonfunctional. Also how about the warts in the very back of my mouth on the back of my tongue? I noticed my breathe has been bad lately, no matter how much I brush and I looked up bumps on the back of your tongue. I read something about bumps on the back of your tongue causing your breathe to smell bad.

          Bad breath is commonly due to some decayed tooth. Other causes include oral thrush or something in the food that is smelly, like garlic etc.

          The two problems may be not be connected.

          The bumps are unlikely to be infected and so unlikely to cause infertility. However, it’s better to see a doctor for physical examination.

      The pimples are unlikely to affect motility. Motility is linked with your daily diet. Eat healthy, avoid caffeine, smoking and alcohol. This would improve motility.

    About 3 weeks ago, I had an uncomfortable bump on my testicle, so I examined it when I got home from work. It was irritated, and red, like a pimple, which I’ve had a few before so I left it alone. After 2 or 3 days, the irritation stopped but it had grown bigger and started to harden. I squeezed it a little, but nothing happened and it was slightly painful so I left it alone. After a week it became really hard, fixed position, and the outside began to get hard and rough like a wart. I started to get a little worried, but I left it alone. 4 days ago, it had begun to rub on my jeans and get irritated again, and I gave it a squeeze, and just a little drop of blood. It hurt so I left it alone till 2 days ago. I was frustrated with it, so I squeezed it hard, which separated some of the skin, which ironically, didn’t hurt. Squeezing it was painful but the “head” of it peeled off without any pain, just a sore. I used medicine and bandaid till just a few minutes ago. When I squeezed it, it hurt and bled, but when I rolled my finger under it, it didn’t hurt and I pushed it upwards. To my relief, it “popped”. I did have a 1 inch long, almost solid, white in color “string” of material come out of it… I know it was not the smartest thing to do, but now I’m wondering what it was/is… I am treating it with antibacterial cream, and keeping the area clean, just wondering what it was… any ideas?

      You are using the right treatment now. It may take a week for it to heal completely.

    A week or so ago the area around my right ball sack became itchy, more so than usual. I am of the larger variety so friction seemed the likely culprit and I treated it as such with baby powder.

    The itch did not go away, getting so bad as to wake me up in the middle of the night. I kep treating it as heat related friction but the problem persisted. Then today I touched the area (after sanitizing my hands) only to discover a odd-smelling fatty substance is being secreted from somewhere (I can’t tell due to being unable to properly inspect myseld in that area). I’m considering visiting my local doctor because it is not going away.

    Any idea of what it could be?

    Since yesterday I noticed on my scrotum lots of white spots, I only noticed them because when I was taking a shower I saw a white spot and I thought it was shampoo, but as soon as I washed my scrotum I felt a burning sensation, so those white dots disappear when I wash them but they appear again after about 5-6 minutes slowly when it’s not wet anymore. I also saw some small fissures and 2 or 3 spots with no skin close to the white spots I was talking about. What should I do?

        It only itches when I walk too much since currently it’s hot weather. Therefore I went to the doctor today, she examined and said it might be dermatitis, then she recommended me to take Fexofenadine, Cefalexin and a cream called Novacort that contains Ketoconazole. But I’m a little worried as I also take rupatadine fumarate daily because of my Cholinergic urticaria, and on the package insert of the rupatadine medication it says it’s not recommended to take cetoconazol which means Ketoconazole in English. As Ketoconazole cream is only gonna be applied on the area, do you think this medication is ok? By the way fexofenadine and cefalexin are oral medications.

          It also says on the Rupatadine Fumarate package insert that Ketoconazole increases in 10 times a sistemic exposure of rupatadine.

            You may still use Ketoconazole cream, since it has been given for topical application only.

            Fexofenadine is to control itch. Since you are already taking a medication for Cholinergic urticaria, this is not required.

          A lot of itch there may indicate a fungal infection. Keep the region down there clean and dry. Wash it more often, say 2 to 3 times a day, with plain water. No soap to be used. Keep the region dry. Wear loose cotton underwears to help dry feel.

          If it doesn’t improve, you may need to use an anti fungal powder over it. Ketoconazole powder may be used twice daily.

          Let us know if it responds.

    Hello doctor, two months ago I had noticed a red pimple at the right side of the beginning of my scrotum sack nearly to the base of my penis, which was quite small. Now it still exists and it has gotten bigger over time. Approximately five months ago I had also noticed a similar red pimple on the left side of my scrotum area which was getting bigger and bigger( for about a month or so) but it suddenly disappeared.Now the same thing happens with the other pimple. It is important to mention that now the ”old”one is still noticeable but its tiny in size (that it can barely be observed). The same happened when the old one was big enough to be observed. The one that is big now existed but it was tiny! Is this some kind of cyst? Should i be worried or is this normal? Thanks!

      May be a cyst, that gets infected and grows in size. However, if you are sexually active, better get it examined.

      Appears to be an allergic reaction to some garments there, or may be you tried a new soap, powder. Start taking an anti histaminic pill twice daily. Let us know if it improves.

      If it doesn’t improve, you need to get yourself examined urgently by a dermatologist. It could be some skin condition like, scabies.

      You have to avoid scratching those bumps to avoid infecting them.

            The bumps that were on the penis are flattened, but those which are on scrotum is still the same. No observable swelling. I want to mention one thing, when the first bump came I tried to pop it like a pimple. But only blood came out of it. Now almost all the bumps have tiny black point on them.

            This looks either like a fungal infection or scabies. Your doctor is treating the condition as fungal. This may be scabies! Can you make the next visit to his clinic a bit early?

            Its not possible to visit early. My father and mother also having same itching problem. They told it to a medical store ( didn’t show any doctor). The store owner gave a lotion “scaboma lotion” for them. It’s written on the lotion that it can treat scabies. Can I use that? Thanks again in advance.

    Ok so recently I first noticed this almost wart like bump on my right side of my scrotum (the sack itself), at first I thought it was a bump, but it sorta looked like a vein, it got pretty big for like 3 days and then went away as I went to the ER, but I went to the ER because my entire sack started to swell up which caused me to panic. So pretty much it started with the bump and then went away (I didn’t get to see if it was connected or moveable) then my entire sack swell and one testicle seemed to have enlarged (pretty firm too) and my other testicle started to shrink it seems. My swollen testicle (right) is painless but the shrinking (left) is where most of the pain is (about a 5/10 painscale). The sack swelling went down after a week, but my testicles (both) are big and small still (being almost 4 weeks now). Pain comes and goes. First doctor (ER visit) told me epipidymitis and mentioned hydrocele and not to worry (prescribed me levaquin) and second doctor (not a urologist) said possibly varicocele and didn’t prescribe me anything (I don’t have insurance btw). After the first week as the levaquin ended I had a painful stool passing and had a decent wipe of blood (bright red) and abdominal pains in left side. Blood stopped after first time, thought it could have been side effect of levaquin, but the pain has gone down mostly in that area (still seeing some red scabbish looking stuff not a lot at all but still there every time I poop, could be from healing though), still having the testicle issues though. Iv been quite stressed from all this so I’m sure some of that could come into effect somehow. Just trying to figure out why my testes are still very uncomfortable and experiencing this, granted still not as bad as the first week but still not completely gone. On week 4 now, and wondering if this will ever go away or if it’s something worse.

        ER visit was like 3 days into it and exactly as the sack started to swell and did a ultra sound and ruled out torsion btw and did blood test and urine. Said white blood cell count was high (because I was taking anti inflamants before hand) and also did a q tip swab down pee hole. They never called me back which implies they found nothing, yet symptoms still present after taking levaquin. Dont have the money to keep wasting doctors time, especially if they are just gonna tell me not to worry, but it’s effecting my ability to work so I really need answers. Again thanks

          Difficult to comment without actual examination.

          From the description, looks like an epipidymitis. There may be a hydrocele, accompanying it.

          Such condition may take time to regress and become normal. There appears nothing to worry.

    Oh and a lot of gas to btw accompanied since the stool stuff started and still going on even though a lot of the pain, not all of the pain is gone from that area. I also pee like every 30 minutes or so. Haven’t had sex or anything in like 3 years now. Pain usually subsides when I’m laying flat on bed. Sorry for all these posts. You more then likely won’t have a real answer for me either but I figured I’d give it a try and see what you might think.

      Very likely to be epipidymitis, that’s troubling you. You are already on antibiotic treatment. May be the treatment needs to be prolonged. This commonly happens, as infections trapped in the sac are quite resistant.

    I’ve been off treatment now for 2 weeks probably. Was on it for 10 days. Do you recommend I get back on it or should I just try to wait it out a little longer? Also do you know what usually causes these sort of things? I’d really like to avoid this again if it all possible. Also are my testes gonna stay in these different sizes or will they eventually go back to normal? My job has been asking me when I will be back and I keep telling them I don’t know cause my job is quite strenuous and kinda takes a lot of my testes in this condition. Thanks again!

      Whether or not to be on antibiotics cannot be said without a physical exam.

      You need to get it checked. Also, in cases of big sized hydrocele, a small surgical drainage of excess fluid is done to relive the patient.

      Likely to be sebaceous cysts. These may occur in young adulthood due to over active glands.

      Keep the area clean and dry. Avoid fried fatty food stuff. Consume more of fresh fruits and vegetables. Drink plenty of water. Wear loose cotton underwears to keep the area airy.

      The lumps are likely to subside with time, as you change your lifestyle.

      Where exactly is it located.

      Try warm compress over it. This may help in draining out the pus filled inside it. You need to be gentle. Do not force drainage.

      Start using an antibiotic cream over and around the cyst, thrice daily. Mupirocin/ bacitracin may be used. If it doesn’t improve, you may need to see a doctor for drainage.

    Hi; I’m 32, I’ve had small white bumps on my balls and underside of penis since I was 14. Never any pain and 3 long term girlfriends and now a wife for 7 years. Tested; as have all of my past relationships have been through the years. Never whatsoever any diagnosis of a VD.

      These lumps need to be examined by a doctor for diagnosis. Though, they are likely to be cysts, examination is essential as you are sexually active.

      Is it spreading?

      This may be contact dermatitis like condition. Caused by wearing condoms without lubrication, some new soap or cream you used over the area.

      You may try 1% hydrocortisone cream over the affected area once daily for a week. Be cautious not to over use the cream.

        No it’s not spreading it’s just around the base of the penis cap and slightly on the head and itches every once in a while besides that and slight pain during urination once in awhile though maybe the condoms might actually be responsible what do u think Doc??

      Since it has leaked out oozing pus and blood, it’s likely to be something that’s infected. May be a cyst.

      Better to fight out the infection completely. You may need antibiotics.

      A small soft bump that disappearing on pressing could be a tiny pimple/ clogged sebaceous gland. Appears to be nothing serious.

      You may leave it as such and wait for some time. It’s likely to regress on its own. Avoid squeezing it. Avoid friction over it.

      If it grows in size or becomes red or painful, re assessment is required.

          Some warts may present like this. They may be caused by some viral infection of the skin.

          Such lesions are treatable with appropriate ointments. You need to see a dermatologist for exact diagnosis.

    I have a lump with something (mostly likely fluid) the size of a pea between my inner thy and my ballsack. It is also preturding half way. It hurts to squeeze it from certain angles but others are painless. I’ve tried to poke it with a pin and some puss and liquid came out. A day later I was squeezing it and more clear liquid came out. It’s irritating is there anyway to reduced the pain or inflammation when it is squeezed? It get inflamed when squeezed. Thanks.

    I’ve had this bump on my scrotum for a little while and I never thought to check on it. It sorta comes and goes and never really causes pain unless I tried to pop it (which failed every time). Then today I was walking and felt a slight sting. I got home and looked and saw that it had finally popped. I tried to squeeze what looked like blood and thick pus out but it stayed right there. After further looking I think it may be a part of something in there but i am definitely not qualified to know these things. I will get it looked at professionally asap but I would like to know what is going on. It’s kinda scary.

        Appears to be a blocked gland, that got infected at some point and oozed out. Nothing needs to be done. Just keep the area clean. Avoid friction over the lesion.

        Skin over the scrotum is generously supplied with oil secreting glands to keep it soft, supple and stretchable. Oil glands have ducts draining out over the skin.

        Any of these ducts may get blocked by plugs of dead skin and dirt. A blocked duct results in gradual swelling of the oil gland. A small bump filled with whitish material soon gets formed.

    I got this painless pimple like on my scrotum skin. When I’m taking a bath sometimes its gotten big for unknown reason and when I try to squeeze it just to make it small again coz’ its kinda disturbing, it has dried pus or zit in it and their exit hole is like a hair follicle’s hole always on the same spot.       Since it’s not attached to the testicle below, this may be a cyst. Nothing needs to be done for this.

      As for itching, what medications are you using? Do you have rashes over the scrotal skin? Is it red or swollen?

          This one is very likely to be a cyst. Most of such lesions may be left as such. They regress on their own with time.

          Intervention may be needed if it becomes painful, grows in size or becomes hard and irregular.

    I have had this tiny pea sized sore on my scrotum for at least 4 or 5 years and it has gotten ever so slightly bigger each year. It fills with dried white puss and then i wind up squeezing/pulling the puss out and the sore remains open. It only hurts after i pull the puss out cause it is red, tender and exposed.         Thank you! Based on my research, all sebaceous cyst cases I’ve read about are all enclosed and ‘pimple-like’ and a lump. Mine is always open with the dry puss substance sticking out. I never really have to ‘pop’ it and it is not a lump after pulling out the substance. I simply pull the exposed puss with my fingers and it comes out leaving an exposed empty ‘crater’. Just wondering if that after reading this description you still feel it could be a sebaceous cyst? I just can’t find any images similar to mine to show that.

    Sorry for leaving a response here but yesterday I got up to go to the bathroom like I do every day before a self exam. I did an examination and everything was fine with my testies. They were smooth as a babies butt. lol

    But a couple hours later on the same day I went back to go to the bathroom when I had an itch. But this time when I went to scratch I noticed near the middle of my left testicle I found a small lump like what was described above. It’s not big, a little hard, and has actually shrunk not by much but it’s about half the size of a pea. I’ve deduced it’s attached to the underside of my scrotum Sack as I can move it around and it’s not connected to the Testie I’ve also noticed today that it’s skin colored and basically if I didn’t know it was there I wouldn’t think it is.

    I’m 23 and this has been on my mind all day, I’d go see my family doctor bout it but he’s been really busy out of late and just want a second opinion. ^^’

      This one has got infected. May be your fingers instilled the infection while you squeezed it.

      For now, keep the area clean. Try warm compress over it to soften it, and help drainage, if any. Also, you need to start applying an antibiotic ointment (mupirocin/ bacitarcin) over it, twice daily.

    I have several small pea sized white coloured lumps on my scrotum region they all appeared over a period of two days i also had a small pimple or something resembling it on penis tip but it went away.
    I checked the lumps and they are not attached to my testicles they seem like they are presnet on the skin but they don’t move when i try to move them on the skin and they are not soft but are rather hard. At first there were 2 of these lumps then in a matter of two days they grew in number and are now 7 of them.
    Since then i have been taking antifungal medicines and antiallergic medicine as i am allergic and often get allergies but there hasn’t been any improvement yet but they have stopped growing in number.
    I also had a rash on my inner thigh but it went away as well after i started medicine
    What can these lumps be and what should i do? I can’t ask my parents to go to the doctor as I don’t want to tell them about this

      You may start using an anti fungal cream over all those lesions before going to bed an night. ANy cream, like Ketonext/ Lamisil can be used. Keep the area clean by daily washing with ample water. Wear loose cotton lowers to let the area be well ventilated.

      Avoid fatty oily stuff in your diet. Consume more of seasonal fruits and vegetables.

      Let us know if it improves.

      It may be left as such, if it’s not bothering you. Or else, needs to be excised out surgically, under all asepsis.

      This is very likely to be a fungal infestation. Try applying an anti fungal ointment or powder (lotrimin powder/ canesten cream) over the affected area, thrice daily.

      Avoid friction over the affected part. Also, you’ll need to keep it clean and dry. Wash it daily with ample water, no soaps required. Dab dry and wear lose cotton undergarments to let it be dry throughout the day.

      You’ll need to wash it again after any strenuous exercise sessions, such as after being to a gym etc.

      They sound more like pimples, and not an infection or any other skin problem.

      You may keep the area clean and avoid wearing tight lowers. Any antibacterial cream, like erythromycin, may be applied to these bumps to limit their growth.

      Eating healthy would help. Avoid all junk food, fatty food stuff. Include plenty of vegetables and fruits in your daily diet. Drink plenty of water throughout the day.

    I am diagnosed with sebaceous cyst on scrotum on November 5th. When i am walking, my thigh frequently rubs against with the cyst. The cyst is inflamed and swollen since November 17th until now. I visited a GP and I am on medicine now. May i know can the cyst be removed permanently including the sac wall? The cyst is small, 0.5 Centimetre X 0.3 Cm.       What medicines are you taking?

      If your cyst is painful, it’s likely to be infected and you need to take antibiotics to treat it. Your doctor may even need to drain it surgically.

      Once it comes out from the infected state, it’s unlikely to bother you in any way. However, it may be removed by a surgeon, if you wish.

My problem is, I am worried the infection will recur anytime because the cyst is located at the lower side of my scrotum. When I walk, the cyst rubs against my thigh.     I just descovred a bump that seems to be on the inside. i took a look and gave it a little squeeze and saw some PINK puss. Should go to the er. whats happening?

      Since this one is a solitary lesion, it’s likely to be an infected cyst. You’ll need to drain it gently by repeated warm compress. Also, start applying any antibiotic ointment over it to prevent spread of infection.

    Hello, about a month ago I found a small hard ball (like a bead) inside my scrotum beneath my left testicle, it does not appear to be attached to the testicle (I feel like I can pull it away from the testicle) but it remains on the left side below my testicle. It moves around either side of my testicle when I try to grab it to examine. It has not changed in size, I feel no pain, I haven’t noticed any changes. What could this be?

          A sebaceous cyst may be left alone. Nothing needs to be done, unless it gives you pain or grows in size.

          Both these cyst occur over beneath the skin surface. While an epidermoid cyst contains trapped protein (keratin ) in it, a sebaceous cyst has sebum inside.

    Dear Doctor,
    For almost a year now I’ve had a pale yellowish ball-like bump on my scrotum. I doesn’t seem to have liquid in it but I cant tell for sure. It is about 3 mm in size. I doesn’t hurt too much if I squeeze it, only a little more than if I squeezed normal skin. I think it might have grown a tiny bit since a year ago but not much action otherwise. I’m a virgin so it can’t be STDs. What is it and what do I do? Will it go away on its own?

        This could be a cyst over your scrotal skin. Likely to go away on its own with time.

    I have 2 pea sized whitish lumps on my scrotum, they have no pain unless I try squeezing or moving them a bit. I’ve been worried if this could be cancer or is it just a cyst? Would I be able to go straight to a urologist to have it checked out? Or would my gp need to check first?

      Likely to be cysts.

      Needs to be examined for diagnosis. You may need to go to your GP first.

    I had what have felt like a small popped or clogged vein inside my scrotum within 3 days it grew. Now it feels like a varicocele, however when I try to feel up and down it, right near where the shaft of the penis comes out from the body on the left side I can feel an enlarged hmm vein I’m guessing?and it hurts a bit there. However the mass has no pain at all. I doubt it could be a hernia since when I do the cough or tense test which creates pressure in the abdomen nothing moves in the scrotum. Will most likely get it checked next week. Any ideas what it could possibly be? Maybe a blocked vas deference?

      Diagnosis requires a physical exam. However, from your description, it sounds like a varicocele.

    It seems that I have the bumps that you labeled as normal hair follicle bumps in the first section of this post. I was wondering if that means that all guys have these bumps as well? If not, how common are they? And, when I search google, I can’t find any information about them even when you claimed that they were the most common hair bumps that guys worry about. Can you provide me with a link or a keyword in google so I can find out more information about them?

    I have these bumps all over my balls my dad says its just hairs but its really itchy at points and im worried and have not gone to a doctor about it but im really scared about it can you tell me what it is and my upper thighs next to my genitals have itched and i would say iv had this for 1to 3or4 months but please help

      Since it’s all over the area, unlikely to be a disease. Most disease would start as a localized lesion.

      Though exact diagnosis would require a direct physical examination,as appears form your description, it appears to be just excess secretions which are common in youth.

      Hormonal surges in early adulthood may cause increased secretions, both in the form of sweat and sebum. These need to be washed off with cool water at regular intervals, lest they may start giving you itch and red rashes.

      Bathing daily with a mild soap is recommended. Using excess soap over the area would render it dry, further making it itchy.

      You may also need to wash the area with plain water after every exercise session, walks or whenever you feel sweaty there.

    I have a minuscule bump On my scrotum with 0 pain and not itchy. The Bump has no califlower characteristics to it and Is the only one there. I don’t think it is a wart but I am not sure. I am stressed out over it. I wish I could attach an image to show. Thank you for your time

      Since it’s solitary and symptomless, less likely to be due to any disease process. Likely to be a pimple, which may gradually regress on it’s own.

    I hope you are still answering to this old thread.

    Ive had a big-ish (pea- bb- sized) white bump on my scrotum thats hard as a rock for maybe a year now. Has not grown. No itching, redness, pain.

    Ive noticed that after hot showers it seems to dissappear briefly and then later (in approx 30min) come back. Sometimes after masturbation it is whiter than usual.

    Tried squeezing it but nothing comes out and it turns red afterwards for a solid 1-5minutes. I’ve also tried “popping” it last year with a sterilized needle but only some blood came out. I dont think i pushed hard enough as it hurted alot.

      Could be a sebaceous cyst, one of your normal sebaceous glands have enlarged due to blockage of its drainage duct.

      It could be left as such, if it doesn’t bother you. However, the bigger ones any require surgical drainage under aseptic conditions.
